A Therapist who excels in this role:
Conducts individual and group therapy sessions to educate patients regarding psychological, emotional, or substance abuse problems
Displays active involvement in treatment planning process
Provides family session counseling to all patients to ensure appropriate communication and involvement of family members and support groups
Actively communicates with clients, families, and outside referral sources
Demonstrates proactive communication with those involved with the patient's treatment

About the Position
We have an exciting opening for a Residential Associate. Residential Associates work as members of a direct care team serving adults with serious mental illness. Their primary responsibilities include supporting the development of life skills and therapeutic interventions, performing housekeeping and meal preparation, administering medication, and keeping accurate clinical documentation. If you have experience working as a Qualified Mental Health Associate (QMHA), Direct Support Professional (DSP), Residential Assistant (RA), Residential Counselor, Skills Trainer, or Caregiving for a client with behavioral health needs, then this position may be a great fit for you!
Vocational Training Program
For applicants starting as a QMHA-R, ColumbiaCare offers a Vocational Training Program to accelerate the training, on-the-job experiential learning, and testing preparation for qualified employees to achieve the 1000 supervised hours required to complete their certification as a QMHA-I. Through this program, this certification can be achieved in as little as 6 months. This is an optional program.

As a Case Manager 2, you will manage a caseload and develop client-centered care plans in collaboration with the client and/ or client's representatives to meet their needs, goals, preferences, and risk factors. Provide initial and ongoing eligibility assessments for services and benefits for a broad spectrum of Federal and State programs including Medicaid programs, nursing facility services, state plan personal care services.
Primary responsibilities include:
Assess client needs and risk status through in-person home visits, phone calls, and connecting them with resources
Develop comprehensive, client-centered long term care plans
Attention to detail regarding paperwork and data entry work
Care Planning and Case Management:
- Develop initial, renewal and updated care plans in collaboration with the client and/or client's representatives that meet their needs, goals, preferences and risk factors. Based on the care setting, identify provider(s) and authorize payment.
- Complete initial functional assessments for new consumers applying for Long Term Care (LTC) benefits and waivered services. Mid-Columbia Center for Living (MCCFL) is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are both trauma-informed and recovery-oriented to persons / consumers with mental illness and addictions. Trauma-informed care includes safety, choice, collaboration, trustworthiness, cultural responsivity, and empowerment for both clients and staff. MCCFL serves Hood River, Wasco, and Sherman Counties and utilizes a consumer-involved, consumer-engaged, and consumer-driven model. MCCFL provides a whole-person care approach to adults and children to address both physical and mental health needs. MCCFL offers a variety of programs including a mobile crisis unit, intensive case management, ongoing counseling, community support, supported employment, jail diversion, hospital discharges, hearings, and more.

What you'll do:
Provide Case Management services to all assigned clients. Services are provided in a variety of community settings, with consideration to client preference.
Assess eligibility of potential clients utilizing agency standards and processes. Case Manager/Parent Coach will work with families who are involved with the ODHS Child Welfare system and/or reside in the Transitional Treatment Recovery Services program. This position will support the Care Management Resource Pool and may be required to support multiple locations system-wide within the Legacy Health hospitals.
Responsibilities
Coordinates and facilitates interdisciplinary provision of comprehensive, patient-centered, quality health care throughout the continuum for patients with acute and chronic health conditions. Fosters achievement of optimal health care outcomes within accepted standards of care. Serves as an expert resource to the healthcare team regarding the continuum of care, efficient use of resources, Best Practice protocols, team-based care, quality indicators and improvements, and regulatory requirements. Ensures a smooth transition of care between multiple health care environments with planned handoffs. Partners with patients and families in identifying health care issues and barriers to self-care in order to set priorities and engage in appropriate interventions. Demonstrates cultural agility and employs health literacy guidelines to provide education regarding self-management strategies. Utilizes rapid quality improvement cycles to continuously monitor, evaluate, measure, and report progress of interventions and outcomes. Paces the case to assure appropriate and fiscally sound care coordination across the continuum.
Qualifications
Education:
Academic degree in nursing (BSN or higher) preferred.
Experience:
This position requires extensive knowledge of disease management to include diagnostics, treatment and prognosis, community resources and healthcare reimbursement. Minimum 2 years clinical nursing experience required. Relevant experience in one or more of the following healthcare areas preferred:
* Coordination of community resources
* Care management of diverse patient populations
* Ambulatory Care
Knowledge of levels of care throughout the health care continuum to include; inpatient, emergency care, rehab, home health, hospice, long term acute care, SNF, ICF, ALF with an overall understanding of utilization management and resource management.
Working knowledge of Care Management models across the continuum.
Knowledge/Skills:
Knowledge of six core components of case management:
* Psychosocial aspects
* Healthcare reimbursement
* Rehabilitation
* Healthcare management and delivery
* Principles of practice i.e. CMS guidelines, Interqual criteria
* Case Management concepts
Excellent organizational skills
Health literate oral and written communication skills for effective interaction with all members of the patient's health care team
Knowledge of transitional planning to and from all venues
Ability to determine and access appropriate community resources
Ability to engage patient/family in discussion of health care goals and decisions with attention to cultural and health literacy implications
Ability to adhere to and implement regulations in an effective manner. Must serve as a resource to all team members regarding regulatory issues.
Keyboard skills and ability to navigate electronic systems applicable to job functions.

Licensure
Licensure/Certification:
Both Oregon and Washington state RN licensure required.
If the employee is not licensed in both states at time of hire, application must be submitted to the licensing agency, in the state the employee is not yet licensed in, before the employee's start date. Licensure in both states is required within 6 months of hire.
AHA BLS for Healthcare Providers required.
Case management certification preferred.
Pay Range
USD $83.60 - USD $83.60 /Hr.

HOW YOU WILL MAKE AN IMPACT:
Provides "hands-on" in-home demonstration, teaching and support to increase family safety and stability, knowledge of child development, and understanding of safety concerns in families referred by DHS/Child Welfare. The Provider will assess the family's needs and co-create a family plan for visitation and action, and collaborate with other DHS providers and community agencies. Ability to travel to families' homes and communities required.
This individual will provide services to families referred through DHS/Self Sufficiency to enhance family stability and support workforce readiness. Provides a comprehensive screening to determine immediate needs. Provides skills training to children, adolescents and adults to address immediate needs. Develops and implements an effective service plan. Collaborates with community partners and connects families to relevant resources to support progress.
Provide a range of services including parent education, communication skills, home management, budgeting, stress reduction, stabilization of home environment, and other appropriate services
Refer client to supportive services to supplement treatment and counseling as appropriate
Help family members overcome barriers and identify strengths
Use Motivational Interviewing, Collaborative Problem Solving, Wraparound, and other effective practices
Identify families with drug and alcohol dependence, physically and sexually abused children, domestic violence and other child safety concerns.
Supervise visits for families with an open child welfare case
Assess and identify family strengths and areas for growth to help resolve child safety and attachment concerns.
Use research-based curricula to teach, coach, and model appropriate play, attachment, and behavior management techniques to referred families, specifically: Circle of Security, Make Parenting a Pleasure, and Nurturing Parenting
Collaborate with referring case workers to support clients' goals and safety plan
Coordinate case activities with Contract Supervisor, ODHS Child Welfare & Self-Sufficiency, schools and other community partners
Report any potentially dangerous situations or safety plan violations
