Conduct thorough assessments of clients' medical and psychosocial needs to develop individualized case management plans. Collaborate with healthcare providers to coordinate and facilitate the delivery of medical services, ensuring a holistic approach to patient care. Assist clients in accessing community resources, social services, and support programs to enhance their overall well-being. Act as an advocate for clients within the healthcare system, ensuring their voices are heard and their needs are met. Provide timely and effective crisis intervention services, offering support to clients facing medical, social, or emotional challenges. Maintain accurate and up-to-date documentation of case management activities, adhering to organizational and regulatory standards. Work closely with interdisciplinary healthcare teams, including physicians, nurses, and mental health professionals, to achieve optimal health outcomes for clients. Provide health education and support to clients, empowering them to actively participate in managing their healthcare.
