WORK SCHEDULE: This hybrid position is 0.75-1.0 FTE (approximately 30-40 hours/week), typically scheduled over 3-4 days per week, with flexibility between outpatient AMC and inpatient withdrawal management settings. Includes one assigned weekend per month for on-site and on-call coverage related to detox admissions and urgent medical needs. Occasional telehealth or extended-hour coverage may be required based on program needs. JOB SUMMARY The Licensed Medical Provider (LMP) delivers comprehensive addiction medicine and primary care services across both inpatient withdrawal management (60%) and outpatient addiction medicine (40%) settings. This hybrid role provides medical evaluation, stabilization, and longitudinal care for patients with substance use disorders and co-occurring health conditions. The provider works as part of an interdisciplinary team including physicians, advanced practice providers, RNs, LPNs, CADCs, QMHPs, and CHWs to ensure high-quality, integrated medical and behavioral healthcare. The position includes scheduled on-site and on-call responsibilities one weekend per month, conducting admission evaluations, issuing orders, medication management, and urgent medical coverage for withdrawal management clients. ESSENTIAL FUNCTIONS Conduct admission histories and physicals for detox and residential patients; complete comprehensive assessments for outpatient AMC patients. Initiate, manage, and adjust treatment plans for substance use disorders, including buprenorphine, methadone coordination, naltrexone, and long-acting injectables under REMS/DEA guidelines. Provide daily and as-needed medical oversight for patients in withdrawal management and early stabilization. Manage acute and chronic medical conditions, including Hepatitis C, contraception counseling, STIs, wound care, and common primary care concerns. Order, interpret, and review laboratory results, diagnostic studies, and urine toxicology screens; utilize the Oregon PDMP as clinically indicated. Formulate and document individualized, evidence-based care plans with patient participation and informed consent. Provide education and counseling on recovery principles, harm reduction, and medication-assisted treatment options. Support and consult with nursing and counseling staff regarding medication management and medical conditions in both inpatient and outpatient settings. Administer or oversee the administration of subcutaneous and intramuscular medications following clean technique and best practices. Coordinate care transitions between detox, residential, and outpatient settings, ensuring safe discharge and follow-up. Maintain accurate, timely documentation in the EHR, including diagnoses, levels of service, and procedure coding. Participate in the monthly on-call rotation (approximately one weekend per month, additional shifts available if desired) to provide withdrawal management coverage, including telephonic support and urgent medical consultation. For individuals in recovery, at least two (2) years of continuous sobriety prior to hire. Licensure & Certification: Current Oregon license to practice medicine. Current BLS/CPR certification. DEA registration and authority to prescribe controlled substances in Oregon. Preferred Qualifications: Experience in both outpatient and residential treatment settings. Bilingual English/Spanish preferred. Familiarity with Epic or similar EHR systems. COMPETENCIES Proficiency in motivational interviewing and trauma-informed communication. POSITION OVERVIEW: The SCMG Cardiology Advanced Practice Provider is responsible for providing Cardiology services in the inpatient and outpatient setting. Provides both inpatient and outpatient care that includes daily rounding, orders, post-op instructions and discharge summaries, clinic consultations and post-operative office visits. The Cardiology team will work in collaboration with other medical specialties to assess and manage patients following established standards and practices. Work schedule: Provider will work a minimum of 40 hours per week with a minimum of 36 hours patient contact time Duties include a blend of hospital rounding, admitting, discharges and scheduled clinic visits. This includes clinical documentation and all other necessary duties. Each week consists of a combination of: Inpatient Service Rotation: Monday- Sunday hospital service rotation, split between the APP team that are working in the inpatient and outpatient setting (current rotation 1:6), No clinic responsibilities the week following inpatient service. This is a day shift only, no night coverage. Hospital rounding, admissions, discharges, consults with Cardiologist Coordinates procedural care for patients within the Cardiology service Patient Education and coordination of outpatient follow up at discharge Clinic day: Bend outpatient clinic: clinic hours between 8am - 4:30 pm, with a minimum of 12 appointment slots daily. Outreach outpatient clinic: Outreach clinic performed at the agreed upon interval based on clinical need in assigned outreach area. Outreach clinic start adjusted depending on distance from Bend campus. Four clinical days per week Call expectation: Cardiology APP has no call responsibility outside of work hours. Work location: Physician's inpatient work location is St. Charles Bend Hospital with clinic in the Bend Heart & Lung Center and agreed upon Heart & Lung Specialist outreach clinic. ESSENTIAL FUNCTIONS AND DUTIES: (May perform additional duties of similar complexity within SCHS as required or assigned) Clinical Expectations: Examines, diagnoses, and provides a broad scope of treatments for cardiac disease processes Performs patient examinations, obtains and records pertinent medical history, subjective, and objective findings. Orders and interprets appropriate laboratory, imaging, and other diagnostic procedures. Utilize appropriate risk calculators as a part of assessment for patients and is familiar with new studies, emerging technology and medications beneficial to the patients being treated. Interprets and integrates data to determine diagnosis, and therapeutic plan. Administers and prescribes medications in compliance with state law and practice guidelines. Orders appropriate medical procedures in accordance with State law and practice guidelines. Instructs patient and family regarding medications and treatments. Educates patients regarding health, illness prevention, and healing. Recommends community resources to meet patient and family needs. Maintains and reviews patient records, charts, and other pertinent information, documenting clearly and concisely. Triages patient telephone calls, providing consultation as needed. Communicates effectively with colleagues, patients, physicians, and administration. Maintains quality of clinical practice by selecting appropriate avenues for professional growth. May perform additional duties of similar complexity within the organization, as required or assigned. Administrative Expectations: Document all patient care within the Electronic Medical Record (EMR) and complete all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing. Be familiar with the SCHS organization structure. Participate in SCMG physicians meetings and assist in policy formation for SCMG and Physician's practice. Keep medical knowledge and skills current through regular CME activity. Must attest to having adequate (CME) to meet maintenance of board certification and compliance with MSO rules and continuing privileges without restriction. Keep all licenses and appropriate certifications current and unrestricted. Maintain active privileges at St. Charles Health System hospitals sufficient to permit performance of job duties. Complies with all applicable laws, regulations, policies and procedures, supporting the health system's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all health system's investigations and proceedings. EDUCATION Required: Successful completion of an approved Nurse Practitioner or Physician Assistant course of study and certification as appropriate to the incumbent's license. Complete annual fire/safety, etc. education in-services as required. Attend mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies. Complete annual CME as required to maintain license and appropriate Board certification. Complete in a timely manner assigned Computer Based Learning modules. LICENSURE/CERTIFICATION/REGISTRATION Required: Unrestricted Physician Assistant and/or Nurse Practitioner licensure in the state of Oregon. Physician Assistant requires NCCPA certification. Nurse Practitioner requires AANC, NCC or AANP, AMCB certification. If Physician Assistant, ability to establish and maintain a Practice Agreement with a Supervising Physician through OMB. Unrestricted Federal DEA number registered in Oregon. Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs. Valid driver's license and the ability to meet SCHS driving requirements. Current ACLS certification required. Additional certifications may be required based on privileges granted to provider by Medical Staff Services at SCHS Inpatient locations. EXPERIENCE Required: Experienced in hospital and outpatient practice environments providing care. Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System. Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results. Preferred: Experienced in Cardiology care PERSONAL PROTECTIVE EQUIPMENT Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
