Provides support for medical claim and internal appeals review activities - ensuring alignment with applicable state and federal regulatory requirements, Molina policies and procedures, and medically appropriate clinical guidelines. Contributes to overarching strategy to provide quality and cost-effective member care.
Job Duties
Facilitates clinical/medical reviews of retrospective medical claim reviews, medical claims and previously denied cases in which an appeal has been made, or is likely to be made, to ensure medical necessity and appropriate/accurate billing and claims processing.
Reevaluates medical claims and associated records by applying advanced clinical knowledge, knowledge of relevant and applicable state and federal regulatory requirements and guidelines, knowledge of Molina policies and procedures, and individual judgment and experience to assess the appropriateness of services provided, length of stay, level of care, and inpatient readmissions.
Validates member medical records and claims submitted/correct coding, to ensure appropriate reimbursement to providers.
Resolves escalated complaints regarding utilization management and long-term services and supports (LTSS) issues.
Identifies and reports quality of care issues.
Assists with complex claim review including diagnosis-related group (DRG) validation, itemized bill review, appropriate level of care, inpatient readmission, and any opportunities identified by the payment integrity analytical team; makes decisions and recommendations pertinent to clinical experience.
Prepares and presents cases representing Molina, along with the chief medical officer (CMO), for administrative law judge pre-hearings, state insurance commissions, and judicial fair hearings.
Reviews medically appropriate clinical guidelines and other appropriate criteria with medical directors on denial decisions.
Supplies criteria supporting all recommendations for denial or modification of payment decisions.
Serves as a clinical resource for utilization management, CMOs, physicians and member/provider inquiries/appeals.
Provides training and support to clinical peers.
Identifies and refers members with special needs to the appropriate Molina program per applicable policies/protocols.
Job Qualifications REQUIRED QUALIFICATIONS:
At least 2 years clinical nursing experience, including at least 1 year of utilizationreview, medical claims review, long-term services and supports (LTSS), claims auditing, medical necessity review and/or coding experience, or equivalent combination of relevant education and experience.
Registered Nurse (RN). License must be active and unrestricted in state of practice.
Experience demonstrating knowledge of ICD-10, Current Procedural Technology (CPT) coding and Healthcare Common Procedure Coding (HCPC).
Experience working within applicable state, federal, and third-party regulations.
Analytic, problem-solving, and decision-making skills.
Organizational and time-management skills.
Attention to detail.
Critical-thinking and active listening skills.

Effective verbal and written communication skills.

PREFERRED QUALIFICATIONS:

Licensed Practical Nurse (LPN) Responsibilities:
Partners with team members in the development and implementation of clinical treatment plans under the clinical supervision of providers and registered nurses and the direction of team members; includes active engagement and participation in all team meetings.
Supports wrap-around clinical services in the home- and field setting, including delivery of care through co-visits with other team members, assisted telehealth visits with providers and other team members, and independent LPN visits.
Provides clinical services and clinical coordination services as directed by a patient's clinical risk assessment and clinical care plan and under the clinical supervision of providers, Practice Managers, and RN Case Managers.
Conducts health screenings for newly enrolled patients and on an ongoing basis.
Performs clinical services in the field, including but not limited to wound care dressing changes, phlebotomy, medication review, and patient status checks.
Meets with unable-to-reach patients in their home setting to re-engage patients in the program.
Provides cross-coverage for planned and unplanned absences for team members to ensure that patient care is not interrupted.
Partners with interdisciplinary team members to continuously improve the quality of care, reduce the cost of care, and improve both patient and staff satisfaction.
Licensed Practical Nurse (LPN) Requirements:
Licensed Practice Nurse, license certification in the state of employment
Experience providing care for the whole person, including patients living with mental illness and substance use disorders.
Current CPR and BLS certification required
Average to advanced computer & software skills (Microsoft Word, Excel, Outlook)
Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

Ensures all relevant data including physician orders, lab results, vital signs and treatment parameters, and patient status are documented appropriately and entered into Medical Information System.

Ensures all FMCNA policies regarding patient admission, transfer, and discharge are appropriately implemented.
Ensures and verify accuracy of Patient Care Technician documentation.
Daily
Reviews and ensures appropriate daily completion of Hemodialysis Treatment Sheets by all patient care staff.
Ensures that all appropriate procedures are followed regarding opening and closing procedures, inclusive of monitoring that all staff and patients have safely left the premises.
Monthly
Initiates, documents, and completes ongoing Continuous Quality Improvement (CQI) activities including monthly reports.
Completes monthly nurses' progress note.
Ensures patient medical records are complete with appropriate information, documentation, and identification on each page (Addressograph label is on all chart forms).
Reviews transplant status and follows established procedure regarding appropriate action to be taken.
Completes patient care plans for new patients within the initial 30 days or any patients deemed unstable requiring monthly patient care plans.
Completes any long-term programs that are due. Annually
Completes initial and annual Nursing History and Assessment physical.
Ensures completion of Annual Standing Order Review with each physician as required.

JOB DESCRIPTION:
School Registered Nurse (RN)
QUALIFICATIONS:
Diploma and certification as Registered Nurse (RN)
One year comprehensive nursing experience
School nursing experience preferred
Transportation Availability as may include positions requiring itinerant travel
PERFORMANCE RESPONSIBILITIES:
Administers first aid according to established procedures; conduct a variety of health screenings; prepares and maintains accurate student medical records.
Administers first aid or nursing care for accidents, seizures, preventive medicine and educational-related medical issues to all stakeholders as necessary.
Conducts height, weight, vision, hearing, scoliosis and other health screenings as required; provide screening for head lice and observe students on a regular basis to detect health needs.
Refers students to appropriate personnel, resources or clinics and follow-up as necessary.
Maintains health records and medical logs; updates immunization and student records as needed; prepare and submit periodical medical reports for appropriate organizations or District personnel and assures compliance with established laws and regulations.
Dispenses and retains medication and performs medical treatment in accordance with physician's instructions within District guidelines.
Communicates with various community health agencies and civic organizations to exchange information, coordinate activities and obtain student needs such as medication, glasses, hearing aids and clothing as needed.
Conducts training for students, parents and school staff as permitted and as required.
Provides information and make recommendations to principals and district administrators regarding health issues of students and school health program needs, develop programs to meet school health needs.
Follows all appropriate state and federal laws as well as district policies and procedures.
Utilizes technology including the student information system to maintain records.
Makes home visits as required; authorize re-admission to school.
Maintains stock of first aid supplies; assures medical equipment is in proper working condition; orders supplies and equipment as needed.
Participates in screening for health defects and sanitary conditions in schools.
Maintains current knowledge of medical advances and information, public health issues and treatment procedures.
Provides health care teaching-preventive and therapeutic-appropriate to educational level (and language barriers)
Provides case management by acting as a liaison between parent/guardian and physician responsible for the student's health condition. Participates as student/family advocate.
Works independently following district guidelines
Attends and provides input at ARC & 504 meetings of students with medical needs, as necessary
Delegates health responsibilities as permitted
Performs related duties as assigned any other job assigned by principal, director of student services, superintendent, and/or designee

Days and salary pursuant to district salary schedules.
JOB REQUIREMENTS:
Strictly adheres to the Kentucky Certified Code of Ethics (704 KAR 20:680) expected for all certified and classified personnel
Work is performed while standing, sitting and/or walking
Requires the ability to communicate effectively using speech, vision, written language, and hearing
Requires the use of hands for simple grasping and fine manipulations

ESSENTIAL JOB DUTIES:
* Facilitates clinical/medical reviews of retrospective medical claim reviews, medical claims and previously denied cases in which an appeal has been made, or is likely to be made, to ensure medical necessity and appropriate/accurate billing and claims processing.
* Reevaluates medical claims and associated records by applying advanced clinical knowledge, knowledge of relevant and applicable state and federal regulatory requirements and guidelines, knowledge of Molina policies and procedures, and individual judgment and experience to assess the appropriateness of services provided, length of stay, level of care, and inpatient readmissions.
* Validates member medical records and claims submitted/correct coding, to ensure appropriate reimbursement to providers.
* Resolves escalated complaints regarding utilization management and long-term services and supports (LTSS) issues.
* Identifies and reports quality of care issues.
* Assists with complex claim review including diagnosis-related group (DRG) validation, itemized bill review, appropriate level of care, inpatient readmission, and any opportunities identified by the payment integrity analytical team; makes decisions and recommendations pertinent to clinical experience.
* Prepares and presents cases representing Molina, along with the chief medical officer (CMO), for administrative law judge pre-hearings, state insurance commissions, and judicial fair hearings.
* Reviews medically appropriate clinical guidelines and other appropriate criteria with medical directors on denial decisions.
* Supplies criteria supporting all recommendations for denial or modification of payment decisions.
* Serves as a clinical resource for utilization management, CMOs, physicians and member/provider inquiries/appeals.
* Provides training and support to clinical peers.
* Identifies and refers members with special needs to the appropriate Molina program per applicable policies/protocols.
REQUIRED QUALIFICATIONS:
* At least 2 years clinical nursing experience, including at least 1 year of utilizationreview, medical claims review, long-term services and supports (LTSS), claims auditing, medical necessity review and/or coding experience, or equivalent combination of relevant education and experience.
* Registered Nurse (RN). License must be active and unrestricted in state of practice.
* Experience demonstrating knowledge of ICD-10, Current Procedural Technology (CPT) coding and Healthcare Common Procedure Coding (HCPC).
* Experience working within applicable state, federal, and third-party regulations.
* Analytic, problem-solving, and decision-making skills.
* Organizational and time-management skills.
