Advance Practice Provider Transition into Practice Fellowship Ann and Robert H. Lurie Children's Hospital of Chicago offers an Advance Practice Provider (APP) transition into practice fellowship for all new graduate APPs. All new hire APPs, even those with experience, are invited to participate in the fellowship. The APP transition into practice fellowship is designed to provide a supportive structure and strategies to facilitate the transition from the academic to clinical setting for newly hired APPs and institution to institution transition for those with experience. The first year of employment will consist of quarterly didactic sessions with integrated facilitator led peer supported transition sessions, structured mentorship, and precepted practice-based learning with standardized competencies. APPs in the fellowship are hired as paid employees of the institution. The fellowship is designed to aid the development of well prepared, clinically competent, APPs as well as to provide support, build confidence and skills to develop a collaborative practice. Essential Job Functions: Area Specific Job Accountabilities: * Provides expert clinical care, education, consultation, advocacy research, leadership and mentorship. * Manages primary, acute and chronic health problems through comprehensive history and physical examination assessment and diagnosis. This may include palliative and end of life care. * Orders, performs and interprets diagnostic and therapeutic tests and procedures. * Orders and prescribes pharmacologic and non-pharmacologic interventions and performs other therapeutic or corrective measures to treat illness and improve health status. * Promotes the advancement of the profession at Lurie Children's and the community-at-large. This may include publication, participation in professional organizations and providing educational support to the nursing staff. * Collaborates and coordinates care with medical team to deliver evidenced-based care and establish best practices in clinical care. * Participates in research design, implementation, and interpretation. * Meets the Medical Staffs Allied Health Practitioners core measures of performance related to patient care, medical knowledge, practice-based learning and improvement, interpersonal communication skills, professionalism, and system-based practice. * Performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers, and others. * Role may include evenings, nights, weekends and holidays. Knowledge, Skills and Abilities: * Master's degree or higher from an accredited school appropriate for certification as a Nurse * Practitioner Physician Assistant. (CNS certification is not acceptable if hired after January 1, 2010) * Two (2) years of relevant clinical experience is preferred. * Licensure in the State of Illinois as an APRN/PA is required. * Clinical privileges through credentialing by the Medical Staff Office achieved within 90 days of hire.
