Campbell County Health is seeking an Advanced Practice Provider (PA or APRN) to join the Department of Geriatrics to support health services in two locations: a rural outpatient practice and in a residential long-term care facility.
The ideal candidate will have a strong background and experience in diagnosing and treating a wide range of geriatric illnesses and chronic disease with a focus on comprehensive patient care and quality of life. This full-time position involves working collaboratively with other healthcare professionals within the hospital-based system to provide high-quality medical services, ensuring the best outcomes for our patients.
Responsibilities:
1.0 FTE clinician alternating weeks between in a typical 5-day M-F workweek outpatient clinic and working in the residential long-term care facility
Care for adult patients in the outpatient and residential long-term care setting
One in three telephonic call for nurse advise at the LTC facility.
Develop and maintain a high standard of medical practice and clinical competency
Recognize and appropriately respond to medical emergencies as needed
Conduct complete or focused patient examinations to include necessary history and physical examinations
Conduct or order, interpret, and evaluate appropriate diagnostic testing to include initial interpretation of electrocardiography and routine radiologic imaging as training allows
Diagnose, formulate treatment plans, and treat a wide range of adult conditions and diseases common to the primary care setting
Conduct minor surgical procedures (i.e. laceration repair, I&D, and removal of foreign bodies from eye, ear, nose) needing no more than infiltrative anesthesia or peripheral nerve blocks
Provide comprehensive and continuous care for patients, including preventive care and disease prevention strategies
Refer patients to their primary care provider or medical specialists as necessary
Adhere to all relevant medical protocols and procedures
Stay informed or conduct literature research as needed on the latest advancements in Geriatric Medicine to improve patient health outcomes and healthcare delivery
Promote and shepherd innovative enterprise practices and changes in standards of care within the Geriatric Medicine specialty
Conduct ad hoc or informal instructional interactions with peers or clinical subordinates when required to ensure the optimal delivery of patient care
Participate in team meetings and collaborate with other healthcare professionals to ensure patient needs are met
Maintain accurate patient records and ensure patient confidentiality
Qualifications:
If an Advanced Practice Registered Nurse (APRN):
Hold a Master of Science in Advanced Practice Nursing from an approved program
Hold/obtain and maintain National APRN Certification
Hold/obtain and maintain APRN licensure in the State of Wyoming
If a Physician Assistant/Associate (PA):
Hold a Master of Science in Physician Assistant Studies from an approved program or 5+ years' experience as a PA
Hold/obtain and maintain board certification from the National Commission on Certification of Physician Assistants
Hold /obtain and maintain an unrestricted PA licensure in the State of Wyoming
Hold/obtain and maintain an unrestricted Wyoming Controlled Substance Registration from the Wyoming Board of Pharmacy
Hold/obtain and maintain unrestricted WY-designated DEA registration
Hold/obtain and maintain a current Basic Life Support certificate
Hold/obtain and maintain medical staff privileges at Campbell County Health
Have and maintain a strong understanding of the principles and practices of Internal Medicine

Set goals for each child's individual development and to document/discuss progress.
Document attendance and all activities and submit to the appropriate staff, any prescribed forms consistently, correctly, and promptly.
Complete and submit required forms and records in a timely and accurate fashion; daily schedule, time approvals, meal reports, etc.
Feed, eat with, and assist children in development of social and self-help skills, and sound nutritional practices.
Guide children's acquisition of social skills.
