GENERAL STATEMENT OF RESPONSIBILITIES
Responsible for providing supervision and training for people with disabilities in a residential setting, maintaining client confidentiality and a professional demeaner.
EXAMPLES OF WORK (NOT ALL INCLUSIVE)
1. Create a congenial and home like atmosphere within the community residence.
2. Promotes a maximum independence of each resident.
3. Facilitates opportunities for residents to be involved in community activities such as shopping, leisure
and religious activities as stated on activity schedule.
4. Supervises and prepares nutritious meals stated on menu.
5. Assist each resident in developing attitudes and behavior appropriate to community living.
6. Be supportive of and encourage each resident's participation in the day training or vocational program
prescribed.
7. Protect each resident's rights and freedoms and maintain confidentiality of personal information
concerning the individual, in accordance with HIPPA and the Bill of Rights for people with intellectual disabilities.
8. Report any unusual incidents to the Residential Home Supervisor or Manager. Always following the
chain of command.
9. Report suspected abuse or neglect.
10. Maintain facility and client records.
11. Provide for proper storage, handling of drugs and medicines. Assist the resident by making
medications available and reminding the client to take them at the appropriate times.
12. Provide for or assist in transportation (using company vehicles) for residents to and from place of employment, personal
appointments, and for recreational and other events.
13. Assist residents in communication with their families.
14. Assist residents in handling of money and making and keeping appointments when indicated.
15. Ensures opportunity for resident privacy.
16. Responsible for household duties as assigned on duty calendar.
17. Participate in monthly fire evacuation drills and all disaster drills with proper documentation of same.
18. Maintain a safe environment.
19. Avoid the exploitation of any resident.
20. Supervise and participate in routine household maintenance. Ensures cleanliness of house/grounds.
21. Assist in the on-going assessment of client progress.
22. Ensure the implementation of program plan (IPP), and input the information in the Therap Software
system.
23. Ensure your work hours have been recorded on the electronic time software daily.
24. Assist each resident as needed with self care and grooming needs.

Review residents Individualized Service Plan (ISP), Health Risk Screening Tool (HRST), Healthcare Plan (HCP) and other care plans as applicable to the individual in order to provide appropriate physical and/or emotional support, care, health, and safety oversight.
Provide constant support and care to residents including but not limited to bathing, grooming, dressing, toileting, assisting with eating and hydration, skin care, positioning, and independent living skills.
Assist residents to prepare meals and snacks according to weekly menus posted by the Site Leader.

The duties are of a high degree of responsibility and are performed under the supervision of the Team Leader Supervisor and the various professional staff working in the facility.
**KEY RESPONSIBILITIES:**
1. Protects the safety and welfare of the clients by providing supervision and training outlined on the Individual Program Plan.
2. Implements the assigned clients' Active Treatment Schedule as written. Works with clients assigned.
3. Implements behavioral procedures when maladaptive behaviors occur.
4. Implements client training activities as outlined on the Individual Program Plan.
5. Assists clients to the extent necessary to maintain their rooms in an orderly manner.
6. Assures that clients are dressed appropriately for the time of day, activity and weather. The amount of assistance necessary will vary from client to client depending upon the level of independence of each client. Assuring means providing only that amount of assistance that is required for the task to be completed.
7. Accompanies and trains clients going outside the facility on planned outings, as assigned.
8. Attends in-service training which is required to maintain employment
9. Completes in-service training and demonstrate competence on the material for which the training is provided.
10. Implements, as trained, all data collection procedures. Protect and implement the rights of the clients.
11. Follows policies and procedures regarding employee conduct and dress code to ensure/protect the interests and safety of clients, coworkers, and professional staff.
12. Advises the Team Leader Supervisor of any needed supplies.
13. Correctly follows supply procedures of unit operations. Complete, as trained and assigned, routine duties of the client's living unit to maintain it in a neat, clean and sanitary condition.
14. Gives written reports to the Team Leader Supervisor of client clothing needs and other special needs. Follow-up to assure that the request is answered. Report through the Team Leader Supervisor any change in the client's condition. This should be done as soon as change is noted.
15. Follows-up to assure these changes are evaluated by medical or behavioral staff as appropriate.
16. Attends Interdisciplinary Team meetings and morning/afternoon Active Treatment Reports as assigned.
17. Implements health care techniques as trained, i.e., oral hygiene, bathing. Operate within the policies and procedures of the facility.
18. Operates within all State and Federal laws, as well as the ICF/MR Conditions of Participation.
**MINIMUM EDUCATION REQUIRED:**
Direct Care Staff shall have at least a tenth (10) grade education.

Provide community integration and community living ex. take individuals to games, concerts, shopping, and other outings

Assist individuals in employment training
Create a person-centered environment of support for the individuals
Participate in the development of attainable outcomes for the individuals' service plan
Follow, address, and assist in the individual's personal care as needed
Report evidence of abuse, neglect and other health and safety issues
Complete incident reports and give to the supervisor timely for review
Develop and maintain an open line of communication for pertinent information with coworkers, nurses, caregivers or employers
Interact with the individuals in a supportive and respectful manner while maintaining authority
Complete documentation pertaining to overall support of the individuals
Keep supervisor or assigned designee informed of any issues or changes with the individual or in daily operations
Provide training to individuals to maintain current skills, learn new work skills, improve social skills and modify behaviors that would interfere with inclusion in the community
Provide transportation of individuals following policies and procedures
Comply with all regulations and procedures, as well as the policies and procedures
Perform other duties as assigned by management
Requirements
High School Diploma or GED
Must be 18 or older
Valid Driver's License
Assertiveness required
Clear tuberculosis screening
Clean drug screen and background check
Ability to lift and move up to 25 pounds
Ability to bend, reach, stoop, etc.
(Preferred) 2 years' experience or a combination of experience equivalent to an Associate's Degree

Maintain resident care records as required to document daily resident care to actively track illnesses, incidents, and other happenings concerning residents.
Maintain strict confidentiality of residents and/or Fellowship Home business so as not to disclose sensitive information and/or convey Fellowship Home in a negative way, including social media outlets.
Maintain understanding and awareness of the Department of Community Healths Healthcare Facility Regulation Divisions Rules and Regulations for Assisted Living Communities, interpret them appropriately, and apply them to effectively achieve and maintain operational effectiveness within your scope of responsibilities.
Qualifications
Qualifications:
A high school diploma or GED is preferred.
At least 2 years of documented personal care experience in the healthcare industr.
Certified Nursing Aide (CNA) license, is required.
Must have compassion and a desire to serve senior adults.
Physical Requirements:
Body Positions: Sitting, standing, squatting, kneeling
Body Movements: Lift (at least 35 lbs with or without assistance), bend torso, reach, turn head and torso, and/or push (at least 100 lbs with or without assistance) for up to 12-hour intervals.
Body Senses: Must have adequate use of eyes and ears; adequate power of speech.
Mental Requirements:
Reading: Must be able to read and comprehend written instructions pertaining to the care and oversight of the residents. Must be able to communicate verbally and in writing with peers and supervisors.
Language: Must be able to communicate in English, both verbally and in writing, well enough to give instructions, relay information, and carry on effective interchange with staff and residents.
Social: Must be able to relate well with others and provide a non-hostile workplace as well as receive direction from peers and supervisors with a positive attitude.
