Requirements:

Job Responsibilities:
Provide in-home intervention to children and their families utilizing skill-based, evidence-based practices to help families implement solutions to problems and strengthen families.
Act as a resource, provider & advocate families to promote child welfare.
Provide evidence-based interventions and case management support under contractual responsibilities.
Utilize collaborative documentation.
Develop and maintain effective working relationships with community partners including DHHS, medical providers, “parent partners,” lawyers, therapists, law enforcement staff, school officials, neighbors, fictive kin, emotional supports, and referral sources.
Provides evidence-based practices to families and individuals associated with the family, utilizing Solution-Based Casework, SafeCare, Child Safety Conferences, Motivational Interviewing, SFM & YTDM meetings.
Maintain an appropriate level of direct contact with families per program requirements and remain available to families 24/7 for crisis intervention as needed.
Ensure professionalism through role modeling, and respect.
Utilize own vehicle to travel to work destinations and transport youth as authorized, provide services in the community, or in the primary home of the child and family.
Report and document emergencies and critical incidents involving family members or staff.
Participate actively in individual, group, and organizational supervisions/trainings.

The Case Manager, Enterprise Absence Management will be responsible for using the CVS Health my Leave absence platform as well as other HR systems to administer FMLA, State regulations and Company leave policies from the start of a leave request through the return to work.
Job Description:Support a service delivery model that reflects empathy and care for all colleagues and fosters an environment focused on the colleague experience Administer leave of absence requests under FMLA, State regulations and Company leave policies in accordance with federal and state regulations Utilize the my Leave system for leave of absence to ensure tasks and cases are completed according to federal and state regulations Review and analyze leave of absence requests that are not eligible for FMLA, State and / or Company leave policies to determine if the ADA is applicable.
