ESSENTIAL DUTIES:
Family Resource Specialist
Be trained and thoroughly understand the guiding principles of Healthy Families America by attending CORE Training, HFA online training modules, and all related trainings as requested by Supervisor or Program Manager
Provide parent counseling during the assessment in areas such as Shaken Baby Syndrome, Safe Sleep, Post Partum Depression and other areas identified as needed
Responsible to make independent and discretionary decisions for the welfare of the client
Provide community referral sources with information and training on Healthy Families in order to keep referral numbers consistent and commensurate with the target population
Provide screens and assessments for assigned new families
Provide referrals for families to other community resources as necessary
Provide transportation to families, as needed, for Healthy Families related services
Documentation Responsibilities
Enter information regarding screens into the HFI database and have this information prepared for Supervisor prior to weekly supervision
Enter information regarding assessments into the HFI database within 48 working hours of assessment and give to supervisor for review
Record all referrals and screens into the referral log and track contact attempts and status of referrals
Maintain all contact records and necessary paperwork for client files
Enter information regarding referrals to other community resources into the HFI database
Complete 90% of assessments prior to the child's three month birthday and have all documentation to supervisor so first home visit can be completed before child's three month birthday
Complete certification for First Aid, CPR, and BBP
Be willing to respond, when off duty, for emergency needs of clients and staff
Attend all required Healthy Families Team Meetings and scheduled supervision
Maintain confidentiality regarding all client contact, personnel and financial matters of the Corporation
Contribute to the New Hope Services, Inc. team to accomplish services area goals
Complete an average of twenty assessments monthly
Family SupportSpecialist
Provide support to parents during home visits in areas such as: child development, parenting, stress management, crisis management, childcare, effects of child abuse and neglect
Ensure that enrolled clients receive at least 75% of the home visits they are scheduled to receive based upon level of service criteria
Provide transportation to families, as needed, for Healthy Families related services
Documentation Responsibilities
Enter information regarding home visits on a weekly basis into the HFI database
Maintain all contact records and necessary paperwork
Complete Family Goal Plans with families
Perform Developmental Screenings in a timely manner, consistent with Healthy Families guidelines
Refer and link families to appropriate community resources
Complete screenings with families as applicable. Provide appropriate follow up support and referral, based on outcomes of the screening.
Ensure that families are linked to a medical provider and that children are completing basic immunizations and attending well child visits
Be willing to respond for emergency needs of clients
Attend all required Monthly Healthy Families Team Meetings
Carry a caseload that meets the standards for Healthy Families Indiana
Schedule and attend weekly supervision 2 hours for staff working 35 hours or more per week
Attend training as assigned
QUALIFICATIONS:
Ability to work a flexible schedule

Knowledge of infant and child development

The Family Case Manager is a caseworker specialist in social services within a Local County Office of the Department of Child Services. You will protect families and children from abuse and neglect, maintain or reunify families whenever possible, and, when in the best interest of the child, achieve a permanent home or independent living for children unable to be reunited with their families. A Day in the Life:
Responsibilities include:
* Investigate reported incidents of child abuse, neglect, or dependency, determine whether the incident is substantiated, and develop recommendations to a Juvenile Court or County Director for disposition.
* Assess the level of risk to the child for additional injury or harm, including whether the child is in imminent danger, and may remove the child from the family as the situation warrants, placing a child in a protected environment.
* Perform needs assessments to determine treatment options for families and children reported to be abused/neglected.
* Testify and prepare pre-dispositional and progress reports, maintain case files, and develop briefing reports for the community child protection team. Develops "informal adjustment" plans with families and children to divert children from the juvenile justice system.
* Develop case plans to assist families and children to become more self-sufficient in a safe and nurturing environment and to ensure a permanent home for the child.
* Develop discharge/transition programming to assist families and children to attain and maintain self-sufficiency in a safe environment.
* Monitor and assess the service capability of treatment programs and providers to ensure consistency with the case plan.
* Accept on-call responsibility as needed to receive and investigate allegations of child abuse/neglect.
* Conduct home studies and family histories and recommend placement for adoption to an adoption team.
* Visit children in out-of-home placements to coordinate family reunification, adoption, or independent living efforts.
* Maintain data to help management evaluate trends in family and child needs.

This position has the primary responsibility of providing the full breadth of Child and Youth Behavioral (CYB) counseling services to children of all ages of military service members and their families at military installations and off military installations. These services may include non-medical counseling, training/ health and wellness presentations, consultation with parents, personnel, schools, and consultation to installation command regarding behavioral health issues affecting children and youth. CYB School counselors will be assigned to Department of Defense (DoDEA) schools and community schools and will need to have experience working in a school setting to qualify for a school position. CYB School counselors will also be assigned to youth programs, summer camps, and on demand assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation and the schools.
Provides non-medical, short-term, solution focused, counseling directly to eligible children and youth of service members at assigned primary and secondary locations. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
CYB counselors will also have a focus on supporting the staff and personnel of DoDEA , community schools, summer camps (if available/assigned) as well as providing parent training and guidance.
Provides training and health and wellness presentations, participates in health fairs and other base/installation activities focused on children and youth.
Enters counselor activity data daily through smart phone, or web application, to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member.
Establishes and maintains working relationships with community resources focused on children and youth and provides appropriate linkages. Consistently visible within the school which allows MFLC services to be accessible when working with children, the counselor must abide by line of sight protocol (another adult with responsibility for the child must be able to see the counselor and child they are working with at all times).
Partners with POC to provide CYB services in a manner that addresses the needs of the installation/facility. Develops an effective and professional working relationship with the installation/facility POC.
Manages duty to warn and restricted reporting situations according to DoD protocol and staffs the cases with Regional Supervisor/Regional Director.
Communicates with Regional Supervisor and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations and in facilities to which they are assigned. Responds to critical incidents and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. The CYB will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.
Participates in regular in-services/training, Quality Improvement committees, initiatives, studies, pilot programs other contract activities as assigned/appropriate as directed by the customer and/or Magellan. ESSENTIAL DUTIES:
Family SupportSpecialist
Provide support to parents during home visits in areas such as: child development, parenting, stress management, crisis management, childcare, effects of child abuse and neglect
Ensure that enrolled clients receive at least 75% of the home visits they are scheduled to receive based upon level of service criteria
Provide transportation to families, as needed, for Healthy Families related services
Documentation Responsibilities
Enter information regarding home visits on a weekly basis into the HFI database
Maintain all contact records and necessary paperwork
Complete Family Goal Plans with families
Perform Developmental Screenings in a timely manner, consistent with Healthy Families guidelines
Refer and link families to appropriate community resources
Complete screenings with families as applicable. Provide appropriate follow up support and referral, based on outcomes of the screening.
Ensure that families are linked to a medical provider and that children are completing basic immunizations and attending well child visits
Be willing to respond for emergency needs of clients
Attend all required Monthly Healthy Families Team Meetings
Carry a caseload that meets the standards for Healthy Families Indiana
Schedule and attend weekly supervision 2 hours for staff working 35 hours or more per week
Attend training as assigned
QUALIFICATIONS:
Ability to work a flexible schedule

Desire to support, strengthen, and establish trusting relationship with families who are pregnant and/or parenting young children
Knowledge of infant and child development
Experience and willingness to work with the culturally diverse populations that are present among the site's target population
Open to reflective practice (i.e. has capacity for introspection, communicates awareness of self in relation to others, recognizes value of supervision, etc.)
Must possess a valid state driver's license, proof of insurance coverage per company policy, and have a vehicle that is reliable for transportation of consumers
EDUCATION/YEARS OF EXPERIENCE:
High School Diploma or equivalent required; Associate's or Bachelor's degree, Council for Higher Education Accreditation (CHEA) approved , in Education or Social Services field such as Sociology, Psychology, Social Work or other degrees deemed applicable in accordance with established service standards preferred

Role Overview:
The Family Case Manager is a caseworker specialist in social services within a Local County Office of the Department of Child Services. You will protect families and children from abuse and neglect, maintain or reunify families whenever possible, and, when in the best interest of the child, achieve a permanent home or independent living for children unable to be reunited with their families. You will conduct home visits, develop case plans, coordinate with community resources and external providers, attend and testify at court hearings, and prepare documentation for case files, court proceedings, and agency reporting.

A Day in the Life:
Responsibilities include:
* Investigate reported incidents of child abuse, neglect, or dependency, determine whether the incident is substantiated, and develop recommendations to a Juvenile Court or County Director for disposition.
* Assess the level of risk to the child for additional injury or harm, including whether the child is in imminent danger, and may remove the child from the family as the situation warrants, placing a child in a protected environment.
* Perform needs assessments to determine treatment options for families and children reported to be abused/neglected.
* Testify and prepare pre-dispositional and progress reports, maintain case files, and develop briefing reports for the community child protection team. Develops "informal adjustment" plans with families and children to divert children from the juvenile justice system.
* Develop case plans to assist families and children to become more self-sufficient in a safe and nurturing environment and to ensure a permanent home for the child.
* Develop discharge/transition programming to assist families and children to attain and maintain self-sufficiency in a safe environment.
* Monitor and assess the service capability of treatment programs and providers to ensure consistency with the case plan.
* Accept on-call responsibility as needed to receive and investigate allegations of child abuse/neglect.
* Conduct home studies and family histories and recommend placement for adoption to an adoption team.
* Visit children in out-of-home placements to coordinate family reunification, adoption, or independent living efforts.
* Maintain data to help management evaluate trends in family and child needs.

You can work as much or as little as you like as an On-Demand Team Member (TM) and your schedule may vary depending on shift availability and store needs. This role is ideal for those that are looking for a personalized work schedule.
As an On-Demand TM, you will not be included on the posted weekly schedule, but rather will have the opportunity to create your own schedule by picking up shifts posted by leaders or other team members (via our my Time mobile scheduling app) that work best with your schedule.

As a Guest Advocate, no two days are ever the same, but a typical day will most likely include the following responsibilities:
* Demonstrate a service culture that prioritizes the guest service experience by delivering the service standard.
* Create a welcoming experience by greeting guests as you are completing your daily tasks.
* When guests need assistance, engage with guests in a welcoming way, to help solve their specific needs.
* Be knowledgeable about the tools, products, and services available in the total store, and specific to your area, to solve issues for the guest and improve their experience.
* Thank guests and let them know we're happy they chose to shop at Target.
* Scan and bag all guest items efficiently, accurately and in compliance with food safety standards and company best practices.
* Work efficiently to minimize guest wait time while maintaining guest service and accuracy.
* Make the guest aware of current and upcoming brand launches, store activities and events.
* Know and speak to the benefits of Target Loyalty programs with every guest and assist with application or sign-up as needed. Be familiar with all fulfillment services, and know how to direct the guest to enroll, activate and use them.
* Understand and show guests how to use Wallet and the other features and offerings within the Target App.
* Attempt every return and follow register prompts, partnering with immediate leaders as needed to help solve for the guest while following Target's policies and procedures.
* Partner with leaders as needed to de-escalate any situations and recover the guest shopping experience while following Target's policies and procedures.
* Deliver easy and accurate service to all Order Pick Up, Drive Up, and Registry guests.
* Maintain a clean, clutter free work area (including gathering abandoned items, baskets, and hangers).
* Stock supplies during store open hours while being available for the guest.
* Demonstrate a culture of ethical conduct, safety and compliance.
* Work in a safe manner at all times; comply with all safety policies, best practices, and training; report hazards and correct where possible.
* Support guest services such as order pick up (OPU), Drive-up (DU) Orders, DU Returns and maintain a compliance culture while executing those duties, such as compliance with federal, state, and local adult beverage laws.
* Support Cash Office processes as needed, including management of cash systems.
* All other duties based on business needs

* Capability to remain focused and composed in a fast-paced environment and accomplish multiple tasks within established timeframes.
* Welcoming and helpful attitude toward guests and other team members
* Attention to detail while multi-tasking
* Willing to educate guests and engage around products and services
* Ability to communicate on multiple frequency devices and operate handheld scanners, and other technology equipment as directed.
* Work both independently and with a team
We are an awesome place to work and care about our teams, so we want to make sure we are clear on a few more basics that we expect:
* Accurately handle cash register operations, cash transactions, and support cash office operations as needed
* Scan, handle and move merchandise efficiently and safely, including frequently lifting or moving product up to 10 pounds and occasionally lifting or moving merchandise up to 44 pounds without additional assistance from others.
* Flexibility when picking up shifts (e.g., nights, weekends and holidays) to help meet store needs
* Active engagement by picking up and working shifts as well as responding to our attempts at contact. You must work at least one shift within 5 months or you will be administratively terminated.
* Capable of working in and exposure to varying temperatures, humidity, and other elements while performing certain job duties including but not limited to Drive-Up, carryout, etc.
* Ability to remain mobile for the duration of a scheduled shift (shift length may vary).
