Key responsibilities this position will perform include:
Facilitates the donation process through coordination and communication of referral information and logistics. Appropriate routes all donor referrals and request from externals.
Maintains accurate documentation of the medical screening process via data entry and follows established referral intake procedures.
Evaluates medical suitability of potential organ and tissue donors by utili
Requests and interprets laboratory and diagnostic tests needed for evaluation of suitability and clinical management of potential donors.
Collaborates with hospital personnel and clinical teams to develop an action plan that supports the option of donation is maintained and activation of the appropriate DCIDS team members.
Participates in training, process improvement, departmental QA/QC activities and special projects as directed.
Performs other related duties as assigned.
The ideal candidate will have:
2+ years emergency or critical care experience in a healthcare setting
Prior experience as a Paramedic or EMT preferred
Allied health experience, nursing students or respiratory therapists preferred
Demonstrated ability to understand medical terminology and read a medical chart.
Exceptional teamwork, communication, and conflict management skills.
Valid Driver's license with ability to pass MVR underwriting requirements

A Medical Front Office Coordinator is responsible for answering phones, interacting with patients in a healthcare setting, and managing administrative tasks for the medical team. Their duties include scheduling patient appointments according to doctor availability, explaining intake forms to new patients, and checking in and checking out of patients, including processing patient payments. ESSENTIAL FUNCTIONS:
Answer phones and greet incoming patients and visitors.
Perform patient registration and insurance verification, and collect co-pays.

Request and obtain medical records for patients, prior to appointment date.
Send medical records of existing patients, upon request and proper authorization.
Scheduling of patients for available times for both the clinic, doctors, and clinical staff.

Work fluidly in our EHR system (EPIC) to ensure info is accurate and complete
Guide patients through the process by kindly talking them through the steps and answering their questions and concerns.

Role Scope:
We are looking for a Home CareCoordinator to ensure that personal and clinical home care needs are delivered to help our participants thrive. The Home CareCoordinator participates in the interdisciplinary team's assessment of needs and approval of services for each participant and activates internal and external resources to address those needs in the home setting.

Attend and participate in interdisciplinary team meetings and contribute to participant care planning to ensure home care services align with participant needs and center level priorities.
Perform home safety evaluations and ensure proper expectations and safety for home care staff
Participate in end-of-life care, coordination, and support
Handle and coordinate incoming calls related to participants, physicians, and agency services regarding physician orders, participant questions, and referrals
Communicate with participants via telephone or in-person, and provide effective communication with Interdisciplinary Team, including nurses, physical therapists, occupational therapists, personal care aides, social services, physicians, and other care team members, regarding changes in participant/staff schedule, test results, etc.

Required Qualifications:

JOB BRIEF:
A Medical Front Office Coordinator is responsible for answering phones, interacting with patients in a healthcare setting, and managing administrative tasks for the medical team. Their duties include scheduling patient appointments according to doctor availability, explaining intake forms to new patients, and checking in and checking out of patients, including processing patient payments. A pleasant, calm, and professional demeanor is essential, as the front office staff are the first and last people that the patients interact with. As a member of our team, we all provide a high level of efficient patientcare, while always presenting a caring, ethical, and professional experience for the patients.
ESSENTIAL FUNCTIONS:
Answer phones and greet incoming patients and visitors.
Perform patient registration and insurance verification, and collect co-pays.
Prior to scheduling patients, check insurance and other sources ensure coverage.
Request and obtain medical records for patients, prior to appointment date.
Send medical records of existing patients, upon request and proper authorization.
Scheduling of patients for available times for both the clinic, doctors, and clinical staff.
Create and submit Requests for Prior Approval as needed
Work fluidly in our EHR system (EPIC) to ensure info is accurate and complete
Guide patients through the process by kindly talking them through the steps and answering their questions and concerns.
Coordinate with office staff for timely care of patients.
Occasional travel to our other nearby clinic locations to receive training and/or help with coverage.
Participate in various projects and/or meetings, and complete other tasks as assigned by management.
Cross-train and help coworkers as needed
Perform general administrative tasks and assist with special projects, as needed
Qualifications/REQUIREMENTS:
Knowledge and understanding of the workings of medical offices
Strong experience working with EHR systems - especially our system, EPIC.
Ability to work independently with minimal supervision
Must have strong organizational and time management skills
Ability to work on multiple tasks and meet deadlines
Excellent communication skills
Detail-oriented
Understanding of medical terminology
Be a team player
MUST be able to communicate effectively in Spanish and English in-person and over the phone
Ability to work on multiple projects and meet deadlines
Education and Experience:
High school diploma required; Associate college degree preferred
1+ years' experience with working in a medical office
Proficient in Microsoft Word, Excel, and other office software

Key responsibilities this position will perform include:
Determines family dynamics and assesses the family's understanding of the patient's prognosis when appropriate to initiate the donation discussion.
Initiates the donation discussion and authorization process for potential organ and tissue donor families prior to, during and after death declaration. Provides families with the detailed information required to give legal informed authorization for anatomical donation.
Responds on site independently and/or in conjunction with assigned staff to all appropriate hospital referrals within designated time outlined per policy and procedure.
Communicates with the attending physician and other members of the healthcare team to establish rapport and ensure a collaborative planned approach for the donation discussion and authorization process.
Obtains authorization for donation per UAGA and verifies appropriate medical and legal documentation necessary.
Visually assesses donors, interpret charts, document information and communicate findings.
Collaborates with hospital and medical staff to provide potential donor families with accurate and timely information regarding the patient's current clinical course. Maintains communication with hospital staff and attending physician regarding the potential donor family's understanding of the prognosis and acts as a family advocate to the health care team as necessary.
Provides education to hospital staff regarding authorization, family care process and donation process.
Responsibilities may be affected by increased donor activity. Performs other duties as assigned.
The Family CareCoordinator will work 15 days per month and 24 hour on-call shifts.
The ideal candidate will have:
A bachelor's degree
2 - 4 years of healthcare experience with families, counseling, bereavement, and/or crisis intervention
Knowledge of medical and legal principles of authorization, donor evaluation, and management.
Exceptional teamwork, communication, and conflict management skills.
Valid Driver's license with ability to pass MVR underwriting requirements
