Perform weekly skin monitoring and wound assessments in PCC
Review new admissions/readmissions for accurate wound data
Evaluate new/worsening wounds
Review pressure ulcer prevention measures
Recommend pressure-relief devices
Update care plans
Review treatment orders & interventions
Complete wound-related audits
Coordinate with interdisciplinary teams (dietitian, rehab, social work, nursing)
New Admissions:
Verify admission skin assessment within 24 hours
Add residents to the wound care provider list
Note: ADON oversees the overall wound program (tracker, new admits, new wounds).
Qualifications:
Nursing degree or approved nursing school graduate
Current NYS RN license
Wound Care Certification preferred
Long-term care experience strongly preferred
Prior wound care experience in a healthcare setting preferred
