Essential Functions:
Establishes and maintains a regional support and information network for parents and families of children with emotional disabilities.
Teaches, coaches, mentors, and models for parents and caregivers regarding basic communication skills and advocacy training, resource identification and management, coping skill development, etc.
Assists parents in locating available resources for their child and in understanding the rights of the child regarding these agencies and advocating for clients and families in relevant situations.
Maintains timely documentation of services provided including billing documentation required in the Avatar system, requiring entry within 48 hours of service.
Serves as an active member or consultant for children's treatment teams by assisting in treatment plan development.
Establishes and maintains parent support groups throughout the community, disseminating relevant information to families, empowering group members to assume leadership roles within the group, etc.
Responds by phone or letter in a timely manner to parent/caregiver requests for assistance.
Attends mandatory in-services, team meeting, and other training/educational programs as required, both internally and with external local, regional, and state organizations.
Fosters collaborative working relationships among members of the leadership and management teams and programs.
Other duties as assigned.
Qualifications
Education and Experience:
High School diploma or GED
Must be a parent or relative caregiver with permanent legal custody who is raising or has raised a child with an emotional, social, behavioral, and/or substance use disability, and whose child had been a client of at least one state-funded service for children with SED.
LICENSURE/ CERTIFICATION:
Must successfully complete core competency training administered by the statewide family network and demonstrate an understanding of the System of Care philosophy, the Wraparound Process, etc.
Required to seek continuous professional development training in order to maintain professional status and meet Medicaid and/or SIAC requirements.
Current Kentucky Certification in Family Peer Support is preferred but not required
Valid Driver's License

Must be willing to work flexible hours and be able to travel within the region served.
Must maintain a non-judgmental attitude toward children with SED and their families.

The Family Case Manager is a caseworker specialist in social services within a Local County Office of the Department of Child Services. A Day in the Life:
Responsibilities include:
* Investigate reported incidents of child abuse, neglect, or dependency, determine whether the incident is substantiated, and develop recommendations to a Juvenile Court or County Director for disposition.
* Assess the level of risk to the child for additional injury or harm, including whether the child is in imminent danger, and may remove the child from the family as the situation warrants, placing a child in a protected environment.
* Perform needs assessments to determine treatment options for families and children reported to be abused/neglected.
* Testify and prepare pre-dispositional and progress reports, maintain case files, and develop briefing reports for the community child protection team. Develops "informal adjustment" plans with families and children to divert children from the juvenile justice system.
* Develop case plans to assist families and children to become more self-sufficient in a safe and nurturing environment and to ensure a permanent home for the child.
* Develop discharge/transition programming to assist families and children to attain and maintain self-sufficiency in a safe environment.
* Monitor and assess the service capability of treatment programs and providers to ensure consistency with the case plan.
* Accept on-call responsibility as needed to receive and investigate allegations of child abuse/neglect.
* Conduct home studies and family histories and recommend placement for adoption to an adoption team.
* Visit children in out-of-home placements to coordinate family reunification, adoption, or independent living efforts.
* Maintain data to help management evaluate trends in family and child needs.

Overview:
Provides non-medical case management services and medical case management services under the Indiana State Department of Health, including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs. Performance Responsibilities
¨ Assists students in evaluating students' aptitudes and abilities through the interpretation of individual standardized test scores and other pertinent data, and works with students in developing education and occupation plans consistent with such evaluation
¨ Assists students in making course and subject selections, as well as with evaluating career interests and choices
¨ Compiles course request data from all students in the high school, creates master schedule, runs master schedule, balances course section numbers, and runs student schedules
¨ Responsible for tracking and ensuring student completion of Indiana Graduation Pathways requirements
¨ Collaborates with district personnel (building administration, central office administration, district CTE coordinator) on state reporting duties
¨ Assists in the scheduling of classes
¨ Obtains and disseminates information regarding occupational opportunities to students and to classes studying occupations
¨ Coordinates and administers APEX/Edmentum online learning platform
¨ Coordinates student enrollment and grade reporting with the regional career and technical center
¨ Assists students with admissions, scholarship and identifying employment opportunities
¨ Coordinates with administrators and other teaching staff members to ascertain individual student's abilities and needs, including students with special needs, and to familiarize stakeholders with guidance services
¨ Participates in annual case review, placement change, and manifestation determination review meetings involving students with IEPs
¨ Researches educational and career opportunities, and coordinates with teachers, college and university personnel, resource specialists, and business and community organizations for the purpose of providing information, and making recommendations
¨ Assists with the registration and orientation students who are new to the school regarding procedures and educational opportunities
¨ Assists with the school district's dropout prevention efforts
¨ Serves as ready resource to students to provide counseling that will lead each student to increased personal growth, self-understanding, and maturity
¨ Continues to acquire professional knowledge and learn of current developments in the educational field by attending seminars, workshops or professional meetings, or by conducting research, and by maintaining professional relationships with members of institutions of higher learning and the business community
¨ Organizes and maintains a system for accurate and complete record-keeping and providing student information to prospective colleges and employers, as required by district procedures and applicable laws
¨ Encourages parental involvement in students' education and ensures effective communication with students and parents
¨ Assists in the orientation of new teachers, and provides in-service training in guidance
¨ Performs other related tasks as assigned by the Principal and other central office administrators as designated by the Superintendent

Job responsibilities: Responsible for directing the mental health program in compliance with government and management directives.
Essential functions:
* Evaluate students referred by staff and document legible assessment notes to include at a minimum reason for referral, presenting problem, history of present problem, mental status exam, diagnostic impression, and a clear treatment plan. Independently diagnose and treat mental health conditions.
* Develop and implement a mental health program with emphasis on prevention, early detection, identification of mental health problems, short term counseling of manageable conditions and crisis intervention using employee assistance model.
* Participate in HIV case management; notify students of the results, conduct mental health evaluation, and provide short term intervention.
* Provide integrated case management by interacting with employees regarding student behavior, compliance, medication, side effects, and treatment progress.
* Draft procedures for emergency psychiatric conditions (suicide attempt, psychotic episodes, etc.) to include 24-hour on-call coverage.
* Review pre-admissions folders, evaluate requests for reasonable accommodations and help form an accommodation plan.
* Recommend medical separation to the center director when a student develops a psychiatric disorder that clearly impairs their ability to complete or take part in the training program.
Education and Experience Requirements:
* Clinical psychologist, LCSW, LPCC, LMFT or equivalent licensed, autonomous practicing, master's level mental health therapist in the state of Kentucky considered.
* At least 1 year post independent licensure experience with adolescents/young adults, excellent written and verbal communication skills, and computer proficiency required.
* Valid driver's license with an acceptable driving record, unless waived by management.

Key Responsibilities:
Ensure the completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation in accordance with applicable federal and state standards and agency policies and procedures.
Provide patients with orientation regarding objectives of the program, phase system requirements, rules and regulations, sanctions, hours of services, patient rights, and additional information to help patients adjust to the program.

Perform treatment plan reviews
Participate in case conferences and case reviews in accordance with established policies and procedures.
Develop and increase knowledge of federal, state, and agency regulations regarding confidentiality, methadone treatment, and substance abuse counseling certification.
Submit all internal and external correspondence to supervisor for review and approval.
Complete intakes, facilitate group counseling sessions.
Collection and testing of urine/swab drug screening following proper chain of custody.
Secure necessary documentation from patients, when appropriate, to ensure compliance with established policies and procedures.
Submit monthly and quarterly statistical reports to supervisor.
Attend supervisions as scheduled
Attend team meetings and complete all training courses timely as required.
