New Season
For over 30 years, New Season Treatment Centers have been a leading national health care service provider of outpatient treatment centers that specialize in providing safe, quality and best in class care for individuals living with Opioid Use Disorder ("OUD").
Operating in over 70 treatment centers in multiple states, our team members are engaged in medication-assisted treatment, counseling, support, and care management of individuals living with OUD. We treat the whole person and, in so doing, address the underlying causes of OUD in an effort to provide a continuum of care that not only addresses treatment needs but supports the patient on their journey to recovery.
Job Summary:
New Season operates a number of nationally recognized and accredited methadone treatment centers dedicated to helping individuals with prescription drug abuse and/or opiate addiction regain control of their lives.

Essential Functions:
Works with patients to complete all intakes, admissions, discharges, and transfer paperwork.
Documents patient progress through counseling and interaction through groups.
Completes patient psychosocial and an individualized treatment plan within the required time frame.
Identifies any clinical/casemanagement needs and works to address those needs.
Performs individual, group, and family counseling as required.
Performs at least 20 hours of direct one-on-one patient contact per week through individual or group counseling.
Reports patient abuse, neglect and exploitation as required.
Reports patient grievances as required.
Educates patient in all aspects of treatment, corresponding health issues and steps to recovery.
Obtains Urine Drug Screens and initial patient photo identification.
Assists in monitoring all patient activities on center premises.
Actively participates in community relations activities as directed and authorized.
Ensures the reading and understanding of the Policy and Procedures Manual.
Ensures compliance with 42 CFR Part 2 (Federal Confidentiality Regulations) and 45 CFR, Parts 160 & 164 (HIPAA)
Is responsible for obtaining or maintaining proper licensure and/or certification according to specific state requirements.
Actively participates in preparation for surveys and inspections conducted by CARF, the State, DEA, Board of Pharmacy and any other agency site visits as dictated by the state laws or regulations.
Participates in all staff meetings
Ensures compliance with local, State, Federal and Colonial Management Group, LP rules, regulations and policies.
Always acts in the best interests of the program and company; honors, supports, and protects the proprietary data and rights of the company.
Performs other tasks as assigned by clinic, region or corporate leadership.
Minimum Qualifications:
Education/Licensure/Certification:
Experience in substance abuse field is not required, but preferred
Qualified candidates will have a counseling Certification in a related field (State Specific)
Bachelor's degree is preferred but not required
Experience Required:
Minimum of 500 hours of experience in substance abuse is preferred

Job Title: Clinic Based CaseManager

The CaseManager plays a vital role in supporting clients by assessing needs, developing individualized service plans, and coordinating care across multiple systems. This position involves advocacy, assisting with aftercare services post inpatient hospitalization, and connecting clients to community-based resources to promote stability and well-being. The CaseManager provides comprehensive services for individuals navigating mental health challenges, substance use disorders, intellectual disabilities, and complex social service systems such as: SSDI, Housing, Medicaid, and Medicare.
Key Responsibilities
Client Support & Advocacy
Conduct IM CANS assessments every 6 months.
Develop individualized treatment plans addressing social determinants of health, financial, mental/medical health, and legal needs.
Review and update treatment plans every 90 days to reflect client progress or changes.
Advocate for clients to access healthcare, financial, housing, and social services.
Care Coordination
Assist clients with applications for IM-CANS, SSDI, LIHEAP, Childcare programs, and other assistance programs.
Serve as a liaison with community agencies, healthcare providers both internal and external.
Arrange transportation for clients to essential services and appointments as needed.
Documentation & Compliance
Maintain accurate, confidential case files, documenting all client interactions, assessments, and follow-ups.
Ensure compliance with organizational policies, state regulations, and legal requirements.
Update IM CANS assessments every 6 months to ensure awareness of current client needs.
Update treatment plans every 90 days to reflect evolving client needs.
Qualifications
Education
Minimum of High school diploma required.
Experience
2 or more years of experience in casemanagement, crisis intervention, healthcare, and/or social services.
Experience with IM-CANS, SSDI applications, and acute mental illness preferred.
Skills & Abilities
Strong organizational skills and attention to detail.
Excellent communication and interpersonal skills, with the ability to build rapport and advocate for clients effectively.
Ability to handle sensitive and confidential information with professionalism and discretion.
Strong electronic documentation skills required (EHRs, Microsoft Teams, Excel, and SharePoint).
Ability to work independently and manage multiple tasks in a dynamic environment.
Bilingual in Spanish preferred.

What You'll Do:

Plan the well intervention activities and review and validation when completed, and for providing solution for wells with integrity issues with support of well completion specialist.

You will have 10-15 years' experience in rigless well intervention operations that includes design, planning and execution of wireline, slickline and coiled tubing operation in critical sour gas and preferably unconventional operational environment.

Manage an assigned caseload of client with HIV/AIDS requiring comprehensive Ryan White Medical CaseManagement Services.
Conducts initial intake and ongoing assessments, medication and treatment adherence assessments, treatment/service plans and medical assessments
Monitors progress toward improving/stabilizing health status
Development of comprehensive care plans with clients that integrates medication and treatment adherence into services
Work in conjunction with clinical providers (e.g. case conferencing, huddles); ensuring all clients are case conference at least twice per year.
Provides and tracks referrals and linkages to services with special emphasis on core clinical services, access to medical, income benefit programs and access to emergency financial assistance.
Monitors treatment and medication adherence for all core clinical services
Maintains client file, records all activity, and maintains appropriate documentation of eligibility
Inputs all data into the client level database.
Completes the CaseManagement Competencies training with a passing grade as determined by the AIDS Foundation of Chicago training policy
Attends at least 12 trainings during the contract year to enhance skills and HIV knowledge; including at least four clinical training
Submits monthly/quarterly/annually reports on program/client activities in a timely fashion.
Attends monthly team meetings and all other relevant program meetings as scheduled.
Conducts presentations and/or educational workshops on behalf of CCHC.
Conducts peer chart reviews to ensure continued quality of services.
Completes at least one telephone contact with each client monthly.
Meets with clients at other agency clinic locations as necessary.
Follows standard operating procedures and AIDS Foundation of Chicago
Performs other duties as required or assigned

The goal is crisis stabilization, safety planning, diversion from juvenile justice involvement, and support in reunification or short-term placement when needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES
* Respond to referring sources (police, hospitals, schools, etc.) by phone within 15 minutes and arrive on-site within 60 minutes (urban) or 90 minutes (rural).
* Serve as the designated crisis responder for LAN regions including Kankakee, Vermillion, Will/Grundy, Kane/Kendall, and South & West Cook.
* Complete the Crisis Intervention Intake Packet during each crisis encounter.
* Document all crisis activity in CareLogic within two hours of response.
* Facilitate family reunification, family-generated placement, or short-term agency placement as appropriate.
* Obtain approval from the After-Hours Crisis Supervisor before any placement occurs.
* Provide linkages, referrals, and resource coordination based on youth and family needs.
* Remain accessible by cell phone throughout scheduled shifts.
* Submit mileage, time sheets, and required documentation per agency protocols.
* Participate in individual and group supervision, team meetings, and program meetings.
* Follow Illinois mandated reporting requirements.
* Complete all Federal, DHS, Agency Core Curriculum, and Mandated Reporter trainings.
* Provide services to program clients throughout the full on-call shift.
