JOB SUMMARY:
The Home Visitor position is responsible for working in a collaborative effort to provide educational and support services to enrolled families who are expectant mothers and/or families with young children, 0 - 5 years in the Kings County area. The Home Visitor's primary responsibility is to plan and implement home-based program activities for families; maintain files and documentation as required and conduct a monthly family socialization event following home visitation curriculum.
DUTIES AND RESPONSIBILITIES:
* Successful completion of the Parents as Teachers (PAT) curriculum course. Certification must be obtained prior to performing home visits.
* Provide monthly home visits to families enrolled lasting a minimum of 60 minutes each using the specified curriculum, i.e., Parents as Teachers, input from parents, and feedback from Kings County Health and Human Services Agency.
* Document and maintain home visit files that include the children's developmental progress, goals, and outcomes including but not limited to screenings, observations, lesson plans, referrals, and communication.
* Utilize each family's home as a learning environment: consider space and materials available for co-planning activities with families.
* Increase the family's knowledge and understanding of the physical, emotional, and intellectual development of their child, and supports parents as they plan activities.
* Together, with parents, complete an on-going assessment.
* Ensure that each child's medical needs are being met by partnering with parents to establish a source of coordinated care that serves as an appropriate medical home.
* Inform, refer, and assist families in utilizing appropriate community services and resources.
* Assist with the planning, coordination and provision of monthly coordinated services and family socialization events for the Kings County families collaborating with the parents and community partners.
* Participate in staff meetings and attend other meetings and trainings as directed.
* Perform other duties as required.
EXPERIENCE/EDUCATION REQUIREMENTS:
Home Visitor I
* High school diploma/GED; AND
* Minimum of 12 units in early childhood education; AND
* Minimum of 2 years of paid or volunteer experience related to community organizing, early childhood education, or related field.
Home Visitor II
* AA Degree in Early Childhood Education or related human service with a minimum of 12 units in early childhood education; AND
* Minimum of 2 years of paid or volunteer experience related to community organizing, early childhood education, or related field.
OTHER REQUIREMENTS:
* Travel and attend out of area meetings and conferences as required per the funding source(s).
* Work variable hours and days, including those outside regular Monday through Friday, 8 a.m. to 5 p.m. business hours.
* Meet and maintain KCAO hiring requirements which include:

POSITION SUMMARY: Provides services in the areas of outreach, assessment, vocational exploration, case management, counseling, development of customers' individual plans, job search assistance and follow-up services; assists in preparation of administrative assignment.
ESSENTIAL RESPONSIBILITIES include the following:

The Registered SUD Counselor working under moderate supervision provides counsel and support to patients suffering from drug or alcohol dependency, educate families in the best ways to help in the recovery process, and work with both inpatient clients and those participating in outpatient programs. Under moderate supervision delivers services to substance abuse clients including client assessments, monitoring client attendance at meetings and classes, individual and group counseling, etc. Must be working toward certification.
How will I contribute
May conduct intake and program orientation for participant including, but not limited to, assessment of psychological and social functioning and the development, implementation, and ongoing review of the participants Individual Treatment Plans in accordance with the Title IX regulations and Program protocol and policies.
Deliver individual and group counseling services, case management services, education and community outreach services, and referrals to appropriate services/community resources as necessary.
Conduct breathalyzer and/or saliva tests on participants as appropriate, documenting results
Prepare participant progress reports, when necessary.
Interact with the criminal justice system and/or referral sources, when necessary.
Monitor and maintain participant files, charts, attendance schedules, face-to-face monitoring forms, group progress notes, completion of vocational/educational and self-help activities, aftercare, scheduled make-up sessions, etc. to ensure compliance with Title IX regulations and Program policies.
Monitor participant payments/fees and assist with fee collections. Conduct financial assessment interview in accordance with Title IX Regulations and Program protocols.
Recommend leaves of absence, dismissal/suspension, reinstatement, completion, etc. for participants in accordance with Title IX and Program policies.
Communicate openly and clearly, on a regular basis, with Clinical Supervisor and support staff to ensure Program effectiveness (appropriate caseload, work hours, scheduling, group capacity, etc.)
Abide by all Kings View policies and procedures, including but not limited to, confidentiality of program participants (Title IX, Title 42, and HIPPA/Privacy).

Alcohol and Drug Registration and actively pursuing Certification, in good standing by an agency approved and recognized by the Department of Health Care Services (DHCS) is required at time of hire.
Associate degree or the equivalent sixty college semester units with emphasis in behavioral or social sciences if required for the position. Three years abstinence if there is a history of substance abuse. Completion of 40 hours per two-year period of related training is required for continued employment for registered/certified counselors.
Must maintain counselor certification as required by State law. Knowledge and compliance with CLAS standards. Familiarity with electronic health record systems (AVATAR). In-depth knowledge of drug and alcohol testing to monitor treatment plans and medical instructions.
Relate to others comfortably and able to establish and maintain effective relationships. Strong verbal, written and interpersonal skills. Integrity to handle sensitive information in a confidential manner.

Felony Disqualification: Existing law provides that persons convicted of a felony are disqualified from employment as peace officers. Such persons are not eligible to compete for, or be appointed to, positions in this class.
Training Requirement: Under the provisions of Penal Code Section 832, successful completion of a training course in laws of arrest, search and seizure, and in firearms and chemical agents is a requirement for permanent status in this class.
Background Investigation: Pursuant to Government Code Section 1031(d), all persons successful in this examination who are not peace officers with the Department of Corrections shall be required to undergo a thorough background investigation prior to appointment.
Work Authorization Requirement: Existing law provides that peace officers be legally authorized to work in the United States under federal law.
Age Limitation: Minimum age for appointment: 21 years, pursuant to Government Code Section 1031(b).

Position Summary:
The Case Management Assistant (CMA) provides administrative and care coordination support to the Population Health Management (PHM) team, including Nurse Case Managers and Social Services staff. Job Duties and Responsibilities:
• Work collaboratively with Nurse Case Managers and Social Services staff on care coordination, member follow-up, communication with agencies.
• Support PHM outreach activities by conducting calls for program enrollment, Health Risk Assessment (HRA) completion and other relevant questionnaires, wellness checks, and quality initiatives to help close care gaps.
• Contact members at regular intervals per their acuity level and care plan needs.
• Document all outreach, coordination activities, and member interactions in the case management system (CM System) accurately and in a timely manner, in alignment with program protocols.
• Support members experiencing transitions of care by assisting case managers with coordination of post-discharge needs, follow-up appointments, and transportation.
• Gather clinical information and assist with coordinating services such as home health care, prescriptions, durable medical equipment (DME), and social service referrals.
• Prepare and send member correspondence, as directed by case managers or in alignment with case management protocols.
• Report and escalate member concerns, variances, or changes in condition to the appropriate care team members to ensure timely intervention and coordinated follow-up.
• Assist members with appointment scheduling, transportation, referral coordination, and other care coordination needs.
• Gather clinical information from outside sources such as PCPs, specialists, hospitals, SNFs, and electronic health records, and upload them into the case management system.
• Verify member eligibility, demographics, and benefits; confirm provider assignments to ensure authorizations are linked appropriately.
• Support closed-loop referrals to community supports, housing, and social service agencies, with follow-up to confirm services were delivered.
• Participate in ICT meetings by gathering and presenting information and communicating member needs and preferences to the care team.
• May be assigned to manage a caseload of low-acuity PHM members, under the supervision of Nurse Case Managers or Social Services staff.
• Serve as an associate and resource to members, providers, staff, and external customers regarding policies, benefits, and care coordination.
• Provide administrative support, including answering phones, assisting with correspondence, filing, scanning, and maintaining department data systems.
• Receive and route incoming faxes, medical records, and other documentation to the appropriate case management staff.
• Track and log outreach attempts and results in the case management system to support reporting and follow-up.
• Conduct outreach to members to verify needs are being met and services delivered.
• May be assigned to conduct in-person meetings with members during clinic visits.
• Assist with training and orientation of new staff, as requested.
• Assist case management team with oral interpretation, as applicable.
• Attend mandatory staff and departmental meetings.

Qualifications:
• High School diploma or GED required.
• Minimum of 3 years of experience in a healthcare or community health setting.
• Knowledge of case management processes and regulatory requirements for Medi-Cal, Medicare, and commercial programs preferred.
• Experience in a managed care environment preferred (IPA, HMO, or health plan).

Expected starting wage range is $21.00 - $22.52. Telecare applies geographic differentials to its pay ranges.
