What You'll Be Doing
Deliver in-person crisis intervention and evaluations across Skagit County

Provide brief stabilization support and coordinate with MCOT team members, hospitals, and emergency services
Offer follow-up care planning and resource navigation

MA/MS/MSW in a Behavioral Science-related field
Mental Health Professional (MHP) status per WAC
Familiarity with RCW 71.05 and 71.34 preferred

Ability to attend DCR Academy within first year
Valid Washington State Driver's License with insurance and reliable transportation

What You'll Do
As a Family Support Case Manager, you will support families through a combination of advocacy, coordination, and strengths-based case management.
Case Management & Family Support
Provide trauma-informed, ongoing case management including needs assessments, goal setting, referrals, and progress checks

The Social Service Specialist:

This assessment is shared with the client and team members to assure that all relevant psychosocial issues are addressed in the care planning.
Provide brief, culturally relevant, counseling to client/families which would include: crisis intervention; mental health concerns; pregnancy and birth options; grief; stress; parenting; coping skills; problem-solving; abuse/neglect; family violence. Educate the client to make informed decision regarding personal, child and family health/mental health care and treatment. Brief counseling is defined as counseling to assist the client in developing some skills to cope with issues presented and developing a plan to address ongoing longer term treatment with appropriate mental health providers.
Communicate clearly, oral and written, with clients, community professionals and interdisciplinary team members. This includes maintaining records that are clear and concise regarding psychosocial and/or needs assessment, service plans, treatment and follow-up care. The documentation must reflect legal mandates for reporting abuse/neglect.
Participate fully in all interdisciplinary team meetings to provide input on client's assessed psychosocial needs and on interdisciplinary team plans for client. The goal is to build family strengths and resiliency.
Develop working relationships within the service system network in particular services that have significant impact on pregnant and parenting clients such as: Mental Health and Chemical Dependency systems; network within service providing community serving this population to develop strategies to improve services delivery.

To be a successful Foster Home Case Manager - Licensor you must become able to do the following always: 1. Follow Agency Policy and Procedure (current J:drive) 2. Operate according to the Department Operations Manual (practice guide and notes within competencies)
3. Meet departmental goals and/or outcomes.
4. Conduct family interviews and complete necessary documentation.
5. Maintain phone calls and emails with collateral contacts.
6. Complete a comprehensive Home Study within 90 days of Assignment.
7. Effectively collaborate with the Division of Licensed Resources.
8. Process and submit general licensing documentation.
9. Manage youth's case plan in collaboration with the child and family team.
10. Provide necessary and appropriate support for birth parents, foster parents, and / or other care-givers consistent with the wraparound-service-model.
11. Coordinate care with the Family Support Specialists.
12. Collaborate with colleagues and respond to additional duties as assigned.
13. Maintain commitment to culturally competent, trauma-informed, family-focused, child-centered, evidence-based, data-driven services.
