Acts as HEDIS champion for the PCP by providing visibility of HEDIS needs for scheduled patients. Regulates and monitors aspects of transportation, including: Driver's lists/patients, medication deliveries, grocery deliveries, driver's behavioral challenges, scheduling and coordinating Uber/Lyft rides for patients.
