JOB SUMMARYThe Licensed Medical Provider (LMP) delivers comprehensive addiction medicine and primary care services inpatient withdrawal management. This role provides medical evaluation, stabilization, and longitudinal care for patients with substance use disorders and co-occurring health conditions. The provider works as part of an interdisciplinary team including physicians, advanced practice providers, RNs, LPNs, CADCs, QMHPs, and CHWs to ensure high-quality, integrated medical and behavioral healthcare.
The position includes scheduled on-site and on-call responsibilities one weekend per month, conducting admission evaluations, issuing orders, medication management, and urgent medical coverage for withdrawal management clients.
ESSENTIAL FUNCTIONS
Conduct admission histories and physicals for detox and residential patients; complete comprehensive assessments for outpatient AMC patients.
Initiate, manage, and adjust treatment plans for substance use disorders, including buprenorphine, methadone coordination, naltrexone, and long-acting injectables under REMS/DEA guidelines.
Provide daily and as-needed medical oversight for patients in withdrawal management and early stabilization.
Manage acute and chronic medical conditions, including Hepatitis C, contraception counseling, STIs, wound care, and common primary care concerns.
Order, interpret, and review laboratory results, diagnostic studies, and urine toxicology screens; utilize the Oregon PDMP as clinically indicated.
Formulate and document individualized, evidence-based care plans with patient participation and informed consent.
Provide education and counseling on recovery principles, harm reduction, and medication-assisted treatment options.
Support and consult with nursing and counseling staff regarding medication management and medical conditions in both inpatient and outpatient settings.
Administer or oversee the administration of subcutaneous and intramuscular medications following clean technique and best practices.
Coordinate care transitions between detox, residential, and outpatient settings, ensuring safe discharge and follow-up.
Maintain accurate, timely documentation in the EHR, including diagnoses, levels of service, and procedure coding.
Participate in the monthly on-call rotation (approximately one weekend per month, additional shifts available if desired) to provide withdrawal management coverage, including telephonic support and urgent medical consultation.

QUALIFICATIONS
Education & Experience:
Medical degree (PA, NP, MD, or DO) from an accredited program.
Minimum one year of experience in addiction medicine, withdrawal management, or dual-diagnosis care.
Strong foundation in adult primary care and chronic disease management.
For individuals in recovery, at least two (2) years of continuous sobriety prior to hire.
Licensure & Certification:
Current Oregon license to practice medicine.
Current BLS/CPR certification.
DEA registration and authority to prescribe controlled substances in Oregon.
Preferred Qualifications:
Experience in both outpatient and residential treatment settings.
Bilingual English/Spanish preferred.
Familiarity with Epic or similar EHR systems.

POSITION OVERVIEW: Neonatal Nurse Practitioner or Physician Assistantprovides leadership during resuscitation at birth of both term and preterm infants. FUNCTIONS AND DUTIES:
Clinical Duties:
Assists in monitoring the effectiveness and quality of patient care with primary focus on neonatal intensive care management, newborn delivery and resuscitation, and neonatal transport.
Provides effective, efficient, appropriate care, with a central focus on infant evaluation, management of acute and ongoing care, and development of treatment plans.
Performs health assessment of at risk infants at the time of delivery.
Performs health assessment of infants admitted to the NICU, completes admission H and P.
Determines normal vs abnormal findings via exam and/or diagnostic tests.
Admits infants to NICU as appropriate.
Acts appropriately and timely in addressing urgent care needs of infants both at the time of delivery and on-going in the NICU - including initiating full resuscitations when appropriate.
Develops plan of care for infants in the NICU consistent with approved protocols - makes adjustments to plan based on changes in infants condition.
Prescribes treatments, medications, respiratory care and miscellaneous care instructions as necessary.
May discharge or transfer infants from NICU - consults with discharge Pediatrician as necessary.
Performs pre-delivery consults as requested by OB services.
Manages newborn transition support as needed.
Management of patients in the NICU may include daily physical exams, evaluation of progress, documentation via progress and procedure notes, rounds with attending physician and other team members, and writing orders.

May participate in ongoing outreach education, simulations and presentations to hospitals in and out of system and EMS units on request. Opportunities exist to engage in EPIC specific optimization related to NICU and other I/T trouble shooting as needed. May participation in NICU/Peds committees and high-risk developmental clinic, and general on-going NICU education- i.e. chart reviews, journal club, article reviews, lectures, case reviews as requested by the Medical Director or NICU Leadership team.

JOB SUMMARYThe Licensed Medical Provider (LMP) delivers comprehensive addiction medicine and primary care services inpatient withdrawal management. This role provides medical evaluation, stabilization, and longitudinal care for patients with substance use disorders and co-occurring health conditions. The provider works as part of an interdisciplinary team including physicians, advanced practice providers, RNs, LPNs, CADCs, QMHPs, and CHWs to ensure high-quality, integrated medical and behavioral healthcare.
The position includes scheduled on-site and on-call responsibilities one weekend per month, conducting admission evaluations, issuing orders, medication management, and urgent medical coverage for withdrawal management clients.
ESSENTIAL FUNCTIONS
Conduct admission histories and physicals for detox and residential patients; complete comprehensive assessments for outpatient AMC patients.
Initiate, manage, and adjust treatment plans for substance use disorders, including buprenorphine, methadone coordination, naltrexone, and long-acting injectables under REMS/DEA guidelines.
Provide daily and as-needed medical oversight for patients in withdrawal management and early stabilization.
Manage acute and chronic medical conditions, including Hepatitis C, contraception counseling, STIs, wound care, and common primary care concerns.
Order, interpret, and review laboratory results, diagnostic studies, and urine toxicology screens; utilize the Oregon PDMP as clinically indicated.
Formulate and document individualized, evidence-based care plans with patient participation and informed consent.
Provide education and counseling on recovery principles, harm reduction, and medication-assisted treatment options.
Support and consult with nursing and counseling staff regarding medication management and medical conditions in both inpatient and outpatient settings.
Administer or oversee the administration of subcutaneous and intramuscular medications following clean technique and best practices.
Coordinate care transitions between detox, residential, and outpatient settings, ensuring safe discharge and follow-up.
Maintain accurate, timely documentation in the EHR, including diagnoses, levels of service, and procedure coding.

DEPARTMENT SUMMARY: St. Charles Gastroenterology Services will employ Advanced Practice Providers with varying levels of experience as well as MD's board certified in Gastroenterology. There are community gastroenterologist and occasionally locums doing contracted work with the team as well.
POSITION OVERVIEW: Advanced Practice Providers (APPs) under the general supervision of the Clinical Division Director and direct supervision of a designated physician, provides medical and appropriate outpatient care to patients. Such care includes evaluation, diagnosis, treatment, and documentation of each care encounter. APPs will assist GI MD's by providing inpatient consults and rounding on department patients. Coordinates medical, ancillary, nursing, and other services. Monitor patients' on-going care plan and refer them to other departments based on their assessment of patients' progress and condition.
Work schedule: APP will work a minimum of 40 hours per week with a minimum of 36 hours patient contact time. Scheduled days may vary depending on clinic operating days and hours.
Call expectation: APP will be expected to participate in the clinic call rotation shared equally by all providers in the designated call group to include evenings, weekends, and holidays. This is scheduled in coordination with other providers in the call group.
ESSENTIAL FUNCTIONS AND DUTIES:
Clinical Expectations:
The Gastroenterology APP will work primarily in an outpatient setting with both adult and pediatric patients, generally 16 years and older.

POSITION OVERVIEW: Advanced Practice Provider, under the general supervision of the Clinical Division Director and direct supervision of a designated physician, provides medical and appropriate outpatient care to patients of all ages. Such care includes evaluation, diagnosis, treatment, and documentation of each care encounter. Coordinates medical, ancillary, nursing, and other services in the context of a Medical Home model to achieve the best and most cost-effective outcomes for patients under his/her care. Create treatment plans that incorporate Behavioral Health, RN Care Management, therapy, pharmacy, nutrition or surgery to manage patient illnesses and injuries. Monitor patients' on-going care plan and refer them to specialists based on their assessment of patients' progress and condition.
Work schedule: APP will work minimum of 40 hours per week with a minimum of 36 hours patient contact time.. Scheduled days may vary depending on clinic operating days and hours.
Call expectation: APP will be expected to participate in the clinic call rotation shared equally by all providers in the designated call group to include evenings, weekends and holidays. This is scheduled in coordination with other providers in the call group. A Family Care physician will provide back up and expertise support as needed by the APP.
DEPARTMENT SUMMARY: An APP is part of a team of dedicated professionals who provide care to patients and families in a Medical Home environment. Our goal is to deliver evidence-based and personalized care to patients based on their individual needs. The APP is supported in the delivery of comprehensive care by in-team Behavioral Health, Pharmacy, Care Managers, Community Health Educators and administrative staff.
DUTIES AND FUNCTIONS
Clinical Duties:
Practice and role model excellence in the Family Practice Medical Home domains of diagnosis, aligning treatment goals to patient values, definitive treatment, supportive care treatment, follow-up care, and preventive care.
Practice evidence based Family medicine within the scope of license, training and credentialing. Cooperate with other SCHS providers and caregivers to insure optimal patient care and outcomes.
Collaborate with other members of the SCMG Family Medicine,Specialty care provider teams and referring providers to ensure a world-class patient experience.
Provide a therapeutic presence in service to others by purposefully responding to the needs of patients/customers in a caring way. This includes introducing oneself and stating your purpose, asking the patient their preferred name, spending time with the patient to determine their care goals, active listening, and using skillful communications (appropriate touch, eye contact, etc.).
Prescribes medications in accordance with SCHS policies and procedures for medication administration.
Consults with Care Team Pharmacist on behalf of poly-pharmacy patients or regarding medication efficacy.
Consults with Care Team Psychiatrist regarding appropriate prescribing of psychotropic medications.
Accurately document, code and bill for all patient care in the EHR or on the appropriate form in a timely manner and in compliance with SCMG defined Standards.
Interpret and report on diagnostic tests in a timely manner in compliance with SCMG defined Standards. This should not exceed 72 hours for Urgent and 7 days for Routine diagnostics.
