* Assist the resident with personal care which may include the following: shampooing of hair, assist with showering/ bathing, fingernail and foot care, and medication administration.

Wage Range:
RN: $34.23 - $40.75 / hour depending on experience
LPN: $28.41 - $36.29 / hour depending on experience
Location: 310 Lake Blvd S, Buffalo, MN 55313
Nurse Responsibilities:
* Displays a courteous attitude and respect for all residents, families, and staff.
* Works to coordinate, oversee, and provide resident-related care.
* Administers medications and/or treatments in accordance with physician's orders and monitors for effectiveness/response.
* Manages medications, supplies, and equipment including ordering, receiving, storing, and disposing of all items in accordance with policies and procedures.
* Documents care using computerized medication administration and charting software.
* Notifies Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, and takes follow up action as necessary.
* Monitors nursing personnel on duty during the assigned shift.
Nurse Qualifications:
* Must be in good standing with the Minnesota Board of Nursing and have current MN Nursing license.
* Current BLS certification required.
* Must have strong communication and organizational skills.
* Self-starter with a desire to be a part of a team.

Trained Medication Aide (TMA) responsibilities:
* Prepare and administer medications in accordance with Elim Wellspring policies.
* Review and follow a written care plan.
* Measure and record vital signs including pulse, temperature, and blood pressure on new computerized software.
* Assist with various resident cares such as answering light calls, feeding and helping residents to the bathroom.
* Observe resident behavior and report any changes in condition or behavior to supervisor.
* Perform other duties as requested.
Trained Medication Aide (TMA) skills and requirements:
* Must hold current, valid CNA and TMA certification in the state of Minnesota.
* Must have 1,000 hours as a nursing assistant.
* Strong communication skills to interact with residents and staff.
* Team player who has a passion for elder care and adapts quickly to changing situations.

Physical Therapist Responsibilities:
* Assess & Diagnose: Evaluate patients' physical conditions, movement abilities, and medical history to develop personalized treatment plans.
* Develop & Implement Treatment Plans: Create and execute rehabilitation programs to improve the functional deficits of the patient.
* Provide Hands-on Therapy: Perform manual therapy, therapeutic exercises, and use modalities such as but not limited to ultrasound and electrical stimulation as needed.
* Educate & Support Patients: Provide patient, caregiver, staff education as deemed clinically necessary such as but not limited to: pain management techniques and fall risk safety. Teach patients and families about exercises, injury prevention, and pain management techniques.
* Monitor Progress & Adjust Plans: Track patient outcomes, document progress, and modify treatment plans as needed.

Physical Therapist Qualifications:
* Must be a graduate from an Accredited Physical Therapy Program and have a bachelor's degree
* Must possess a current accredited MN Physical Therapist License
* Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care.
* Must be capable of performing the essential functions of the job, with or without reasonable accommodations.
* Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.
