Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky's talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to individuals served.
The Assistant Program Coordinator takes on a leadership role working alongside Residential Counselors to provide direct support to people served and direct supervision to the overnight staff. They also provide administrative assistance to the Program Coordinator such as assisting with program scheduling and budgets.
Other Key Responsibilities:
Complete required documentation, assist with group therapy, transportation, behavioral intervention, role modeling, and recreational activities.
Implement activities that facilitate the development of valued roles and personal relationships in the community.
Develop and implement all habilitation, whole life, and other service plans.
Participate in on-call rotation.

Many changes have been made as the needs of our students change and grow. This building, however, still serves its original purpose and offers itself as a place where students can spend time socializing and expanding themselves outside the classroom. Hammond Hall houses everything from offices and services for students to the Gallucci-Cirio Library and the North Street Bistro.
The Falcon Community Outreach Center is an area of the Campus Center. The Staff report to the Manager of the Campus Center, and the Office of CARE and Case Management. Duties include, but are not limited to, assisting with community outreach, customer service, and maintaining the Falcon Bazaar Food & Necessities Pantry and Professional Clothing Closet.
Requirements:
Staff will provide friendly customer service to patrons, including maintaining a professional appearance. Staff will assist patrons of the Falcon Bazaar Food and Necessities Pantry and Professional Clothing Closet to locate, select, and sign-out items. Staff will provide support to maintain accurate records of patron visits and inventory of items. Staff will also complete tasks to maintain bazaar stock and tidiness, including moving and unpacking boxes, and organizing shelves and clothing racks. Each staff member will attend bi-weekly/monthly 1:1 meetings with supervisors, and monthly in-service staff training sessions. Staff will be expected to support volunteer center initiatives by offering programming ideas to supervisors, promoting services and upcoming events to patrons, and attending events when possible. Staff will complete other duties as assigned. Good computer and typing skills, as well as effective verbal and written communication skills are needed. Staff will also be expected to lift and move objects up to 25 lbs.
Additional Information:
This is a part-time, non-benefited, student only position. Schedule varies not to exceed 20 hours per week and the hourly rate is$15.00.

The Transitional Assistant Program Manager will oversee participants transitioning from skilled nursing facilities to community living with appropriate supports. These services assist adults with developmental disabilities, individuals who are blind or visually impaired, and adolescents with brain injury. The TA Coordinator will assist in the execution of service plans designed to develop, maintain, and/or maximize the participant's independent functioning in self-care, physical and emotional growth, socialization, communication, and vocational skills.
Requirements:
Have a college degree (preferably in a human service field), plus experience in providing community-based services to individuals with disabilities; or at least 2 years comparable, community-based, life or work experience providing services to individuals with disabilities and/or persons 65 years or older
Valid driver's license, clean driving record, and a reliable clean, insured vehicle that has had manufacturer's recommended scheduled services and transports client as appropriate.
Ability to work flexibly to meet the occasional requirements to work outside normal hours.
Maintain confidentiality of sensitive information
Utilize effective time management techniques to ensure tasks are prioritized, and deadlines are met.

Reviewing team billable hours and provide guidance for maximization.
Serving as the Guardian Angel Ambassador/Liaison in asking questions and seeking guidance from MassAbility.

Assisting with housing search (interviews and tours), voucher, and housing applications
Assisting with core documents applications required by housing authorities, such as an ID or Social Security Card
Setting up apartments as preferred by participants.
Maintaining active communication with MassAbility, ASAP and SNF case managers with updates on participants and attending transition meetings prior to participant discharge
Tracking and documenting direct services, housing application, shopping expenses and all communications related to participants.
Coordinating with MassAbility. CM on home modifications needs
Initiating home assessments on housing space for suitable housing goods and furnishing
Purchasing furniture, housing goods and all essential items based on Participant preferences.
Setting up utilities services as needed
Adequately updating service activity log, and submitting reimbursements in a timely fashion.
To assist in the development of information concerning care plans.
Arranging transportation under TA plan
Identify opportunities for SNF education on other services beneficial to all resident types provided by Guardian Angel.

Your Role: A Supporter and Builder
As the Volunteer Coordinator at Alyssa's Place, you will play a key role in nurturing a strong, connected peer recovery community. Your work will help volunteers and peer leaders discover their strengths, overcome barriers, and reach their goals.
What You'll Achieve
Support Volunteer and Peer Leader Development
Recruit, train, supervise, and schedule volunteers and peer leaders from our member community.
Provide mentorship, guidance, and encouragement to volunteers working toward peer leadership roles.
Identify barriers to growth and work collaboratively with volunteers to develop personalized plans for success.
Supervise the Peer Leader Supervisor, offering feedback and support to foster their professional development.
Provide Direct Member Support
Offer one-on-one recovery support services and recovery coaching to members as needed.
Collaborate with members, volunteers, peer leaders, and the Assistant Program Director (APD) to create weekly and monthly activity schedules chosen by the member community.
Manage the Volunteer Stipend Program
Coordinate with the Program Director and agency business office to ensure accurate reporting and proper distribution of stipends.
Strengthen Community Connections
Represent Alyssa's Place PRSC professionally in the community and participate in outreach efforts, events, and collaborations that promote recovery and member engagement.
Accompany members on activities and outings, providing transportation and supportive oversight.
Support Center Operations
Attend all required meetings and trainings.
Perform other duties as assigned by the Program Director.
