The RN CaseManager provides patient care management across the continuum of care. The RN CaseManager is responsible for ongoing assessment of patient needs and coordination of the delivery of patient care through the unit manager, Social Worker, and the multi-disciplinary health care team. The RN CaseManager is accountable to ensure patients meet criteria for admission, continued stay, and discharge. They focus on facilitating the development of discharge plans and the appropriate movement of patients from one level of care to another. In collaboration with the Social Workers, they insure a smooth transition to other levels of care. The RN CaseManagers function as a liaison with insurers to advocate for patients based on defined needs. There is a significant focus on patient care outcomes and the RN CaseManager oversees the evaluation of outcomes (treatment and financial outcomes). The practice of the RN CaseManager is highly collaborative with physicians and other health care team members. They are involved in quality improvement utilizing trended data to focus on issues identified for improvement. RN CaseManagers are teachers to patient/families and members of the multi-disciplinary team. RN CaseManagers are accountable for the training related to the principles of care and casemanagement. They assist the Social Worker with the complicated medical issues. The RN CaseManager is responsible for completing Interqual Reviews for initial and concurrent reviews.

This position is the supervisory position for professional field staff. Licensed Addiction Counselor (LAC)
Providence House is seeking a dedicated and compassionate Licensed Addiction Counselor (LAC) to join our professional team. The LAC will be instrumental in implementing and managing comprehensive addiction treatment programs, aligning services with our organization's mission and adhering to all regulatory standards.
About Providence House: Providence House offers a range of specialized addiction treatment services, including adult residential care, outpatient programs, individualized and group counseling, comprehensive drug and alcohol evaluations, and personalized treatment planning.
Responsibilities and Duties:
Conduct comprehensive patient assessments, documenting substance use history and evaluating physical, mental, emotional, social, and spiritual impacts to inform treatment recommendations.
Develop and effectively implement individualized treatment plans tailored to each patient's unique needs.
Coordinate with healthcare providers and insurance companies, facilitating clear communication of observations, treatment recommendations, and patient progress evaluations.
Regularly monitor patient adherence to treatment plans, assessing goal achievement and making necessary adjustments.
Facilitate educational sessions and group counseling focused on addiction and recovery.
Maintain accurate, detailed, and timely documentation of assessments, evaluations, and therapeutic notes.
Obtain necessary releases of information, ensuring compliance with federal confidentiality regulations (42 CFR) when communicating with clients, families, guardians, and healthcare providers.
Regularly evaluate program effectiveness against established goals and implement improvements as necessary.
Participate actively in daily clinical team meetings to collaboratively evaluate and track residents' treatment progress.
Fulfill on-call responsibilities as required by the role.
Qualifications:
Current licensure as an Addiction Counselor in the state.
Proven experience in addiction counseling and treatment planning.
Strong organizational skills, attention to detail, and ability to maintain confidentiality.
Excellent interpersonal and communication skills, both written and verbal.
Ability to work collaboratively within a multidisciplinary team.

Women in Recovery Casemanager provides gender-specific advocacy, assessment, linkage, referral, education, resource acquisition, family, group, monitoring and crisis intervention services to women and their families in the Women in Recovery (WIR) program in a culturally sensitive way. Provides a single point of contact and work in partnership with WIR staff and court system personnel as part of a multidisciplinary team to provide a set of social service functions that helps women at risk of incarceration access the resources they need to recover and avoid incarceration. Provides and advocates for gender-specific therapeutically effective best practices, services, incentives, and sanctions.
Requirements
Oklahoma level II CaseManagement certification or willingness to obtain within the first 60 days of employment.
Minimum one-year experience in the mental health/substance abuse field.
Must possess substance abuse related knowledge, skills, and attitudes in order to provide treatment-specific services.
Ability to establish rapport quickly, establish appropriate boundaries and possess non-judgmental attitude toward clients.
