Purpose:
Ennoble Care is hiring for a full-time Hospice RN Case Manager who resides within or in close proximity to Philadelphia, PA. Partner with clinical leadership to manage/address any issues, share professional support, and exchange feedback to enhance professional growth.
Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation
Supervise and document the NAC every two weeks or as assigned
Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during off hours time or as requested by the Patient Care Coordinator to fill patient/family needs during the absence of other primary nurses
Provide appropriate support at time of death and period of bereavement for family members and loved ones
Participate in the Hospice orientation, in-service training, and community programs as related to hospice care
Maintain familiarity with policies of the agency and rules and regulations of State and Federal bodies which govern hospice
Accept other assignments as appropriate
What You'll Need:
Valid State license to practice as a Registered Nurse in the State of Pennsylvania
1-2+ years of work experience as a professional nurse preferably at least one of the years in hospice care
Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
Skilled in nursing practice, able to cope with loved ones/caregivers' emotional stress and tolerant of individual lifestyles
Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports
Must be a team player who supports an interdisciplinary team concept of caring for patient and loved ones/caregivers
Must have a valid driver's license, a good driving record, the use of a reliable vehicle, and the ability to drive year-round to locations

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
Qualifications
Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
Work closely with patients on a 1-1 level but get strong support from a patient care team.
Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
Ability to learn an electronic medical records system.
