Overview

What You'll Do

* Create strong measurement and feedback loops using adoption analytics, user feedback, and outcome-based metrics to continuously improve training, user experience, and measurable productivity impact.
* Lead Copilot and Copilot Studio governance, defining features and capabilities are available, determining access rights and conditions and managing permissions and guardrails in partnership with Security and Compliance teams.

What You'll Bring to the Table
* Bachelor's degree in Information Technology or a related field (or equivalent experience) required; Microsoft certifications in M365/Power Platform/security are a plus.
* 8+ years in Modern Workplace / Microsoft 365 leadership with demonstrated success driving enterprise enablement, adoption, and change management for new capabilities.

GENERAL DUTIES/RESPONSIBILITIES
1. Conducts telephonic outreach to assigned members to assess health, environment, nutrition, and psycho-social areas of concerns using a variety of assessments.
a. In response to assessments, coaches and problem solves with member to identify and address specific goal(s) to support health and behavior change.
b. Provides appropriate interventions to optimize health and well-being. Interventions may include education, the coordination of community-based support services, and other resources.
c. Charts member's treatments and progress in accordance with state regulations and department procedures.
d. Makes referrals to case manager, as appropriate, and/or refers member's family to community support services and resources.
2. Provides home assessment to high-risk members and develop an individual care plan
3. Collaborates with physicians in screening and evaluating members for psychotropic medications.
4. To better serve members and implement the model of care, understands the clinical program design, program monitoring and reporting.
5. Practices as an interdependent member of the health team and provides important components of primary health care through direct social work services, consultation, collaboration, referral, teaching, and advocacy.
6. Assess' and treats outpatients in individual and family modalities exercising mature professional judgment and using a wide range of social work skills to include individual and family counseling to assist patients and their families in dealing with chronic and acute diseases/injuries.
7. Conducts psychosocial assessments to determine patient needs and resources (both family support and community support). Provides counseling to patient and family in matters directly related to patients' limitation, adjustment to medical condition, and ongoing treatment. Develops and implements discharge plans, follow-up care, and transfers to other health care facilities (e.g., nursing homes, rehabilitation hospitals, etc.)
8. Provides consultation services to medical, nursing, and ancillary hospital staff regarding psychosocial issues, discharge plans, and follow-up care for patients and families.
9. Provides crisis intervention services.
10. Responds independently, and with various media, to appropriate community requests. Take the initiative in seeking out opportunities to present programs to meet the needs of patients/members and their families.
11. Consults with Hospital administration, and Plan supplying information and feedback regarding procedures and services provided by the Psychology Division.
12. Develops and maintains working relationships with community resources. In this role, the LCSW/LICSW will provide and oversee the provision of psychiatric, psychosocial, and overall mental health services and referrals for military Special Operators into the Home Base program. This position will collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.

Essential Functions
-Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.

Job Duties, these are considered essential to the successful performance of this position:
Use multiple data sources (EPIC, Care Everywhere, hospital EHR systems, Clinisync, and payer provided patient information) to identify patients who have had inpatient and/or hospital or emergency department visits.
Initiate and document timely outreach calls which are in alignment with TCM billing criteria. Connect patients to transitional care services with a primary care provider, OB/GYN, or behavioral health provide with in specified time frame depending on admission type.
Provide basic education to postpartum patients such as: home visiting programs, contraception, safe spacing, safe sleeping.
Provide basic education on appropriate ER use- When to go to ER, urgent care, or PCP.
Review hospital records and discuss with patient post-discharge needs like durable medical equipment, medications, home healthcare, follow up care with specialists, and SDOH needs. Complete SDOH screenings with patients.
Refer patients to internal providers/resources as needed such as PCP, behavioral health, in-house pharmacy, CHW's, diabetes group, HTN group, etc.
Record appointment dates and other relevant information in tracking spreadsheet. Tracking attendance of visits and intervening to improve visit attendance. Flagging charts of patients who are high utilizers.
Consult with nursing or other medical staff as needed if pt has urgent medical questions or concerns outside scope of practice.
Track billing codes used for follow-up visits. Collaborate with OSU billing dept as needed to notify of incorrect billing/billing codes and/or to clarify if a visit is eligible to be billed as TCM.
Assist in providing education to providers surrounding criteria for billing TCM visits. This may include presenting at provider meetings, sending EPIC messages, or discussing face to face.
Social workers who accept this position would be expected to assist clinic staff and patients in managing mental health crises or social crises (ex: domestic violence) as needed. This could include, but is not limited to initiating wellness checks, assessing for safety/safety planning, mandated reporting (CPS/APS), and de-escalation.
Social workers who accept this position may be expected to provide brief psychosocial intervention/support to patients experiencing grief and/or fetal/infant loss.
