Job Description
Assist with the instruction of preschool and school-aged children in activities designed to promote social, physical, and intellectual growth needed for primary school in a child care center. This position requires North Carolina state certification (NC Qualifing Letter)
Essential Functions:
Fills in classrooms as needed and provides lunch breaks as dictated by the schedule of the staff. Encounters children of a variety of ages from six weeks to 12 years.
Enthusiastically greets and engages with parents at drop-off and pick-up times.
Maintain a positive relationship with parents and staff.
Inputs each child's daily activities into an interactive app for parents in a consistent, timely and accurate manner throughout the day, as needed.
Create a soft, calm, safe and loving environment.
Implement daily scheduled activities for children, with a focus on Language Development, Social Skills, and Gross Motor/Coordination Skills.
Ensure the daily care of every child by following all licensing guidelines and implementing all ACWLC company standards.
Clean and organize classrooms as directed.
Works cooperatively with and assists Teachers and other staff members.
Works effectively with ACWLC Corporate personnel members and implements strategic tactics as indicated.
Adhere to all ACWLC policies as set forth in the Team Orientation and Personnel Handbook.
Qualifications
A High School Diploma or equivalent. Associate's Degree in Early Childhood Education or related field preferred.
Experience in a licensed child care facility preferred.
Must meet licensing, education, and degree requirements established by the state of North CarolinaNC Qualifying Letter Preferred

Provides information on child care options; assists clients in arranging child care placement in the program of the parent's choice.

Location: This is a field-based position. Regional Specialty Provider Liaisons must reside and work within the aligned Department of Social Services (DSS) administrative region. This position will support the following communities in DSS Region 3.
* DSS Region 3 includes - Alamance, Caswell, Chatham, Davidson, Davie, Durham, Forsyth, Guilford, Orange, Person, Randolph, Rockingham, Stokes, Surry, and Yadkin.
Travel: Travel within the assigned Region is required.
Field: This field-based role enables associates to primarily operate in the field, traveling to provider sites or designated locations within their region(s) as the role requires, with occasional office attendance for meetings or training. The Regional Specialty Provider Liaison (CFSP) is a field-based role responsible for spearheading initiatives with specialty providers serving the child welfare population. Responsibilities include but are not limited to working with providers and internal teams to ensure timely provider contracting, education on specialty topics, developing value-based payment strategies, and aligning providers with quality and population outcomes. This position collaborates with each of the 100 Department of Social Services (DSS) and service providers across the state to meet the needs of children and families involved with child welfare services. This position works directly with our network of providers to identify and align with CFSP and NCDSS shared priorities and strategic visions. Primary duties include:
* Assist with provider contracting efforts.
* Assist with implementing incentives and value-based arrangements.
* Inform, develop, and execute strategy to address provider access and gaps in treatment.
* Provider support including implementation of specialty training, identifying outcomes, consultation on complex needs and treatment models, and identifying opportunities to increase quality and service capacity.
Minimum Requirements:
* BA/BS degree and a minimum 3 years of related experience; or any combination of education and experience, which would provide an equivalent background.
Preferred Skills, Capabilities, Experiences and Qualifications:
* Any combination of experience providing or implementing mental health, physical health, substance use, behavioral health, pharmacy, intellectual/ developmental disability (I/DD) services, and long-term services and supports are preferred.
* Master's degree in counseling, social work, or health/human services preferred.
* Clinical licensure preferred. (i.e. LCSW, LCMHC, LCAS, etc)
* Experience building relationships with Department of Social Services (DSS), service providers, and other stakeholders preferred.
* Experience working in payer space directly with providers preferred.

Essential Functions:
Functions independently to perform age-appropriate, disease specific history and physical examination of complex acute, critical, and chronic advanced heart failure patients.

Manages care and treatment plan as the front line provider for their patients

BLS for Healthcare Provider from AHA required with expiration date greater than 90 days from date of beginning practice. Position:
This APP position will be a hybrid role to include both inpatient and outpatient coverage, offering a varied patient population. When outpatient, the APP will have their own clinic template with a major focus on long-term management of our large population of enterally tube fed patients (i.e gastrostomy tube, gastrojejunostomy tube), and patients with general gastroenterology conditions including gastroesophageal reflux disease, constipation, abdominal pain, failure to thrive, and milk protein intolerance. While inpatient, the APP will see hospitalized patients in collaboration with the gastroenterology attending physicians. The inpatient GI service is primarily a consultation service offering recommendations to the primary team (general pediatrics, PICU, Cardiology, NICU or hematology/oncology), but at times it has primary patients. The candidate will have excellent mentorship from current experienced GI APP's and supportive supervising physicians.

* Have Physician orders signed by the Primary Physician (yearly with annual)
* Complete labs as ordered by physician or as requested
* Have Physician Orders signed by Psychiatric Physician (upon each visit)
* Any Medical changes need to be updated on the Physician Orders including (Medication changes, Adaptive Equipment, diets, allergies etc.)
* When labs are completed; follow-up and obtain results from Physician (Psychiatric or Primary)
* Complete annual visits and follow up requirements (get clarification from doctor if not sure)-Physical, Hearing (Not required unless hearing issues/concerns), Dental, Vision
* Limited travel
* All other duties as assigned

* High School Diploma or GED Equivalent
* Training in the CAP Core Competencies
* Valid driver's license, own reliable vehicle, and good driving record
* Ability to communicate (verbally and written)

A Host Home provider is an independent contractor who opens their home and heart to an adult with a development disability. Depending on the individual, supports will likely include support with personal care and hygiene, meal planning and cooking, medications, recreation, and leisure activities in and out of the home, transportation, shopping and more.
Responsibilities
Receive the Individual into the Provider's home and to provide the Individual with appropriate daily care, activity, and full-time supervision in accordance with applicable laws, the Individual's Service Plan and with due regard for the Individual's emotional, intellectual, physical, and spiritual needs.
Prevent any acts of disrespect, abuse, neglect, or exploitation of the Individual receiving services from the Provider and to immediately report any such acts to the company and the applicable state/local agencies.
Document the Individual's progress and unusual events or occurrences. To assist the company in meeting the company's obligations under state and federal law and regulation, Provider shall complete and submit reports to the company.
Arrange and establish routine or emergency medical and dental care for the Individual consistent with the Individual's Service Plan and with due regard for the Individual's physical and emotional needs.
Provide transportation for the Individual to service appointments and /or community activities, consistent with the Individual's Service Plan.
In the event of serious illness, serious injury, or death, to immediately notify the company's Clinical Supervisor or appropriate on-call staff as soon as possible and after emergency medical or dental care is obtained for the Individual.
Communicate fully, promptly, and regularly with the company about matters affecting the care or placement of the Individual, as provided by state law and regulations and the Independent Contractor Agreement.
Maintain all information and records (including logs, notes, etc.) regarding an Individual in a confidential and privileged manner at all times.
Complete daily notes/grids-thoroughly
Attend ISP, IEP, or BSP meetings monthly or as scheduled
Be on time to all meetings, appointments
Call with 24 hours-notice if cancelling or rescheduling meetings/visits
Help on the job train Relief providers/staff PRIOR to first day of relief
Turn in necessary training documentation for compliance (insurance, trainings, etc.)
Report new or changed information to QP (such as frequent visitors, family members in home, legal info, medical changes, behavioral changes, etc.)
Review new MARS and Physicians Orders with the previous months and making any necessary changes when received from Pharmacy, contact your QP if issues/corrections/changes are needed or for assistance with documenting changes on new MAR
