Position Overview:
Provides carecoordination, case management, community support services (assistance with maintaining income, basic needs, independent living skills, crisis intervention, and carecoordination) for NARA NW clients. Works as a part of the Mental Health at the Totem Lodge location and in close collaboration with the Medical services and other NARA NW programs. Assists clients and providers in developing and implementing strengths-based service plans for case management and community-based services. Acts as liaison between Totem Lodge clients (clients experiencing Severe and Persistent Mental Illness) and medical providers (internal and external).
What you will do:
* Provide support, case management, and community-based services to clients living with severe and persistent mental health concerns that significantly affect their functioning.
* Assist with obtaining benefits such as insurance and Social Security benefits, SNAP, job training, housing, health care, socialization, and other community resources as identified in a strengths-based plan.
* Provide skills training, such as finance management, medication management, disease/illness management, self-advocacy, and other skills training as needed including applying for housing, applying for jobs, use of public transportation, nutrition, hygiene, etc.
* Targeted crisis intervention in collaboration with clients' primary behavioral health and medical providers.
* Assist clients with SUD in finding AA/NA meetings and other recovery supports, in close collaboration with Recovery Support Services and Outreach Team.
* Refer to, and work closely with, Housing and Supported Employment teams.
* Assist clients in learning about and accessing cultural resources and activities.
* Monitor well-being of clients in the community setting on a routine basis as determined by service plan (outreach).
* Send and track / follow-up on referrals to internal and external resources.
* Deliver food boxes to clients as-needed.
* Provide outreach calls and letters to clients not following-up with services. Assisting with closing of client charts.
* Assist Behavioral Health RN CareCoordinator and Psychiatric Providers with coordinatingcare as-needed.
* Assist Adult MH Intake Department and Operations Supervisor with tasks, as assigned.
* Work collaboratively with Elder Mental Health Case Managers, Elder Tribal Navigator, and support line specialists within NARA NW to assist clients with a variety of care management needs.
* Provide a warm handoff for clients who are being referred to internal and external resources, (e.g. attending first appointments with client at health clinic, addictions intake, housing, cultural activities, etc.).
* Coordination of care - actively initiate and maintain communication and collaboration with all members of client's support system and service team as permitted by the client.
* Provide hospital follow-up services as directed by NARA NW's Life is Sacred Team.
* Close collaboration with Life is Sacred Team for case management, coordination of care, and connection to resources for clients who are at elevated suicide risk and/or experiencing current or recent DV/IPV.
* Maintain thorough, accurate, and timely documentation as required by agency policy and OARS.
* Participate in multi-disciplinary team meetings to coordinatecare.
* Attend all mandatory meetings and clinical supervisions.

Assessment and Care Planning
* Assess for and identify carecoordination needs.
* Identify risk factors and service needs that may impact member outcomes and address appropriately.

* Ensure discharge/transition plans are evaluated holistically from physical and behavioral health perspectives.
* Ensure members on the state hospital Ready to Transition (RTT) list are prioritized for referral into appropriate transition setting through collaboration with and community partners including the CHOICE ENCC.
* May compile and distribute referral packets to residential and foster care facilities as needed.
* Coordinatecare for members residing outside of service area as required in contract.
Compliance
* Maintain unit compliance with CoordinatedCare Organization requirements.
* Maintain tracking data for program evaluation and reporting purposes.
* Maintain timely and accurate documentation about each member per program policies and procedures.
* Maintain working knowledge of COA and OHP benefits, including Addictions and Mental health benefits.

Minimum of an associate degree from a respiratory therapy education program supported or accredited by the Commission on Accreditation for Respiratory Care (CoARC), or a related field. Bachelor degree in Respiratory Care or Healthcare administration preferred.
Minimum of one specialty practice credential issued by the National Board for Respiratory Care (NBRC) preferred, including Adult Critical Care Specialist (ACCS), Neonatal Pediatric Specialist (NPS), Sleep Disorder Specialist (SDS), or Registered Pulmonary Function Technologist (RPFT), or the National Asthma Educator (AE-C) credential issued by the National Asthma Educator Certification Board (NAECB).

This position partners with the following including the undergraduate Medical Education programs; administers MMI's, OSCE's, CSAs, CPX, Physical Exam's, IPE training and all other standardized clinical teaching and simulation experiences within the four-year medical school curriculum. In our inter-professional facility this position is also instrumental in the integration of Advanced Practice Nursing and other programs that are integrating SPs, simulation and our AV “SimCapture” technologies. This position will work in collaboration with other simulation partners across the university and hospital departments including Medicine, Physician's Assistant, Nursing, Dentistry, and the College of Pharmacy.

This role will work under the clinical direction of the physicians and nurses and directly reports to the Clinical Support Supervisor.
Function/Duties of Position
* PatientCare Management - Responsible for coordinating and managing aspects of the patient's clinical care process. Works with the providers by providing support with medication refills and providers support to the patient and family. Places lab, diagnostic and scheduling orders as directed by the physicians and coordinates with facilities and patients. Monitors the Epic in Basket messages and ensures timely response is received by patient, providers and answers messages within their scope. Monitors refill request received in Epic and from Right fax and manages this efficiently to avoid disruption in patientcare. Manages Prior Authorization and processes appeals appropriately. Insures all needed testing is resulted and in the patients EMR prior to appointments/procedures.
* Carecoordination - Ensures patients follow through with outside referrals to other specialties, labs and diagnostic imaging. Works with provider and RN to insure patients are completing needed testing according to their surveillance schedule
* Documentation/Data Management - Documents legibly, concisely and completely to ensure that others can accurately assess the status of patients' progress by reviewing that documentation. Participates as assigned with database input. Initiate and implement processes to facilitate new protocol implementation and tracking tools as indicated.
* PatientCare/Rooming: Accompanies patient from waiting room and verifies patients identity with two identifiers, takes and records vital signs such as blood pressure, pulse rate, respiration rate, weight, height and temperature; enters patient medications and allergies into EHR; refers patient to medical staff for immediate examination or treatment; transports patient to examination or treatment room with use of wheelchair or gurney or assists patient to walk; observes and reports changes in patient's conditions or fall risk; works with front desk staff to communicate delays to patients; monitors in-room wait times and work to facilitate efficient clinic flow; assists physician in emergency procedures such as cardiopulmonary resuscitation. Chaperones for sensitive examinations regardless of patient's gender.
Required Qualifications

It creates the need to be able to deal with phone noise, many simultaneous conversations and frequent interruptions. There may be opportunities for remote work with this position. Work schedule is five days a week; 8 hours per day. 8:00-4:30pm
Physical demands include pushing, pulling, stooping and lifting up to 50 lbs. Must be able to support the full weight of a patient up to 200 lbs if necessary.
