Trains & mentors team staff in IDDT technology and other intervention modalities. Performs psychosocial interventions with client in an individual, family or group forum. Monitors clients response to treatment as prescribed in the client's treatment plan. Assists clients in securing benefits & other community resources. Provides outreach services to CSN clients & counseling services in the community setting such as client homes;Daily travel into the Franklin County community to visit clients in their residence, assist them with appointments, facilitate therapy sessions (individual or group), etc. Must have valid driver's license.Conducts & facilitates individualized service planning with assigned clients.
