Certified Nursing Assistants at Masonicare at Ashlar Village assist residents with activities of daily living and personal care to maximize independence, freedom of choice and quality of life. The official title for this role is “Residential Care Assistant”.
CNA - Essential Duties and Responsibilities:
1. Provides assistance with essential activities of daily living and personal care to residents based on age, needs and abilities of the resident, including bathing, dressing and undressing, ambulating, toileting, assistance with meals and nourishment's, personal hygiene (shaving, dental hygiene, nail, hair and foot care), transferring using proper body mechanics and appropriate assistive devices and care of ADL devices such as eyeglasses and hearing aids.
2. Provides assistance with instrumental activities of daily living to residents based on age, needs and abilities of the resident: placing and/or receiving telephone calls, performing light housekeeping tasks such as vacuuming, emptying trash, cleaning bathroom, bed making and bed changes, assisting with laundry, escorting the resident and coordinating internal transportation needs and assisting with mail delivery and correspondence
3. Assists the resident with self-administration of medications, through cueing only
4. Observes and reports changes in the residents' physical condition and cognitive/emotional status to ALSA Supervisor or Director of Assisted Living Services, as needed
5. Conducts room checks on resident rounds, monitors for environmental safety hazards, records incidents, errors or accidents and records resident and resident related events.
6. Assists in a variety of tasks related to dining (escorting residents, set-up and clean-up, serving meals)
7. Responsible for participating in and supporting the resident centered activity program. Completes required written documentation concerning resident care and activities during shift
8. Is a resident advocate at all times
9. Follows agency policies concerning confidentiality
10. Uses tactful, diplomatic communication in potentially sensitive or emotionally charged situations and follows up with appropriate staff, resident or others regarding reported
complaints, problems and concerns.
11. Acts as ambassador to guest and other off-campus visitors and maintains certifications required for employment
12. Performs other duties as required

Current Basic Life Support (BLS) certification (or ability to obtain before starting).
Urgent care or emergency room experience highly valued.
EMR proficiency (Experity preferred); comfort with payer portals (e.g., Waystar).
Proven venipuncture and specimen collection skills.

The Treatment Advocate (TA) will observe client's daily activities and assist in their recovery. Conduct patient belongings search when new patients are admitted.

Participate in coordination of patient's daily schedule including treatment meetings, behavior interventions, and overall general observation of behavior and level of participation.
Provide support and encouragement to patients.
Support and encourage patient participation in group, individual, and family counseling services.
Support clinical and nursing services of patients as assigned.
Participate in weekly treatment team meeting and program staff meetings.
Ensure that all documentation necessary meet contract, licensure, accreditation and standards for services in a timely manner following the AAC's documentation policy guidelines.
Communicate efficiently with other departments to ensure the best care for patient.
Provide supportive services to outpatient and inpatient adult population.
Identify situations which may be potentially harmful to patients and report any patient or safety concerns to Clinical Manager and/or CEO.
Understand and abide by State Department standards, facility accreditation, AAC's policy and procedures.
Adhere to company code of ethic and maintain professional boundaries with patients at all times.

Qualifications
Education/ Experience:
High school diploma or GED required from an accredited institution

Valid Driver's License required with insurable driving record
Working knowledge of vehicle safety and control systems.

Knowledge of utilizing electronic health record software and documentation preferred.

I. Major Responsibilities:
1. Provides patient care. Assesses health status, implements care plans, documents interventions and evaluates patient progress following standard procedures.
2. Coordinates patient flow and prepares patient for examination. Takes and records vital signs and EKG's. Documents intake questions. Assists physician and others in clinical procedures as required or directed. Performs other basic clinical procedures such as venipuncture.
3. Performs basic office laboratory procedures such as urinalysis, urine culture, , pregnancy tests, throat cultures and related procedures. Draws blood as required or directed.
4. Returns patient calls and documents in patient record as required or directed by the RN or physician.
5. Obtains laboratory results. Ensures results are brought to the attention of the physicians and others as appropriate.
6. Maintains records, reports, logs and other statistical information as required or directed.
7. Performs a variety of clerical duties such as typing, filing, scheduling appointments, making phone call and related duties as required or directed.
8. Maintains assigned departmental and/or clinical supplies. Assures accurate inventory of supplies based on approved levels. Rotates stock as required. Initiates requisitions for re-supply or special items.
9. Meets established productivity standards.
10. Facilitates and promotes the sharing of knowledge and content throughout departments.
11. Takes responsibility for ensuring that all work outcomes satisfy the UMass Memorial Health System True North.

1. Certification as a MedicalAssistant is required within 6 months of hire and must be maintained.

WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:
Observe and support residents in taking medications as prescribed.
Directly connect residents with providers and prescribers to initiate or continue medications.
Work with pharmacies and providers to coordinate refills and prevent interruptions in treatment.
Assist with lab samples and facilitate provider communication as needed.
Monitor medication adherence and notify Program Director, Clinical Director, or designee of any concerns.
Maintain accurate records of medications stored, administered, returned, or disposed of.
Track medications in the possession of residents and the program, ensuring proper custody.
Ensure safe return or release of medications at discharge.
Dispose of expired or discontinued medications in compliance with BSAS, state, and federal regulations.
Witness and document medication disposal on a regular schedule.
Ensure the medication refrigerator is in working condition and temperatures are recorded daily.
Check and replace expired facility-supplied medications, including first aid kits and naloxone (Narcan) rescue kits.
Maintain and organize the medication storage area.
Stock the medication room with required supplies (cups, counting trays, spatulas, etc.) and ensure supplies are cleaned regularly.
Orient new residents to the program's medication policies and procedures.
Coordinate annual naloxone training for staff and ongoing education on medication management, including scheduling external trainers as needed.
Document all medication-related activities accurately and timely.
Report medication errors, incidents, or concerns to supervisors and regulatory authorities (BSAS and others) as required.
Participate in audits, compliance reviews, and technical assistance provided by BSAS or partner agencies.

Responsibilities include panel management, scheduling, special projects management, and maintenance of supplies, as well as clinical tasks such as telephone and pre-visit triage, vital signs, pre-visit planning, room cleaning and stocking, record reviews, referrals, and EMR documentation. The position supports the patient centered medical home model of care (PCMH) and oversees specific population health interventions. The Psychiatric Clinical Coordinator will draw on specialized knowledge developed by working with particular populations under a provider in order to serve as a resource for other Psychiatric Clinical Coordinators. In addition, other duties may be assigned as required by behavioral health and broader FHCW management teams.
Program Specific Essential Duties and Responsibilities:
Serves as a resource for the entire psychiatric services team, particularly in regards to any specialized patient populations served by their provider.
Assists with special projects being developed by the psychiatric services team.
Attends regular team meeting with psychiatric providers to discuss high-risk cases and develop strategies for delivering patient-centered and trauma-informed care.
Serves as primary point of contact and care for patients between visits via direct telephone line provided to all patients.
Ensures all scheduling needs are met for their assigned provider(s).
Completes and documents clinical tasks at the request of the provider, including completing and tracking prior authorizations, sharing lab results, and making medication-related calls to patients or relevant third parties.
Tracks follow up and health maintenance needs for all patients on the assigned providers' panels to ensure proper care for at-risk populations.
Manages a database of patients requiring psychiatric consultation to ensure follow-up.
Engages patients in health coaching and motivational interviewing.
Provides patient advocacy and navigation for FHCW services and services in the community.
Sets up room with appropriate equipment supplies for visit such as psychotherapy, pelvic exam, suturing, incision and drainage.
Directs patients into the exam room in a timely fashion after registration to determine reason for visit.
Completes medication reconciliations.
Reviews information based on patient presentation and discusses appropriate medical concerns with nursing staff or the provider.
Maintains and prepares note templates based on the purpose of the encounter.
Maintains a repository of psychiatric clinical questionnaires, registration and intake forms, and consent documents in multiple languages and explains the purpose of all forms to the patient, emphasizing need for completion.
Demonstrates and documents quality control measures - glucometer- and procedures for point of care devices
Demonstrates and documents procedures for Infection Control for sterilization and cleaning.
Responds to patient complaints and escalates those requiring further action as appropriate.
