Strong patient assessment, patient management, and documentation skills.

FIRST THINGS FIRST... WHAT IS A TRANSITIONAL LIVING HOME?
These group homes serve teens who are on probation but cannot return home safely from a youth development center or other residential placement. Referred by juvenile court counselors, they stay for six to 12 months, working to finish school, identify career goals, get a job, volunteer, earn a driver's license, and learn to budget, cook and manage bills.
WHAT DO YOUTH CARE WORKERS DO?
The primary responsibility of Youth Care Worker is to provide program services to intervene with adolescents being served in the residential group home. Key responsibilities include:
Implementing direct treatment services through use of the agency's model of care.
Providing residential care services.
Working with adolescents, families, volunteers, and community representatives in the youth home and community settings.
QUALIFICATIONS:
Must be at least twenty-one (21) years of age.
Minimum of a High School Diploma and experience working with youth.

You can work as much or as little as you like as an On-Demand Team Member (TM) and your schedule may vary depending on shift availability and store needs. This role is ideal for those that are looking for a personalized work schedule.

These positions are in support of Seymour Johnson Air Force Base either working on base with military members and their families or in local schools with high populations of military children attending. Provides the full breadth of MFLC consultation and counseling services to the command structure and military service members and their families at military installations. Responsible for providing MFLC services including non-medical, short-term, walk-around counseling, training/health and wellness presentations, and consultation to installation command regarding behavioral health issues with an understanding of the military unit, cultural and mission. Works closely with the Regional Supervisor/Regional Director, installation, and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation.
* Provides non-medical, short-term, solution focused, walk-around counseling directly to eligible service and family members at assigned primary and secondary locations. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
* Delivers training and health and wellness presentations, participates in health fairs and other base/installation activities as requested/directed.
* Enters counselor activity data daily through smart phone, or web application, to assure that reporting is accurate from assigned installation, while maintaining client/service member confidentiality.
* Consistently visible within the brigade/regiment which allows MFLC services to be accessible. Establishes and maintains working relationships with community resources and provides appropriate linkages for service members and their families. Partners with POC to provide MFLC services in a manner that addresses the needs of the installation. Develops an effective and professional working relationship with the installation POC.
* Provides MFLC response to critical incidents, surge and on demand events, and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. Counselor will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.
* Manages duty to warn and restricted reporting situations according to Department of Defense (DoD) protocol and staffs the cases with Regional Supervisor/Regional Director.
* Communicates with Regional Supervisors and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations to which the counselor is assigned.
* Participates in regular in-services/training, Quality Improvement committees or other contract activities as assigned/appropriate.
* Participates in initiatives, studies, and pilot programs as directed by the customer and/or Magellan. This includes participation in pilot program certification and training processes, completion of activity documentation, integration of pilot activities in non-medical counseling work, and the utilization of applicable technology to complete required activities.
* All other duties as assigned.

Responsibilities
Master's degree from an accredited graduate program in a relevant field of study to include, but not limited to, a mental health related field such as social work, psychology, marriage/family therapy, or counseling.
Valid unrestricted independent counseling license from a State, District of Columbia, US Territory or Commonwealth that grants authority to provide counseling services as an independent practitioner.
A minimum 2 years of post-licensure clinical experience.
Demonstrate intermediate-level competencies in technology and software (i.e., Microsoft Suite, Internet navigation, Smartphone/Tablet and application navigation, video-conferencing platform navigation).
Requires ability to engage and communicate with military members or children as assigned, in order to accomplish job functions, and to respond quickly to emergent situations in any physical location on a military installation and/or within a school setting, which includes the need to traverse short and/or long distances within the base to both indoor and outdoor locations, to maneuver through rugged, outdoor or uneven locations (e.g., steep inclines, stairs, grass), and work in outdoor weather and other military base conditions. May require travel to locations outside of a military base installation in a variety of physical environments. Due to the nature of working on military installations or related worksites, counselors may need to comply with various site-specific requirements to work at designated locations. For example, for some assignments, counselors will need to have certain immunizations or vaccinations and provide record of receipt.

The Foster Care Case Manager works under the supervision of the Program Manager. They are responsible for the delivery of case management services, individual and family counseling and training and supervision of foster families. Places children in state licensed or state certified foster homes.
Obtains all state and agency required documentation for child in care.
Provides weekly documented contact and at bi-weekly visits to child and foster home.

Prepares a treatment plan for the child within 21 days of placement and quarterly thereafter.
Prepares monthly progress reports for assigned children.

Position Overview
The Case Manager I - Housing Coordinator at Wilson Community Health Center provides case management to HIV clients utilizing The Housing Opportunities for Persons with AIDS (HOPWA) program funds, resource identification and assist clients in accessing supportive services. THIS POSITION OFFERS A 40-HOUR WORKWEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-WEEKLY PAY.
Essential Tasks
Provide outreach to Region 8, Part B counties Housing Authorities and private landlords in order to locate and identify housing locations for clients utilizing HOPWA funds. Develop and maintain a resource guide. Report on housing availability at monthly staff meeting. Act as liaison between housing authorities and Ryan White Program.
Accept referrals from HIV case managers and conducts initial assessment of need. Initiate quarterly contact with HIV Case Managers to case conference on shared clients.
Certify an applicant for participation in the R8NHP program. Certification includes making income verifications and eligibility determinations.
Work with clients and HIV Case Managers to collaboratively develop housing stability plans and recommend available R8NHP and community-based housing assistance. Housing Coordinator will contact the HIV Case Manager at least every six months to case conference regarding the client's housing stability.
Compute utility allowances, tenant rent and housing assistance payments for long-term rental assistance.
Explain program rules and expectations, and client's obligations and rights under program, including grievance process.
Experience and Education
Bachelor's degree in social work, sociology, health education, or equivalent; or Licensed Practical Nurse (LPN) with two years of experience working in human services.
Effective communication and interpersonal skills.
Must demonstrate the skills and judgment necessary to provide direct care to patients and nursing interventions under the direct supervision of providers.
Strong computer skills and working knowledge of the windows-based environment and Electronic Health Record (EHR) experience.
Active North Carolina Driver License.

Job DescriptionKind Behavioral Health (KBH) is looking for ROCKSTAR BCBAs to join our team in Greenville, North Carolina (Address: 2412 Emerald Place Greenville, NC). Our team provides life-changing ABA treatment to children and their families with Autism. Position SummaryThe Behavior Analyst designs and oversees ABA treatment programs. This includes continuously evaluating client needs, providing parent education and consultation, and problem-solving to help each client reach his or her potential. Additionally, this position is responsible for supervising behavior technicians, ensuring that they are professional, learning, and implementing treatment plans with a high degree of reliability and fidelity.

Job Duties/Responsibilities:
Conduct behavior assessments according to needs of the client
Design Treatment Plans of Care
Design measurement and data collection systems
Provide case oversight in compliance with the BACB, ~20% oversight (or more if requisite)
Motivate and provide mentorship to assigned Registered Behavioral Technicians (RBTs) to help them learn, grow professionally, and implement treatment plans effectively
Attend monthly off-site team RBT meetings to drive client outcomes and performance
Attend monthly off-site Client Outcomes review
Conduct Family training
Billing and documentation for all clinical activities, same day as activity
Responsible for attending regularly scheduled monthly meetings with BCBAs and other staff as needed
Necessary skills/attributes:

Requirements:
1 plus years experience in ABA preferred
Board Certified Behavior Analyst (BCBA) certification, in good standing
Experience with ASD, highly preferred
Valid driver's license
Must pass a background and reference check

Coastal Horizons Center, Inc. has an opportunity for a Family Centered Treatment Therapist to become part of our team in Lenoir County, NC. This position will provides a full range of mental health therapy services for clients with complicated mental health disorders using the Family Centered Treatment model. The FCT Therapist will also:
• Provide independent diagnostic and psycho-social evaluations and therapy services which focus on complicated mental health issues, family issues, parenting issues, adolescent issues and substance abuse and addiction issues and their impact on daily living and individual and family relationships.
• Evaluates the client and family situation and their ability to deal with their problems; conducts an in-depth comprehensive clinical interview to determine psycho-social, mental health, substance use and medical needs and documents diagnoses and recommendations; develops a person-centered plan based on this assessment; provides ongoing monitoring, assessment, and updates to the Person-Centered and Crisis Plans
• Provides in-depth therapy which can range from crisis intervention to treatment of complex co-occurring mental health and personality disorders.
• Provides individual, group and family counseling and activities adapted or specifically designed for persons with physical disabilities or persons with co-occurring disorders or mental illness and substance abuse/dependence or intellectual disability/developmental disability and substance abuse/dependence.
Qualification/ Skills:
Education:
Completion of Bachelor's Degree, within Human Service field preferred.
Meets requirements under 10A NCAC 27G.0104 as a Qualified Professional for Mental Health.
Preferred graduation from an accredited program with a Master's degree in a human services or related field and two years of progressive child and family mental health/substance use counseling experience, or an equivalent combination of education and experience.
A valid driver's license and/or government-issued ID is required. Must have reliable transportation.

The Community Support Team Licensed Substance Abuse Professional works as part of a team for adults to plan, coordinate, and monitor services through the provision of direct and indirect services to eligible persons served. Persons in this position are required,
at a minimum
, to have a four-year degree in a human service field and two years of experience with the mental health population and be a licensed Substance Abuse Professional. Licensure qualifications may include CCS, LCAS, LCAS-A, or Certified Alcohol and Drug Counselor (CADC). Persons with a non-human service degree may be considered if they have 4 years of mental health experience. At least one year of the experience must be with adults. Staff with a higher level of credentials may fulfill this job position.
Community Support Team service recipients must meet medical necessity criteria, per the CST service definition. Persons employed in these positions work together with the team lead to develop the person-centered plan. As such, they must be knowledgeable of the adult mental health population, able to plan and engage in activities, provide supportive counseling, and crisis management. This position requires the ability to work in a mutually respectful partnership with the persons served and their support to gain the skills and confidence to address any issues and problems they face. A working knowledge of practical application of the agency Evidenced Based Model as well as Motivational Interviewing is required. They are required to attend and participate in meetings on behalf of the person-served, participate in weekly case consultation meetings with the team, and participate in clinical training events and meetings as required by the Clinical Director and/or Medical Director.
