Job Summary
The Care Supervisor provides direct oversight and management of their assigned division's Success Coaches (Case Managers). They are accountable for ensuring 1) that all assigned staff are being managed, trained, supported, and held accountable, 2) that all participants within their assigned division are being consistently provided with care, resources, support, and services that best meet their needs 3) the operational, staff scheduling, and licensing/contractual related the care needs of the division are met in a timely, consistent manner - and that both 1), 2) and 3) are done so in accordance with the organization's mission, vision, values, frameworks, and philosophies. This position requires a significant and frequent amount of timely responsiveness to deadlines, crisis, needs, emergencies to include, but not limited to covering shifts that have not been sufficiently covered and after hours in-person response to operational/facility emergencies. This position provides direct support to the Division Manager and is a part of the division's leadership team. The program requires frequent collaboration with other organizational divisions and community stakeholders.
What You'll Do:
Conduct weekly case staffing with each case worker all programs within The Neighborhood (approximately 1.5 - 2 hours each). Attend weekly shelter calls hosted by CYFD
Manage the census spreadsheet for all housing programs. Manage the entry & discharge information of all youth residing at My Friend's Place
Update the NM CYFD Shelter Daily Census form as often as needed to reflect accurate shelter availability, or utilize designated census tracking platforms. Ensure the contact information for the shelter on the shelter census page is accurate.
Report to the Licensing Authority within 24 hours, any serious incident or unusual occurrence which has, or could threaten the health, safety, or welfare of the clients or staff of the facility.
Provide reasonable access and referrals to community based physical and behavioral health services and supports to meet the needs of the participants to ensure the continuity of care
Complete the Adverse Childhood Experiences (ACE) trauma screen with each eligible participant receiving services
Complete the Child Sexual Exploitation Identification Tool (CSE-IT) with each participant ages 10 and over
Ensure every participant is enrolled in an appropriate education program in accordance with New Mexico State law
Ensure each participant is properly immunized according to the immunization schedule of the New Mexico Health Department, Public Health Division.
Arrange for each participant a general medical and dental examination by a physician/dentist for each participant in care
Conduct admissions/coordinated entry and conduct both coordinated/uncoordinated discharge
What You Need:Education & Experience
Bachelor's degree in Social Work, Family and Consumer Sciences, Criminal Justice, Psychology, Sociology, or Criminology from an accredited College or University
Must have a minimum three years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health and/or health care experience, and relevant work experience with juvenile justice systems
Must have a minimum of two years of experience in a position of formal leadership and supervision of a
team of at least 4 persons

Job Description
A Direct Support Professional (DSP) caregiver provides direct support, positive direction, instruction, and assistance to individuals with mental, intellectual & developmental, and physical disabilities. The ultimate goal is to empower individuals with disabilities to live as independently and comfortably as possible while maintaining their dignity and respect. We have many programs throughout Albuquerque!

Provide hands-on assistance, encouragement, mentoring, and guidance.
Ensure the comfort, safety, and personal growth of the individuals we serve.
Assist with daily living tasks such as moving around, getting in and out of bed, dressing, bathing, personal hygiene and more.
Light housekeeping tasks and meal preparation.
Build relationships with persons served and teammates.

Transport persons served to appointments or other activities outside the home.

We are looking for individuals to work at our program with persons served who may exhibit challenging interfering behaviors, such as physical aggressions, verbal aggressions and property destruction. To be successful in this role we are looking for candidates with prior experience with intense high behaviors, both verbal and physical. You will provide supports to people with intellectual and developmental disabilities, people who are faced with metal health, behavioral, and/or significant medical challenges.
We are looking to hire staff who are patient, have good decision making and experience working with behavioral management or crisis plans.
Prior IDD, mental health, psychiatric, and/or security experience is strongly preferred.
