Job Summary: The Residential Counselor is responsible for providing direct clinical services to individuals who have been affected by chemical dependency, psychiatric problems and/or co-occurring disorders.
Functions as a case manager for patient caseload.
Maintains clinical record including biopsychosocial, SNAP and BAM assessments, treatment plans, clinical summaries, progress notes, case consult notes, discharge summaries, and continuing care plans.
Ensures clinical record is compliant with all licensing, regulatory and accrediting agency requirements.
Completes continued stay reviews using ASAM criteria.
Coordinates services including aftercare planning and makes referral recommendations when necessary.
Maintains regular communication with referral sources, legal systems, and outside providers.
Responds to and addresses unusual or emergency situations.
Encourages patients to understand the benefits of recommended level of care.
Operates as part of multi-disciplinary treatment team.
Assesses healthcare equity for current and future patient needs.
Conducts educational seminars and lectures to patient population in a professional and competent manner.
Facilitates individual, group and family counseling sessions.
Assists with the collection of random urine drug screen samples from patients as needed.
Participates in clinical supervision, department meetings and trainings as required.
Protects the confidentiality of patient information.
Performs other related duties as assigned.
Qualifications
Education and Experience:
Master's degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in human services) or other related field which includes a practicum in a health or human service agency preferably in a drug and alcohol setting.
Bachelor's degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in human services) or other related field and one-year of clinical experience in a health or human service agency, preferably in a drug and alcohol setting.
Must hold a professional license through the PA Department of State or be credentialed through PCB at the time of hire or be actively working towards licensure or certification. A PCB credential must be obtained within two years of hire or licensure within 4 years of hire.
Knowledge of Substance Use Disorder (SUD) treatment and/or ASAM criteria.

What You'll Do

· Remain awake and alert during the entire shift
· Complete hourly phone checks through the automated time and attendance system
· Monitor and attend to residents' needs at any time during the night
· Support morning routines to ensure individuals are awake, groomed, and prepared for their day
· Maintain a home-like, safe, and secure environment, including performing assigned cleaning tasks
· Ensure the home and furnishings remain in good condition; report maintenance needs promptly
· Provide direct support with laundry, hygiene, grooming, cooking, cleaning, and other daily living skills
· Administer medications as required and follow all health, dietary, behavioral, and adaptive equipment plans
· Respond to emergencies and provide crisis intervention when necessary
· Complete all required documentation, including daily logs, communication notes, medical records, and tracking forms
· Know and follow each resident's Individual Habilitation Plan, including implementing goals and documenting progress
· Protect residents' rights, dignity, privacy, and personal choices

Ensure that the Individuals in the bed at a reasonable hour, properly bathed, and readied;
Complete household cleaning as designated by the Direct Support Supervisor;
Record significant shift happenings and observations in the daily log;
Be responsible for group home laundry, and for Individuals laundry as assigned;
Monitor each Individual and attend to Individuals needs throughout the night;
Complete a daily medication count;
Waken the Individuals at an appropriate morning hour;
Train and supervise Individuals around morning outlines;
Review the implantation of the IHP and monitor continuing IHP development;
Administer medication, record the administration of medication in accordance with DDD
guidelines. Assists the Direct Support Supervisor with continuous monitoring of
medication administration procedures;
Requirement to cooperate with the Department of Human Services, Division of
Developmental Disabilities licensee and staff in any inspection or inquiry, investigation;
Assure that each Individual is free from physical or mental abuse;
