Responsibilities Accurately and thoroughly document patient visits and procedures in real time, including medical history, physical exams, assessments, and treatment plans, as dictated by the provider. Transcribe physician-dictated diagnoses, prescriptions, and instructions for patients or caregivers regarding follow-up and self-care. Identify and correct inconsistencies or potential errors in documentation to support accuracy and reduce clinical risk. Record all relevant clinical data, lab results, and physician interpretations into the electronic health record or designated systems. Ensure compliance with medical documentation standards, legal guidelines, and patient confidentiality requirements (e.g., HIPAA). Collect and organize data for quality improvement programs such as the Physician Quality Reporting System (PQRS) and assist in preparing submissions. Support systems for tracking patient follow-up, treatment compliance, and care coordination. Review and edit medical documents for clarity, grammar, spelling, and accuracy prior to provider sign-off. Assist in the development and management of provider schedules. Perform administrative and clerical tasks, including making phone calls, organizing documentation, and supporting clinical staff as needed. Perform other duties as assigned.
