Key Responsibilities Client Support & Advocacy Conduct IM CANS assessments every 6 months. Develop individualized treatment plans addressing social determinants of health, financial, mental/medical health, and legal needs. Review and update treatment plans every 90 days to reflect client progress or changes. Advocate for clients to access healthcare, financial, housing, and social services. Care Coordination Assist clients with applications for IM-CANS, SSDI, LIHEAP, Childcare programs, and other assistance programs. Serve as a liaison with community agencies, healthcare providers both internal and external. Arrange transportation for clients to essential services and appointments as needed. Documentation & Compliance Maintain accurate, confidential case files, documenting all client interactions, assessments, and follow-ups. Ensure compliance with organizational policies, state regulations, and legal requirements. Update IM CANS assessments every 6 months to ensure awareness of current client needs. Update treatment plans every 90 days to reflect evolving client needs. Qualifications Education Minimum of High school diploma required. Experience 2 or more years of experience in case management, crisis intervention, healthcare, and/or social services. Experience with IM-CANS, SSDI applications, and acute mental illness preferred. Skills & Abilities Strong organizational skills and attention to detail. Excellent communication and interpersonal skills, with the ability to build rapport and advocate for clients effectively. Ability to handle sensitive and confidential information with professionalism and discretion. Strong electronic documentation skills required (EHRs, Microsoft Teams, Excel, and SharePoint). Ability to work independently and manage multiple tasks in a dynamic environment. Bilingual in Spanish preferred. POSITION OVERVIEW The Residential Programs Therapist is responsible for the provision of client services to Shelter Youth and Family Services clients in the Emergency Shelters, Transitional Living Program (TLP), and young adults in the Youth Experiencing Homelessness (YEH) Program. In general, these include the provision of clinical services to youth and their families and other duties in compliance with the Illinois Department of Children & Family Services' (DCFS) rules and regulations. This position operates under the direction and supervision of the Clinical Supervisor, who is supervised by the Clinical Director. KEY RESPONSIBILITIES PROGRAM DAILY OPERATIONS: Responsible for providing trauma-informed therapeutic services (i.e., TF-CBT, CBT, DBT) for clients within the Residential Programs. Responsible for delivering training to direct service staff as needed. Mentoring interns within the emergency shelters. Attending Multidisciplinary Team Meetings every Wednesday at 10AM for each of the programs. Responsible for working with the Clinical Supervisor to implement trauma-informed programming for the clients. Responsible for a caseload of 12-15 clients and providing therapy groups once a week. Other tasks as assigned by the Clinical Supervisor. CLIENT SERVICES AND SUPPORT: Provide individual therapy and group therapy to clients weekly. Provide psycho-education and trauma-informed therapeutic groups to clients and/or families based on interest and need. Facilitate the development, review and modification of clients' clinical treatment, discharge, and aftercare plans. Collaborate with case management teams to assess client needs, develop individualized treatment plans, and track progress. Collaborate with multidisciplinary teams and other systems (schools, DCFS, etc.) to support the client as clinically necessary. Promote a trauma-informed and culturally sensitive approach to care, prioritizing safety, dignity, and well-being of each client. COMPLIANCE AND QUALITY ASSURANCE: Maintain compliance in documentation with all relevant licensing, accreditation, and legal requirements, including DCFS, COA, and Medicaid standards. Maintain comprehensive client records, adhering to the requirements set forth by funding entities, licensing regulations, and accreditation standards. Ability to complete required documentation within 48 hours. CRISIS INTERVENTION AND SUPPORT: Respond to crises or emergencies involving clients, providing immediate support and stabilization. Provide post-crisis support to clients and staff as needed for debriefing and processing. Provide on-call availability on a rotation basis with other clinical staff. ACCOUNTABILITY The Residential Programs Therapist reports directly to the Clinical Supervisor and works collaboratively with other program leaders to ensure the highest quality of care and service delivery. HOURS OF EMPLOYMENT The work hours for this position will generally be 10 a.m.-6 p.m., during which time the therapist will also be on-call for all Shelter residential programs. QUALIFICATIONS Education: Master's degree in social work, counseling, or related field Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) preferred and willing to obtain professional license within first year of hire.
