A Brief Overview Responsible to plan, organize and implement social work services for patients and families that are medically and socially complex and greater psychosocial acuity; assuring that the medically related emotional and social needs are addressed on an individual basis. The Social Worker will facilitate quality care/education for patients and their families. Is skilled in counseling individuals and groups. Responsible to assess needs and facilitate plans with patients/families requiring intensive discharge planning. Maintains a current knowledge base of community agencies and resources to facilitate appropriate referrals. The Social Worker provides services in a compassionate, culturally competent and tactful manner in order to help facilitate access to and provide continuity of care and patient advocacy. Works collaboratively with other members of the multidisciplinary team. What you will do Interviews patient and family members to obtain information about home environment, family systems, health history, and patient's personality traits. Evaluates data gathered in terms of the medical plan of treatment, available social service programs, financial counseling and plan a pertinent therapy program that will provide maximum benefits and safety for each patient. Assists patients and families to understand, accept, and follow medical recommendations with emphasis on assisting patient to be medically compliant with care plan. Performs a variety of services such as advising on social problems, arranging for discharge or post-operative care at home, assisting patients/families needing placement into assisted living or nursing homes, arranges for transportation to appointments and treatments, and works to alleviate anxiety and fears of patients and family members. Conducts supportive visits by interviewing patients and their family members to obtain information regarding their response to their diagnosis including evaluation of strengths, coping skills, supports, characteristics of family, health history and personality traits. Prepares and maintains patient documentation in EMR. Maintains department policies, procedures, objectives, and quality assurance program, safety, infection and environmental control. Assists with data collection, analysis and reporting as required by the hospital, administrative council, care teams, JCAHO, and other agencies. Conducts individual, marital, family and group therapies as necessary. Coordinate care conferences as needed. Serves as the social work representative in multi-disciplinary team approach to patient care. Utilizes research findings to improve outcomes and support, and provide quality of life for patients and their families. Works collaboratively with Case Management in providing coordinated services, which facilitates continuity of care for our patients. Works with emotional problems including but not limited to assisting patients, residents and families with anxieties and stress caused by illness, admission to hospital/nursing home, crisis situations, difficulties in coping with physical disabilities, fears related to helplessness and death, the need for nursing home/assisted living/specialized care, and providing educational materials. JOB SUMMARY: The Behavior Support Specialist helps ensure Boys & Girls Club members thrive by supporting their behavioral, social, and emotional growth. Serving youth ages 5-18, this position combines direct member support, family engagement, and staff coaching to create a safe, inclusive, and fun Club environment. Using trauma-informed practices and individualized Success Plans, the Specialist equips youth with coping and self-regulation skills, strengthens partnerships with families and schools, and provides guidance for staff in managing challenging situations. JOB DUTIES: Administrative/Management: Provide immediate redirection and de-escalation during behavioral incidents to ensure a safe environment for all members. Build supportive, consistent relationships with youth and families that encourage resilience, growth, and positive decision-making. Partner with staff, families, and schools to create, implement, and monitor individualized Success and Medical Plans, ensuring clear communication and documentation. Track progress, update success plans, and communicate regularly with families and staff on outcomes and adjustments. Ensure staff are aware of and consistently implementing strategies outlined in Success Plans. Facilitate mentoring, small groups, and family meetings that strengthen coping skills, conflict resolution, and self-regulation. Connect families to additional community resources as needed to address barriers to success. Recognize and celebrate youth accomplishments to reinforce growth and confidence. Model evidence-based, trauma-informed practices that foster safe and inclusive program environments. Provide “on-the-floor” behavioral support in program areas, assisting staff with high-need members. Deliver staff training on proactive behavior management, trauma-informed approaches, and de-escalation techniques. Maintain accurate documentation of all behavioral incidents, interventions, and follow-up actions. Contribute to the design and delivery of programs that strengthen members' social-emotional learning. Monitor trends in member behavior and provide feedback and recommendations to leadership. Collaborate with schools, social service providers, and mental health agencies to expand resources for members and families. Represent the Club in community coalitions, committees, and initiatives related to youth wellness and behavioral health. Pursue training, certifications, and professional development to remain current on best practices in youth development, behavioral health, and trauma-informed care. Share knowledge and resources with staff to strengthen the Club's culture of support and inclusion. Employment Qualifications Bachelor's degree in social work, psychology, education, or related field. Minimum of 2 years of experience in behavioral health and/or working with youth in group settings. Strong knowledge of child and adolescent development, trauma-informed practices, and behavior management. Skilled in family engagement, mentoring, and group facilitation. Excellent verbal and written communication, documentation, and organizational skills. Ability to remain calm, flexible, and professional in high-energy and stressful situations. Familiarity with community resources available to youth and families. Commitment to diversity, equity, inclusion, and belonging. Completes annual review of insurance pamphlet and coverage options as mandated by Conditions for Coverage. Communicates with supervisor and various departments such as Nursing, Physicians, Case Management, Social Work, Patient Access, Patient Relations, Quality Resource Management and Social Services to address patient concerns. Complies with other job-related responsibilities as directed by Leadership. Demonstrates a caring and respectful attitude to all patients, family members, visitors and fellow employees as defined in the mission and values of the organization. Qualifications Bachelor's degree in social work, case management, public relations or related field required. In lieu of degree, consideration may be given to six years equivalent work experience.
