Job Title & Role Description:
The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes.
Skills Required:
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.).
High school diploma or GED required.
A valid drivers license, auto liability insurance, and reliable transportation to travel within the assigned territory.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Experience with motivational interviewing, trauma-informed care, and care coordination.

Qualifications

Current BLS certification required.

AECOM is seeking experienced and professional Community Development Block Grant - Disaster Recovery (CDBG-DR) Case Managers to join our team in providing applicant intake, outreach, and case management services across 27 Texas counties: Anderson, Bell, Brazoria, Caldwell, Cooke, Dallas, Denton, Fort Bend, Galveston, Guadalupe, Hardin, Hays, Henderson, Hockley, Jasper, Kaufman, Liberty, Matagorda, Montgomery, Newton, Polk, San Jacinto, Smith, Tyler, Trinity, Walker, Wharton.
The CDBG-DR Case Manager will coordinate and oversee services and provide support for program applicants. This is a position where a skilled Case Manager partners with a disaster affected individual or family. The Case Manager serves as the primary point of contact, assisting the applicant in navigating through the program requirements to ultimately address housing recovery needs. Travel to regional and satellite locations will be required. Travel requirement of up to 100 miles roundtrip, 2-3 times per week ; must reside within commuting distance of the assigned office.
The CDBG-DR Case Manager will be responsible for the following:
* Perform outreach, intake and assessments of program applicants and gather required documentation to establish program eligibility.
* Once established, maintain a caseload of applicants and maintain regular and required contacts to adhere to program guidelines.
* Maintain a high standard to protect applicants' information and provide excellent customer service.
* Maintain client related data systems, including case notes and completion of record entries on a timely basis; upload and correctly categorize applicant documents.
* Provide timely updates to applicants regarding case status and next steps.
* Attend meetings and trainings provided by the Program, internal and external; attend pre-construction and key release events with applicants.
* Assist applicants with temporary relocation selections, when applicable.
* Attend program related community, coalition and committee meetings as assigned; network and advocate for applicants and their housing needs.
* Work collaboratively with Team, Team Lead and Program Management to meet program goals and performance targets.

* Maintain a complete understanding of all applicable Program policies, procedures and requirements.

Qualifications
Minimum Requirements
* High School + 4 Years of case management experience, preferably in a housing recovery program; or experience as a case manager or customer service representative with skills transferrable to a housing recovery program or relevant experience or demonstrated equivalency of experience and/or education.
* Must pass a state and federal criminal history/security background check.
* Must pass a pre-employment drug screen.
* Valid US Driver's License for travel to various regional and satellite locations to support project task(s) assignments and scope of work. Travel requirement of up to 100 miles roundtrip, 2-3 times per week ; must reside within commuting distance of the assigned office. For positions with driving responsibilities, Motor Vehicle Records are reviewed as part of AECOM's background check process to ensure compliance with the Company's driving standards.

They work closely with parents, extended family, and legal parties to help children find a permanent, safe place to live. Case Managers also provide leadership in partnering with families and appropriate others in the community to assure that the therapeutic needs of a designated caseload of children are adequately addressed. The Case Manager will assure the services delivered both to the child/adolescent and to the family are individualized, appropriate to need, child centered, family focused, culturally competent, strengths based, and goal driven.
Essential Roles and Responsibilities
Work with children, families, and communities to plan for a child's permanency. The Case Manager will ensure all work is geared toward positive permanency for the child/youth/family.
Supervises activities related to the treatment and needs of children/youth in care through regular home visits and phone calls.
Creates and monitors a plan of service for each family and involves the family in the creation of the plan. Modifies the plan of service as needed to address current needs.

Responsibilities
Summary
Under close supervision, provide support and structure to residents in developing program plans. Control and secure facility and residents, and operate within legal constraints and contract agency stipulations.
Primary Duties and Responsibilities
May perform all duties of Monitor I.
Prepare written personalized program for each resident to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals. Review plan with residents on a regular basis and document progress and/or revisions.
Assist residents to establish personal budget, locate suitable housing, enroll in educational and vocational programs and participate in family and community activities, as appropriate, to ensure that they are working towards financial independence.
Monitor facility sanitation and safety. Assign daily housekeeping chores to residents to comply with facility standards.
May orient residents upon arrival at facility to provide smooth assimilation into program (e.g., have resident complete paperwork, take on facility tour, introduce to other residents, etc.).
Make entries into confidential resident files to provide the structure for case analysis and program planning and to provide documentation of program participation. Ensure files are secure to protect resident confidentiality.
Verify residents' jobs to ensure residents' employment is suitable under established program plan. Counsel with unemployed residents by providing guidance in obtaining employment in cooperation with the Job Developer.
Verify residents' movement according to contract, so that the resident is accountable for his/her whereabouts in the community.
Prepare terminal/discharge report within established time parameters upon resident release to summarize program performance and to satisfy contract requirements.
Participate in staff meetings by sharing observations and recommendations regarding the facility, residents and staff relations to identify and solve problems and function as a unified team.
Maintain confidentiality obtained through job duties regarding employees, vendors, residents, outside agencies, etc., so that sensitive information is only given on a "need to know" basis.
Comply with policies and work rules as described in the company Employee Handbook and the Facility Policies and Procedures Manual, including directives of managers and supervisors in a cooperative spirit to ensure conformance to company standards.
Adhere to attendance policy; be available for all shifts; work overtime as required; to ensure adequate coverage of the program and to ensure the safety and security of the residents.
Comply with Facility safety rules; take appropriate corrective action to ensure work is performed in a safe manner and without injury to self or others.

Minimum Requirements

Job Summary
Odyssey House Louisiana, Inc. is seeking a full-time Licensed Counselor or Social Worker. The schedule is Monday-Friday 8 am-5 pm and weekends as needed.
