* Responsible for taking immediate action when a sick/injured pet is identified in the store; transport to the vet as needed.

Position Overview:
We are seeking a dedicated and experienced Basic Life Support (BLS) Instructor to join our team. The BLS Instructor will be responsible for teaching American Red Cross Basic Life Support courses, averaging 1-2 classes per week. Work Schedule :

IRIS SDPC RNs are responsible for administering the Wisconsin Personal Care Screening Tool; creating person-centered plans of care; providing personal care oversight to a group of people in IRIS, providing education and training for IRIS participants and care providers, and conducting the required documentation and follow-up. As an IRIS SDPC RN, you'll play an important role in helping people of various backgrounds and abilities live their lives the way they choose.
Knowledge/Skills/Abilities
Provides personal care assessments and oversight to the My Cares Groups by administering the Wisconsin Personal Care Screening Tool and addendums as required
Documents assessment as required by individual tool and Department of Health Services policies and by completing oversight visits and calls as required
Oversees a My Cares Groups of participants, develops individual plans of care, ensures physician orders for care are obtained and reviews and revises plan of care as needed
Submits for Prior Authorization for personal care services
Complies with all Department of Health Services policies and SDPC Guidelines, procedures, and practices along with documentation and program regulations
Provides personal care training to participants or care providers as requested and provides educational materials as needed
Completes collateral contacts with IRIS Consultants and Long-Term Care Functional Screeners and physicians to ensure care needs are met
Completes other duties as assigned
Overtime work may be required
May be required to drive 50% of the time during a given day of member home visits
Exposure to members homes which may include navigating stairs, exposure to different environments, and pets
Required Qualifications
• At least 2 years nursing experience, and at least 1 year of experience serving the target groups of the IRIS program (adults with physical/intellectual disabilities or older adults), or equivalent combination of relevant education and experience.
• Active and unrestricted Registered Nurse (RN) license in the state of Wisconsin.
• Associate's degree in nursing.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements unless otherwise required by law.

PERSONAL CARE ASSISTANT RESPONSIBILITIES
Remind clients to take prescribed medication
Assist clients with ambulation and mobility around the house or outside (doctor's appointments, walks etc.)
Assist clients with personal care and hygiene
Help clients complete physical therapy and other recommended exercises
Do the client's shopping or accompany them when they shop if needed
Perform light housekeeping duties that clients can't complete on their own, including preparing meals
Collect information about conditions and treatment plans from caregivers, nurses, doctors and family members
Report any unusual incidents to nurses, doctors and family members
Act quickly and responsibly in cases of emergency
PERSONAL CARE ASSISTANT REQUIREMENTS
Previous caregiver experience preferred but not required
Current CPR (American Heart Association) Certification
Willingness to adhere to health and safety standards
Knowledge of housekeeping activities and cooking with attention to dietary constraints
Must be respectful and compassionate with a good bedside manner
Outstanding communication, time management and interpersonal skills
Physical endurance to complete required tasks
High school diploma or equivalent

Job Description
What You Get To Do:
The Direct Support Professional (DSP) Float is a position designed to provide fill in or substitute coverage at any assigned service site and may be scheduled for hours that are vacant due to staff turnover, changes in scheduling, or staff paid time off. Work assignments may be during any scheduled hours and at any sites where Dungarvin Eau Claire, WI. provides services.
Working directly as a Direct Support Professional (DSP) / Caregiver Float, you will be able to assist in a residential group home by assisting people with disabilities achieve a variety of outcomes, assist with personal cares and provide normalization/community integration. The DSP (caregiver/mentor) is responsible for providing:
Support and mentor with daily activities to those we support in a manner that ensures their health and safety. Some examples include helping with meals, cleaning, housekeeping, laundry and facilitating fun activities.
Hands-on mentoring to those we serve to help them become more independent.
Positive direction to the individuals to encourage them to achieve their personal goals.
Provide medication administration and ensure person's medical well being.
Qualifications
What Makes You A Great Fit:
Excitement for working with adults with disabilities
Must be at least 18 years old
Good communication is key in this role. You'll rely on your reading, writing, and communication skills to document care, follow support plans, and collaborate with both internal and external team members.
Valid Drivers License (due to transporting persons served)
Candidates must be comfortable driving company vehicles to transport persons served as required
Some positions / locations may require own personal vehicle and auto-liability insurance based on the role
A successful background clearance is required as part of the onboarding/employment process
