JOB SUMMARY:
As a Field Medical Case Manager at ForzaCare, you'll help injured workers navigate their recovery and return to work. You'll act as the central point of coordination, connecting the injured worker, medical providers, employers, and insurance carriers to ensure timely, transparent, and effective care management.
This role is ideal for licensed nurses or certified rehabilitation counselors. While prior experience in workers' compensation is strongly preferred, those who have it will find their background especially valuable in this role.
KEY RESPONSIBILITIES:
Coordinate care between medical providers, employers, insurance carriers, and injured workers.
Attend appointments with the injured workers, which may include visiting employers and injured workers at their place of employment.
Develop, document, and monitor individualized recovery goals and return-to-work plans.
Provide consistent communication and detailed progress reports to clients and stakeholders.
Ensure all case management work meets or exceeds customer and compliance requirements.
Build and maintain strong relationships with clients, providers, and internal team members.
QUALIFICATIONS:
Education & Licensure:
Active Registered Nurse (RN) or Certified Rehabilitation Counselor (CRC) license with associated college degree is required.
Additional certifications such as CCM, CIRS, or other case management credentials are preferred.
Must comply with all state-specific licensure and certification requirements.
Prior experience in workers' compensation case management is strongly preferred.
Valid driver's license, reliable transportation, and auto insurance with ability to travel to appointments.

Summary
The Johnson Beth-El College of Nursing and Health Sciences at the University of Colorado Colorado Springs will establish and maintain a pool of Lecturers from which future appointments to temporary, non-tenure track positions will be made. Appointments are part-time (less than 50% time), usually for only one course/lab section, and made on a semester-by-semester.
The position is responsible for teaching and supervising a clinical group of students at the clinical sites for the Department of Nursing. Specialties include Medical-Surgical, Pediatric (Skills lab/Simulation labs only), Maternal Child Care, Mental Health and Public Health.
Essential Functions
The duties and responsibilities of the position include, but are not limited to\:
The primary role of the Clinical Lecturer is to provide direct supervision and teaching to a cohort of clinical students in the clinical setting with a focus on patient safety
Clinical hours will vary depending on class. Range from 7 hours once or twice per week to 12 hours twice per week for a duration of 8 to 16 weeks
Commits to teaching all scheduled clinicals for the assigned class
Provides advanced communication to Lead Faculty when an emergency arises which conflicts with attending clinical
Selects and schedules student learning experiences to meet course and clinical objectives, links theory to clinical
Flexible with student learning experience based on patient availability; works with agency staff for alternate placement
Creative in use of alternate learning experiences which support meeting clinical objectives
Arranges and leads pre and post conferences with each clinical
Utilizes Active Learning Teaching Strategies
Informs students of their progress through formative and summative evaluations

Responsible for reviewing patient medical records to ensure they meet clinical, regulatory, and billing compliance standards. This role combines clinical expertise with a deep understanding of healthcare regulations, including those from the Centers for Medicare and Medicaid Services (CMS).
Conducts thorough reviews of clinical documentation during the patients' stay, often within 24-48 hours of admission, such as patient charts, care plans, and physician's orders, to ensure they are complete, accurate, and support the patient's initial and ongoing eligibility for hospice services.
Identifies missing, vague, or conflicting documentation regarding a patient's condition and treatment and works closely with physicians and other providers to gain clarity or obtain additional information.
Verifies that all records and billing practices adhere to applicable federal and state regulations, including CMS and HIPAA standards.
Analyzes audit findings to identify trends and patterns in documentation quality. Generates quality reports and presents findings to clinical staff and leadership to facilitate continuous improvement.
Educates clinicians and other staff on proper documentation procedures, compliance essentials, and best practices. May help develop training materials for staff development.
Assists in preparing for internal and external audits, including those from regulatory bodies like the Office of Inspector General (OIG), Accreditation Commission for Health Care (ACHC) & Colorado Department of Public Health and Environment (CDPHE).
WHAT WE ARE GOING TO LOVE ABOUT YOU:
Graduate of an accredited school of nursing.
5 years' experience in hospice or home health medical record review, medical terminology, and relevant regulations, including Medicare and Medicaid rules is preferred.
2 years' experience with electronic health records (EHRs) and auditing software is necessary. Proficiency with basic computer programs, including the Microsoft Office suite, is also required.
Excellent written and verbal communication skills are critical for reporting findings and providing feedback to staff.
Strong analytical and critical-thinking skills are needed to research complex coding and compliance issues.
Current Colorado RN license or a license from the state of primary residence, if participating in the NLC (Nurse Licensure Compact). OASIS D and ICD-10 certification is preferred but may be substituted for actual hands-on experience.

REWARDING WORK YOU WILL DO:
Provides assessment, supportive care and education to prospective patients and families regarding hospice services. Determines hospice appropriateness of the patient; assists in admitting patients to the hospice program. Acts as a liaison with assigned hospitals, facilities and physicians.
Assesses and counsels the patients and families during the initial interviews to properly determine the level of care as well as the physical and psychosocial needs of the prospective patient. Accurately gathers information and data, reviews charts, and records information. Provides education on hospice philosophy.
Efficiently and accurately completes all admission paperwork and initiates the clinical record through the EMR process demonstrating knowledge of the admission protocol.
Assesses and places orders for immediate medication needs. Oversees the placement orders for durable medical equipment, supplies and oxygen as needed.
Consults with staff of hospital, skilled nursing facility (SNF) or Assisted Living Facility (ALF) to determine discharge orders and to facilitate discharge to home, SNF or other residence. Interfaces with referring physicians, nurses, ancillary providers and discharge planners to facilitate a well-coordinated transition into hospice for patients and families.
Completes the full Comprehensive Assessment at admission.
Completes inventory of current medications, and determines which medications relate to the terminal illness.
Communicates pertinent patient/family information to the interdisciplinary team and follows through to facilitate a smooth transition.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
Minimum Experience: One to two years of experience in hospice palliative and/or home health care, medical/surgical, public health, oncology, acute care or home health.
Required License: Current Colorado RN licensure. Current CPR certification. Valid Colorado State driver's license and auto insurance.
PHYSICAL REQUIREMENTS:
Ability to lift minimum of 35 lbs.
