Responsibilities:
Provide routine and preventive care, as well as diagnosis and treatment of acute and chronic illnesses.

Collaborate with multidisciplinary team members, including nurses, specialists, behavioral health providers, and care coordinators.
Educate patients and families on health promotion, disease prevention, and management of chronic conditions.

Qualifications:
Required:
MD or DO, or certified NP/PA with appropriate state licensure.
Board-certified or board-eligible in Family Medicine (or relevant certification for NP/PA).

Qualifications:
Flexibility required for shift worked and clientele served.
18 years of age or older
Ability to pass a MN Criminal Background Screen
Hold a valid driver's license and have an acceptable driving record; have reliable transportation and proof of current car insurance.
Rochester based Float DSp shall have essential physical skills - be able to walk, squat, stoop/bend, kneel, climb stairs, be able to lift at least 50 pounds, push and pull against potential resistance, and be able to implement non-physical and physical interventions with people supported when needed.
Due to the safety and regulatory requirements, and the need to communicate and supervise our clients, an applicant must have adequate vision, reading, writing, and documentation skills and comprehension of the English language.
An applicant must also be able to perform the essential functions of the position (personal cares, behavior, etc.) for all departments included in the identified geographical area supported.
