Provides assistance in gaining access to essential community resources. Provide ongoing support of consumers' individual recovery processes. Provide quality chemical dependency treatment according to program and agency requirements. Use appropriate motivational techniques to engage consumers in successfully completing their treatment. Perform counseling, case management, crisis intervention, client education, and reports/recordkeeping under the supervision of a licensed independent chemical dependency counselor or licensed professional clinical counselor. Contribute to development and implementation of an individual treatment plan collaboratively with each person and the treatment team. Continually monitor and evaluate each client's progress and recommend treatment plan revisions as indicated. Provide community referrals, information and coordinate care with all other professionals involved in the person's care. ?Maintain an awareness of community resources relevant to person's needs. Provide case management and individual skills training to clients as indicated in treatment plan. Facilitate group sessions or co-facilitate on topics relevant to recovery. Fully understand and use ASAM level of care to determine appropriate recommendations for people. Knowledge and understanding of the treatment of mental health and addiction as co-occurring disorders and ability to implement in care for people. Communicate with supervisor or designated back-up for clarifications of procedures or clinical assistance as needed. Obtain specimens for Urine Drug Screens per protocol. Complete documentation requirements and maintain quality, up-to-date clinical records, including billing in client records within 24 hours. Ensures that all records and documentation meet HIPAA, Medicaid, OMHAS and CARF standards. Fully understands and apply all relevant rules for treatment, documentation and billing. Participates in regular supervision, team meetings, staff in-services and ongoing training. ?Maintain all required hours for license renewal. Promote and embody the Racing for Recovery Lifestyles. Participate in community events as a representative of Racing for Recovery. Meet productivity standards. May facilitate support group meetings in accordance with the Racing for Recovery model.     Case Manager Next Step - Geauga & Lake Counties Full-Time $40,000/Year SCHEDULE: Full-time, determined on a case-by-case basis GENERAL STATEMENT OF DUTIES: Provides community-based case management and advocacy for at-risk youth and young adult (YYA) households facing homelessness. ESSENTIAL RESPONSIBILITIES: 1. Outreaches at local programs/agencies to ensure effective local outreach and coordination. 2. Assesses needs, risks, and obstacles applicable to household stability and long-term permanent housing. 3. Assures the provision of financial assistance as needed and requested. 4. Assists participants in developing realistic and achievable housing plans that focus on housing stability and include measurable action steps and timetables. 5. Maintains regular contact with the household to provide crisis intervention, referrals, and ongoing life skills development. 6.
