Responsibilities:
* Responsible for assisting patient with and per plan of care, the following;
* Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc
* Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
* Safe transfer and ambulation techniques
* Exercise according to the plan of care
* Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
* Patient's laundry
* Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
* Observation and teaching of home safety practices
* Recognition of an emergency situation and knowledge of emergency management procedures
* Must follow patients plan of care and complete all tasks assigned
* Communicates in a constant, clear, and effective way with the client, family/caregiver and office
* Observes and documents patient condition and reports any changes, to the assigned nurse supervisor
Requirements:
* Meets all training requirements in accordance with state and federal laws when applicable
* Ability to read, follow written instructions and document care given
* Possess and maintains current CPR certification in accordance with state regulation
* PPD or chest x-ray if applicable
* Successful completion of Maxim screening examinations
* Participation and completion of Maxim's competency program
* Must meet all federal, state, and local requirements

What You'll Do
Medication Reconciliation
Review and document patients' medication histories for accuracy and completeness.
Collaborate with pharmacists and care teams to resolve discrepancies in medication lists.
Patient Education and Support
Educate patients on medication use, side effects, and adherence strategies tailored to their needs.
Address patient inquiries or connect them with the appropriate provider for further support.
Medication Adherence Monitoring
Use EHR systems and other tools to track adherence to prescribed regimens.
Proactively follow up with patients at risk of non-adherence and communicate concerns to care teams.
Care Coordination
Assist with transitions of care, ensuring seamless medication management during discharges or care setting changes.
Coordinate medication refills, prior authorizations, and delivery services to keep patients on track.
Data Entry and Documentation
Maintain accurate, up-to-date patient records in the EHR system.
Document patient interactions, interventions, and communications to support continuity of care.
Support for Healthcare Teams
Collaborate with pharmacists to identify potential drug interactions or contraindications.
Provide data and insights to enhance care plans and improve patient outcomes.
Compliance and Quality Assurance
Ensure adherence to HIPAA, state pharmacy laws, and all relevant regulations.
Participate in quality improvement initiatives aimed at improving patient safety and medication management outcomes.
What You Bring
Qualifications
Certification: Current Pharmacy Technician License from a recognized certifying body (e.g., PTCB or NHA).
Experience: Previous experience in a pharmacy setting, ideally with a focus on care management or chronic disease management.
Knowledge: Strong understanding of pharmacology, medication therapy management (MTM), and patient-centered care practices.
Skills
Technology Proficiency: Familiarity with EHR systems, pharmacy software, and telehealth tools.
Communication: Excellent verbal and written communication skills for patient and provider interactions.
Attention to Detail: Exceptional accuracy in documenting patient information and managing medication orders.
Problem-Solving: A proactive approach to identifying and resolving medication-related issues.
