Dell Medical School in conjunction with Dell Children's Medical Center of Central Texas (DCMCCT), is seeking Physician Assistants with cardiac critical care credentials to be part of the Texas Center for Pediatric and Congenital Heart Disease, working collaboratively with a multidisciplinary team to serve the growing need of the regional pediatric population, delivering world class pediatric cardiac critical care.
Responsibilities
Counsels and educates cardiac critical care patients and their families/ caregivers concerning health conditions, treatment options and community resources.
Documents delivery of cardiac critical health care and nursing processes in accordance with care standards and network nursing policy.
Participates in orientation and education of staff and students.
Obtains cardiac critical care patient history and performs assessment via observation, interview and examination.
Orders, performs, and interprets diagnostic studies for cardiac critical care patients. Performs health assessments, screening, and patient care.
Provides direct treatment and management of cardiac critical care conditions via prescriptive authority or referral to other healthcare providers or community resources. Engages in discharge planning.
Other related duties as assigned.
Required Qualifications
Physician's Assistant or Licensed Registered Nurse credentialed from the Texas Board of Nursing. Licensed Advanced Practice Nurse Practitioner with Prescriptive Authority specializing in Cardiac Critical Care. Credentialed from the Texas Board of Nursing in Acute Care nursing. Experience in caring for children with complex congenital and acquired heart disease in a Pediatric and Congenital Critical Care Unit. Highly proficient in Microsoft Office applications. Ability to work efficiently and flexibly in a fast-paced environment with minimal guidance. Ability to address individuals and information with a high degree of sensitivity and confidentiality in various situations. Capacity to work well with physicians and other care providers as part of a multi-disciplinary healthcare delivery team.
Relevant education and experience may be substituted as appropriate.

What You Will Do - Essential Functions

Job description
The HCBS Direct Support Professional (DSP) provides personalized support to individuals with developmental disabilities within their family home and community. The DSP helps individuals achieve their personal goals, maintain independence, and enhance their quality of life by providing assistance with daily living activities, personal care, skill development and community integration.
About the role:
Full-time, Part-time and on-call shifts available. Schedules available are based on member and family needs and may vary and may include morning, afternoon, evening and/or weekend availability. May involve exposure to various household environments, including pets and varying noise levels.
Responsibilities:
Personalized Support & Care:
Assist individuals with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and meal preparation.
Support individuals with mobility assistance, including transfers and the use of adaptive equipment if needed.
Administer medications as prescribed and document accordingly (if trained and authorized).
Provide companionship and emotional support while promoting self-advocacy and independence.
Skill Development & Community Engagement:
Assist individuals in learning and practicing life skills such as cooking, cleaning, budgeting, and communication.
Encourage and facilitate community involvement, including outings, recreational activities, and social interactions.
Support individuals in achieving goals outlined in their Individual Support Plan (ISP).
Behavioral Support & Safety:
Implement Positive Behavior Support (PBS) strategies to address challenging behaviors.
Ensure a safe and supportive environment while respecting the rights and dignity of each individual.
Respond appropriately to emergencies and follow all safety and incident reporting procedures.
Documentation & Communication:
Maintain accurate and timely documentation of services provided, progress notes, and incident reports as required.
Communicate effectively with family members, guardians, and supervisors regarding any concerns or changes in the individual's needs.
Follow all company policies, state regulations, and HCBS program requirements.
Requirements:
A least 18 years of age.
A High School diploma or GED.
Able to obtain Arizona State level one fingerprint clearance.
Able to obtain certifications for CPR/First Aid, Article 9, and Prevention and Support.
Experience working with individuals with Intellectual/Developmental Disabilities is preferred

General Position Description: Provides supports and assists to individuals with intellectual, physical and/or developmental disabilities to live as independently as possible.
Essential Duties and Responsibilities:
Assists individuals with personal care and hygiene needs. This includes assistance in the restroom, assistance with eating, and moving, carrying, and lifting (using one-person, two-person, and mechanical lift transfers).
Performs housekeeping tasks such as cleaning, cooking, laundering, making beds, yard work, and other household chores or tasks.
Provides transportation and support out in the community to our members for outings/field trips, medical appointments, and shopping.
Provides guidance on community participation, household budgeting, exercise, nutrition, and supports vocational responsibilities.
Administers medications and/or assists members in the self-administration of medication including non-prescription and prescription as prescribed by licensed medical specialists.
Keeps individual file on each person up-to-date. Reads and follows members individual support plan (ISP).
Reports incidents to appropriate staff and assists in the completion of incident forms.
Assesses and provides behavior/physical management techniques in crisis situation and attains assistance if needed. Perform standard First Aid and Perform Cardio Pulmonary Resuscitation (CPR) if needed.
Job Coach/Direct Support Professional: Provides on-the-job training tailored to the needs of the member and supports them to adjust to their chosen work environment. Discovers member's motivation and helps develop their skills. Presents and teaches job searching skills and techniques. Counsels members on fine-tuning work habits/skills in preparation for competitive employment.
Performs other duties as assigned.
Requirements
Minimum Requirements:
High School Diploma or GED preferred.
Three (3) months of caregiving experience preferred.
Regulatory:
Must be at least 21 years of age.
Current, unrestricted drivers with no more than two (2) minor moving violations or one (1) accident within the past three (3 years). Three (3) years of driving experience required.
Must be able to pass a criminal background check.
Ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card (paid by organization).
Ability to obtain and maintain CPR, First, Aid, Article IX, and Prevention & Supports certification (paid training).
This position requires that employees remain awake during working hours.
Skills/Job Knowledge/Abilities: Able to establish and maintain team atmosphere of communication and collaboration for with team and members. Must be self-directed and be sensitive to cultural and linguistic diversity. Excellent customer service in stressful situations. Maintains a professional in appearance, communications, and actions. Must be able to direct and assist members for their safety. Basic knowledge of principles and practices of self-determination and individualized support. Able to work in a fast-paced environment with multiple interruptions. Able to remain calm in emergency situations.
Working Conditions/Physical Requirements (with or without accommodation):
In home, day program, or group home environments. May required visiting different sites or homes for member care. Driving to member homes and other locations on a regular basis. Interacts with employees and members on a daily basis. Able to lift up to 50lbs to support member care. Visual acuity to assess the wellbeing of members and drive. Able to communicate verbally with members and emergency personnel as needed for daily operations. Hearing ability for communication in persona, phone, and/or other electronic methods. Manual dexterity to assist members with paperwork and daily functions. Able stoop, squat, reach, pull, push, stretch, ascend and descend stairs, stand and sit for long periods of time. Able to use required personal protective equipment (PPE). Work outdoors and indoors as needed. May be required to work additional hours or days depending on circumstances.

The Coordinator works with clinical staff to ensure appropriate supervision of Direct Care Workers and facilitates conferences with Arizona Long Term Care System Case (ALTCS) Management. Additionally, the Coordinator is responsible for hiring, orienting, and counseling Direct Care Workers.

Is available on an on-call basis outside of normal business hours for needs associated with the Direct Care Worker Program
Maintains accurate census of clients
Coordinates with the Clinical Supervisor to assign staff to ensure in-home oversight of Direct Care Workers
Performs accurate and timely data entry of payroll information and ensures appropriate compensation of Direct Care Workers
Ensures authorization is obtained in advance for provision of all services
Ensures that services provided are within the parameters of authorizations
Maintains records documenting the training and testing of Direct Care Workers
Assists the functions of billing and care management through timely and accurate data entry
Ensures compliance with Direct Care Worker Training and Testing guidelines
Completes the Non-provision of Services Log and submits it to ALTCS/DDD in a timely fashion.
Performs other duties as assigned
