The Youth CaseManager (CPST) provides an array of services to the community and individuals, including comprehensive case coordination, assisting youth, adults, and their families with skills building, crisis management, and providing linkages to community resources such as housing, transportation, vocational, medical, and financial services. Services are typically delivered in the community (mobile). The CaseManager works under the supervision of the LPC/LPCC/LSW/LISW to provide services. RESPONSIBILITIES
Collect patient information from significant others, physicians, teachers, therapists and other agencies serving children and families to assess patient needs.
Identify and prioritize problems, needs, and opportunities to determine level of assistance needed. Possesses knowledge of community resources and coordinates all needed referrals for residential, financial, vocational, psychiatric, educational, health and counseling needs. Consult with patient and families to develop and implement individual treatment plans, identify services, strategies and/or treatment options and assist with accessing natural support systems, linking to community services and monitoring symptoms. Revises service plan with the patient at annual reviews and as care needs change.
Charts his/her patient's progress in their clinical record and completes all paperwork necessary by MHS policy for professional, administrative, financial, and evaluative purposes of monitoring clinical care and of assuring efficient and effective agency operations. Ensures that needed release forms are signed and correct.
Maintains a professional relationship and provides face to face contacts with patients regardless of service environment (e.g. residential, inpatient). Engages in outreach, problem solving, advocating, crisis intervention, skills training.
Screens for pain and encourages patient to follow up with a health care provider of their choice.
Address urgent needs with supervisor, such as stabilization, medication, homicidal and suicidal behaviors by assisting the patient in accessing appropriate intervention.
Participate and present patient cases for review, participate in Quality/PI activities when appropriate,
Responsible to deliver the number of units of service budgeted by the Vice President in order to assure the fiscal viability of MHS.
Represents MHS to the community caregivers, citizens, and community leaders, explaining MHS policies. Consults with other providers and community caregivers to strengthen and coordinate social intervention programming while representing MHS.
Actively assesses patient for crisis stabilization and intervenes during signs of crisis, utilizing de-escalation techniques and crisis management skills.
Assist patients with entitlements.
Follows all universal precautions for safety, infection control and follows all state, federal and joint commission health and safety standards.
Treats patients and family with dignity and respect at all times, and holds all patient information in the strictest confidence.
Adhere to professional standards, policies and procedures, federal, state and local requirements and Joint Commission standards, including National Patient Safety Goals. Presents a positive image of MHSCC to other community agencies, caregivers and citizens
Completes all MHS required education and training, including initial agency orientation, mandatory trainings and educations, and up keep of all required certifications and licensure as required by state, federal and regulatory requirements.

* Maintain accurate and timely documentation within 24 hours of resident contact.
* Facilitate psychosocial, relapse prevention, and life skills group sessions weekly.
* Ensure minimum monthly contact with all assigned residents (caseload up to 40).
* Provide transportation or accompany residents to essential appointments, including medical and behavioral health visits.
* Participate in Blended Management Team meetings weekly.
* Submit complete service logs weekly (50% of hours must be direct service).

Minimum Qualifications
* Bachelor's degree in Social Work or a related human services field (required)

Job Summary:

The on-call nurse will provide direct hospice services to the patient and their families, within their homes and nursing communities.

Provide health care instructions to the patient as appropriate to the assessment and plan of care.
Prepare clinical notes and updates the primary physician when necessary.
Administer medications and treatments as prescribed by the physician.
Provide direct patient care as defined in the State Nurse Practice Act.
Counsel the patient and family in meeting nursing and related needs.
Communicate with the physician regarding the patient's needs and report any changes in the patient's condition; obtain/receive physician's orders as required.
Ensure that arrangements for equipment and other necessary items and services are available.
Complete the initial assessment of the patient and family to determine hospice needs.
Provide complete physical assessment focused on disease progression and decline including history of current and previous illness(es), pain assessment, safety assessment, wound assessment as applicable.
Perform the admission process which includes signed legal forms, verbal physician certification for hospice service, completed IPOC, completed narrative notes of admission process, medication entry to computer, completed appropriate clinical summary, copied patient records to support hospice appropriateness, and copied Advanced Directives.
Initiate the POC based on the patient and family/caregiver needs reflecting assessment data obtained to determine the nursing diagnosis and establish goals for care incorporating palliative nursing actions.
Include both the patient and family/caregiver in the care planning process.
Re-evaluates this POC regularly and revising as needed to reflect the patient and family/caregiver care needs.
Complete, maintain and submit accurate and relevant clinical notes regarding patient condition and care given at the time the assessment is done or care given.
Record pain/symptom assessments and management, changes/outcomes to pain/symptom management as appropriate.
Additional Duties:
Participate in on-call duties as assigned by the VP of Hospice and defined by the on-call policy.
Ensure that equipment, medication and other necessary items and services are available to the patient.
Supervise ancillary personnel and delegates responsibilities when required.
Assume responsibility for personal growth and development maintaining and improving professional knowledge and practice skills through attendance and participation in continuing education programs.
Fulfill the obligation of case assignments.
Actively participate in quality management program.
Attend the Interdisciplinary Group/Team meetings regularly.

Essential Functions
Ensure appropriate delivery of hospice services to eligible patients and families.

+ L ead or facilitate Interdisciplinary Group (IDG) meetings, including presenting patient updates, reviewing charts, coordinating care plans, and documenting minutes
+ S erve as a mentor, trainer, and clinical support to associates, supporting development, performance, and compliance
+ E nsure all care delivery meets federal, state, and local regulatory standards, as well as company policies and procedures
+ S upport performance improvement initiatives, quality assessments, and operational efficiencies within your location
About You
Qualifications - What You'll Bring:
+ Current RN license in the state of employment
+ Graduate of an accredited nursing program
+ Minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care setting
+ Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC)
+ Knowledge and compassion for terminally ill patients and their families
+ Ability to lead a diverse care team while adapting to the emotional and clinical needs of patients from various backgrounds
+ Strong interpersonal, organizational, and leadership skills
+ Competency in clinical documentation and technology systems
Preferred Qualifications (Not Required):
+ Bachelor of Science in Nursing (BSN)
+ Previous leadership, team management, or supervisory experience in hospice or home health
+ Familiarity with EMR systems and telehealth platforms
+ Certification in Hospice and Palliative Nursing (CHPN)

Responsibilities
Otterbein is seeking an enthusiastic and professional Hospice Nurse.
This role is responsible for primary caseload.
This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures.
Qualifications
Education: Bachelor's degree in nursing preferred. Graduate of approved nursing program
