Position Summary:

Essential Job Functions:
Deliver life-changing mental health services to clients in the community
Empower clients through behavioral interventions, utilizing techniques like motivational interviewing and cognitive processing
Foster recovery and resilience by promoting improved functioning and meeting unique client needs
Manage a rewarding caseload and collaborate with supervisory staff to ensure a fulfilling workload
Respond to emergencies with confidence, using sound judgment and discretion to provide timely assistance during mental health crises
Ensure continuity of care by providing comprehensive follow-up services to clients
Disease Management Responsibilities:
Review and prioritize outreach efforts to individuals identified through the Disease Management Cohort List
Engage individuals using motivational interviewing techniques and a person-centered approach
Coordinate with the intake department to ensure timely admission into clinically appropriate programs
Complete required documentation for the Disease Management program, including reviews and updates to cohort lists and state reporting
Collaborate with the Healthcare Home (HCH) Department to ensure all Disease Management individuals are enrolled in HCH
Knowledge, Skills, and Abilities:
Knowledge of case management and rehabilitation methods, principles and techniques as they relate to mental health and/or substance use disorders.
Knowledge of the problems encountered by persons with mental health and/or substance use disorders and how to access the various community resources available.
Knowledge of the methods and modalities as well as the legal requirements and court procedures related to treating mental health and substance use disorders.
Ability to problem solve and communicate ideas clearly and effectively, verbally and in writing to produce clear and concise reports.
Ability to be self-motivated to provide high-level performance while meeting the complex needs of the clients and their families.
Ability to multi-task and coordinate services from a variety of community resources, while maintaining proper communications with all staff (e.g. admissions, process, and discharge plans).
Ability to establish effective relationships with youth and their families via telephone and personal contacts.
Ability to follow direction and accept supervision.
Experience and Education Qualifications:
An individual with a Bachelor's degree in Psychology, Social Work, or other human services specialized field of study; or
An individual with a Bachelor's degree in an unrelated field and two years of related work experience; or
Qualified Addictions Professional (QAP) - CADC, CRADC, CRAADC, CCJP, RADC-P, RADC, CCDP, CCDP-D
Any four-year combination of higher education and two years of related work experience, or
An Associates of Applied Science in Behavioral Health Support will also fulfill educational requirements; or
An individual with four years of qualifying experience*.
*Qualifying experience must include delivery of services to individuals with mental health disorders; substance use disorders or developmental disabilities. Experience must include some combination of the following:
Providing one-on-one or group services with a rehabilitation/habilitation and recovery/resiliency focus;
Teaching and modeling for individuals how to cope and manage psychiatric, developmental, or substance use disorder issues while encouraging the use of natural resources;
Supporting individuals in their efforts to find and maintain employment and/or to function appropriately in family, school, and community settings;

Job Description
We are currently hiring for
Overnight House-Sitter/Pet-Sitters.
**We are especially interested in House-sitters available during our busiest time of year -- winter holiday break -- sometime between 12/23-1/2.
This position requires OVERNIGHT STAYS (sleeping over in a client's home) with the client's animals. Each overnight stay begins from the dinner period (arrival between 5-7pm) and continues until the morning period (departure between 7-9am).
Please note: House-sitters are NOT able to bring guests with them into the client's home during their stay.
House-sitting includes:
walking the dog(s)
feedings
medications if necessary
maintaining each animal's routine.
House-sitting may also include other pet-sitting or vacation care tasks such as bringing in the mail/newspaper, plant care, cleaning up messes (ie: hairballs, accidents), and keeping the home in order while the clients are away.
Fees
The base rate paid to a house-sitter for each night of house-sitting is $32.50 for one dog and goes up from there depending on client surcharges. There are extra fees given to the house-sitter for additional animals in the home, extra visits scheduled during the daytime, and for holidays. (Potential gratuity from clients is not reflected in this rate.)
Scheduling
House-sitting jobs last anywhere from 1 night to 2 weeks. House-sitters typically will take one booking per month. They may take on more bookings depending on their availability and client requests.
House-sitters are independent contractors with Heaven Sent and are offered client "bookings" as clients schedule vacation care with us. House-sitters work on an "as-needed" basis and accept or reject each booking on an individual basis.
Holidays
Our preference is that house-sitters are available for at least 2 out of 3 of our busiest holidays (Thanksgiving, Christmas/New Years, Spring Break). There is an extra surcharge paid to sitters for holiday work.

9. Must be able to assist individual in a wheelchair to transfer to another chair, bed, toilet, or bath.
10. Must be able to assist someone in the shower or bath.
11. Must be able to make multiple trips from car to house carrying purchases such as groceries, etc.
12. Must be able to perform snow removal as much as necessary for individual and staff safety.
