MONDAY THRU FRIDAY
The Lutheran Home at Hollidaysburg, a skilled nursing and independent living community, is actively seeking candidates for a full time/exempt Social Services Professional. This position will assist in planning, organizing, implementing, evaluating, and directing the overall operation of Social Services to ensure that the medically related emotional and social needs of the residents are met and maintained on an individual basis.
Responsibilities:
Participates in discharge planning, development, and implementation of social care plans and resident assessments.
Supports the referral and admissions process for the entire campus.
Develops, administers, and coordinates social services' policies and procedures.
Responsibility to include intake and referral of prospective clients or campus residents and assist in locating appropriate services and case management needs.
Participates in discharge planning, development, and implementation of social care plans and resident assessments.
Provides information to the residents and families as to Medicare/Medicaid and other financial assistance programs available to the residents.
Coordinates social service activities with other departments, programs, and services.
Interprets social, psychological, and emotional needs of the resident to the medical staff, attending physician, and other resident care team members.
Assists in obtaining resources from community social, health, and welfare agencies to meet the needs of the campus resident; refers residents and families to appropriate social service agencies when the facility does not provide the services to meet the needs of the campus residents.
Visits residents to deal with any concerns.
Serves on, participates in, and attends various committees of the facility, (i.e., care plan, ethics, quality assurance, etc.); provides written and oral reports of the social service programs and activities.
Encourages residents to participate in the development and review of their plans of care.
Serves as the grievance officer for the healthcare center.
This position reports to the Nursing Home Administrator.
Qualifications:
Minimum of a Bachelor's degree in a Health/Human Services-related field; BSW preferred.
Three years of experience in social services in a long term nursing community, hospital, or other related medical facility.
Experience with MDS, Matrix, Epic, and MS Office preferred.

**What you'll do**
As a Direct Support Professional (DSP), you'll provide one-on-one support to individuals with intellectual/developmental disabilities - both at home and in their community - helping them live as independently as possible.
Your work may include:
+ Reading, understanding and following the client's Individual Support Plan (ISP), and reporting back to the IDD Program Specialist
+ Supporting individuals with daily living skills such as cooking, cleaning, personal care (i.e. grooming, hygiene etc.), or transportation

**What you need**
+ High school diploma or equivalent; or completion of a state approved training program; or demonstrate skills and receive a passing score on a written competency test
+ Successful background check and required in-service trainings
+ At least 18 years old
+ Valid driver's license and reliable transportation with proof of insurance, inspection and registration

*$1,000 sign-on bonus is valid for applicants effective 1/1/2025.
