Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning

Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care

Intact Family Services
Intact Family Services maintain and strengthen family units, by providing in-home services designed to prevent children from entering the foster care system. This program, works with family strengths, monitors the family dynamics, provides linkages to community resources, assists with housing or school issues, and provides information regarding child development/age-appropriate behavior and discipline as well as general case management services. Emphasis is on child safety and keeping the family together. The program provides intensive in-home case management as well as 24-hour response to emergencies.

Linkage with substances abuse treatment programs- Referrals for both inpatient and outpatient drug treatment programs are available to all family members.

* Create monthly, quarterly, and annual reports as required by funder contracts.
* Record client data, outcome measurement and other significant information into ETO and other required database systems in a timely manner to show the effectiveness of the Chafee program.

Pay Rate: $36 per hour
Two counselors needed for each session. Applicants have the opportunity to support one or both sessions.

BJC Behavioral Health is a community health center that provides and coordinates behavioral health services for more than 8,000 seriously mentally ill adults and seriously emotionally disturbed children in St. Louis City, St. Louis County, St. Francois, Iron and Washington counties. As an Administrative Agent of the Missouri Department of Mental Health (DMH), BJC Behavioral Health serves as a major point of entry for people eligible for mental health services funded by DMH and is responsible for serving as gatekeeper to the public mental health system.

Role Purpose
Responsible for providing case management services to maximize opportunities available to children exhibiting emotional disturbances in the assigned embedded school district(s).
Responsibilities
Completes timely documentation of services that clearly describe the need for the service, the intervention provided, the relationship to the treatment plan, the provider of the service, the date, the actual time and setting of the service, and the individual's response to the service.
Identifies, screens and refers children exhibiting emotional disturbances. Plans and implements appropriate academic and /or educational supports. Coordinates referral of children and families to community service agencies as appropriate for the mental health needs of the child. Consults with school staff and/or parents regarding the social/emotional/behavioral needs of children.
Provides resources to school staff and/or parents regarding characteristics, intervention and treatment of disorders. Consults with school staff regarding classroom and/or school wide approaches to behavior to develop positive behavior supports and interventions. Provides school staff development on topics such as positive behavior supports and intervention, prevention of violence, crisis planning and intervention, etc.
Conducts functional behavior assessments and/or social skills instruction. Measures progress and improvement of assigned children. Implements interventions for students with chronic behavioral and emotional needs.
Provides small group and/or individual counseling for such issues as social skills, anger control, etc. to assigned children.

$18.54 per hour As a Street Outreach Case Manager, you will have the opportunity to provide case management services for Epworth's Street Outreach Program. This employee is responsible for supporting youth with basic needs, crisis intervention, goal planning and attainment, and developing linkages within the network of community service providers.
This position demonstrates commitment to the mission, vision, and values of the agency by engaging in respectful, cooperative relationships with customers (i.e., clients, employees, and external stakeholders); being accountable for providing services and ensuring safety to customers while empowering customers in achieving the agency's mission.
Primary Responsibilities:
* Complete enrollment process with new Street Outreach clients; conduct required individual assessments and complete goal plans as needed; provide follow-up support as needed to reach goals.
* Complete required assessments to place homeless youth on the housing priority list.
* Build rapport and establish trust with homeless, runaway, and at-risk youth. Ensure a Strength-Based Treatment approach is utilized with clients.
* Deliver tangible items to clients as needed, including but not limited to food, hygiene items, safe sex kits, and clothing.
* Distribute written information, participate in community events and facilitate community presentations about the Street Outreach Program in an effort to reach the target population.
* Attend meetings in the community with participants as needed.
* Assist with the development and implementation of community service learning and other events for clients as assigned.
* Responsible for making decisions in a timely manner; independently determining next steps related to youth services for housing, medical, education, social and emotional supports; monitor and ensure youth receive identified, necessary resources for quality services.
* Provide coverage as needed in the Drop-in Center and conducting street outreach.
* Keeps supervisor informed of events, concerns, needs as appropriate.
* Work as a partner in the community to build new relationships with community members such as mentors, potential employers, other community resources and funding sources on behalf of the youth the program serves.
* Develop and maintain knowledge of programs in the St. Louis area that could be used as resources for youth.
* Create monthly, quarterly, and annual reports as required by funder contracts.
* Record client data, outcome measurement and other significant information into ETO and other required database systems in a timely manner to show the effectiveness of the Street Outreach program.
* Ensure program complies with accreditation and funder standards.
* Adhere to agency rules, policies, and procedures, including attendance, program/department-specific procedures, workplace safety, code of conduct, social media, training requirements, and confidentiality.
Supervisory Responsibilities: May supervise student interns and volunteers.
Qualifications:
* Bachelor's degree or equivalent, preferably in social work, drug and alcohol education, counseling, psychology, sociology, education, or criminal justice.
* Experience with runaway, homeless, at-risk, and foster care youth is preferred.
* Valid driver's license, dependable personal transportation, and valid auto insurance
* Age 21 years or older
Job Knowledge, Skills, and Abilities:
* Strong oral and written communication skills, including speaking to diverse audiences
* High level of integrity and demonstrated ability to handle sensitive and confidential information in a professional manner
* Basic knowledge of dealing with the older youth population, dealing with crisis intervention, resource sharing, and client needs.
* Proficiency in MS Office, Email, Word, & Excel

The physical requirements of this job include sitting, standing, walking, and climbing stairs on a regular basis, lifting up to 25lbs, and pulling and/or pushing on occasion
Work Environment:
Work is community-based and performed on-site. Will need to transport clients to activities or appointments in personal and/or agency vehicles. Evenings and some weekends required.
