Your Job as a Substance Use Counselor:
The Substance Use Counselor is instrumental in our patient's treatment and recovery from opioid use disorder. Substance Use Counselors provide high quality, compassionate care through various mediums which include both individual and group counseling sessions.
Job Responsibilities:
Provide high quality, compassionate guidance in both individual and group counseling sessions.
Plan, oversee, facilitate and document patient's recovery.
Co-facilitate assigned group or family sessions as needed.
Ensure all documentation regarding patient care, treatment, and incidents is completed timely and in a clear, concise manner.
Prepare individual treatment plans for each assigned patient.
Initial assessments as well as follow up assessments.
Evaluate patient needs and determine if referrals to other programs or facilities are needed.
May plan for aftercare for assigned patients.
Provide crisis intervention to patients, as needed.
Provide case management duties for patients, ensuring individualized quality care as needed.
Act as a liaison between referral sources and patients, as needed.

Required Education, Skills and Qualifications:
Minimum of an Associate's Degree in social or health services field required;
Prefer Bachelor's or Master's degree in social or health services field;
Degree must be from an accredited college or university.
Previous experience in addiction recovery or behavioral health settings, such as outpatient, residential, or correctional facilities.
* Familiarity with Medication-Assisted Treatment (MAT), including methadone, buprenorphine, and naltrexone.
* Experience conducting individual and group counseling sessions focused on substance use recovery.
* Knowledge of evidence-based practices, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and relapse prevention techniques.
* Documentation experience, including maintaining clinical notes, treatment plans, and progress updates in accordance with state and federal guidelines.
* Familiarity with state regulatory standards (e.g., 42 CFR Part 2, HIPAA, Joint Commission standards).
* Experience collaborating in a multidisciplinary team with medical staff, case managers, and peer support specialists.

What You'll Do

EXECUTIVE SUMMARY:
The Special Education Behavior Interventionist will be deployed and assigned as an extension of the tier 3 Social Emotional Behavioral Health (SEBH) Support team. This team member will receive ongoing training, coaching, and regular direction from SEBH Lead Support Teachers and tier 3 SEBH Coordinator(s). The Special Education Behavior Interventionist will coach school team members on using evidence-based instructional and behavioral strategies within the school setting. This position will build capacity through coaching and modeling to meet the academic, behavioral, and social-emotional needs of students.
The Special Education Behavior Interventionist will assist in assessing and implementing high-quality behavioral interventions, including modeling in daily practice for continuous improvement of social-emotional instruction, and academic instructional practices impacting classroom management. Overall, the job of the Interventionist is to support the Social Emotional Behavioral Health Support team in building the capacity of the school and its teachers to meet the learning and behavioral needs of all students with a focus on students needing tier 3 SEBH supports. The interventionist's goal is to ensure that school staff acquires the understanding and skills to:
1) Enhance SEBH instructional practices at the classroom level by aligning tier 3 BIPs to universal classroom practices
2) Support the development and maintenance of Tier 3 MTSS (Multi-Tiered Systems of Support) and PBIS systems, and Tier 3 Brief FBAs and BIPs
3) Support, model, and coach the use of BIP fidelity checks and respond to the gathered data to improve implementation fidelity.
PERFORMANCE RESPONSIBILITIES:
Work under the direction of the Special Education Tier 3 Social Emotional Behavioral Health team to implement behavior supports aligned with district and building procedures.
Support in the implementation of and modeling of both Tier 3 behavior plans, as well as behavior intervention plans (BIPs) for students eligible for special education services.
Raise student achievement, graduation rates, and outcomes
Develop tier 3 behavioral interventions and intensive strategies to implement with identified students, including but not limited to:
Provide direct skill instruction
Review, observe, record, and chart behavior information
Utilize the data-driven decision-making process
Communicate with the appropriate staff

Attend team meetings within the building for the purpose of communication and service coordination, when requested
Assist special education teachers and paraeducators by demonstrating strategies, modeling effective behavioral intervention strategies, and assisting with individual and/or team planning and problem-solving.
Promote the structure of the building MTSS, PBIS, and other applicable systems to support student achievement and behavioral success
Assist during a needed crisis, behavior management situations &/or office discipline referrals as directed by the building Administrator, Director of Special Education, or any members of the Special Education Tier 3 Social Emotional Behavioral Health team.
Provide QBS Safety Care training when needed, and support in the modeling, coaching, and problem solving of strategies with building-based staff

1. Client Services
Provide direct case management using the Critical Time Intervention (CTI) model.
Conduct in-reach within hospitals and correctional facilities to engage eligible individuals during the discharge planning process.

Key Responsibilities:
1. Client Services
Provide direct case management using the Critical Time Intervention (CTI) model.
Conduct in-reach within hospitals and correctional facilities to engage eligible individuals during the discharge planning process.
Assess clients for housing instability and determine appropriate diversion or prevention interventions.
Develop individualized housing and service plans that address short- and long-term stability.
Connect clients to housing resources, including, but not limited to, mediation with landlords, family reunification, emergency assistance, and community-based supports.
Assist with securing documentation (ID, income verification, housing history, etc.) necessary for housing placement.
Facilitate warm handoffs to community providers when appropriate to ensure continuity of care.
2. System Coordination & Partnership
Build and maintain collaborative relationships with social workers and reentry staff at healthcare systems and correctional institutions.
Coordinate closely with emergency shelter staff, outreach specialists, behavioral health services, and housing providers.
Participate in case conferencing and coordination meetings with community stakeholders.
3. Documentation & Compliance
Maintain accurate, timely, and confidential documentation of all client interactions and service plans.
Input all client data into the HMIS (Homeless Management Information System) in compliance with grant requirements.
Track program outcomes and complete reports required by funders and supervisors.
4. Professional Development
Participate in ongoing training in CTI, trauma-informed care, motivational interviewing, and other relevant models.
Stay informed about local housing resources, policies, and service system changes.

Qualifications:
REQUIRED:
Bachelor's degree in Social Work, Psychology, Human Services, or related field (equivalent experience may be considered).
Minimum 2 years of experience working with individuals experiencing homelessness, behavioral health needs, or involved in reentry systems.
Knowledge of housing-first and trauma-informed care principles.
Strong understanding of institutional discharge systems and housing instability indicators.
Proficient in case documentation, goal planning, and client engagement techniques.
Valid driver's license and access to a reliable vehicle; ability to travel locally to sites and partner locations
PREFERRED:
Familiarity with the Critical Time Intervention (CTI) model.
Experience working with hospitals, mental health facilities, or corrections.
HMIS or comparable data entry system experience.
