Position Information:
Boone County Family Resources is seeking a qualified candidate for the position of Support Coordinator. Support Coordinators work collaboratively with individuals, families, and other professional team members to develop and implement individual support plans. Support Coordinators connect individuals and families with resources and services in the community and then monitor the effectiveness of those supports in helping individuals meet their needs and achieve personal goals.
Minimum Qualifications:
Applicants must be at least 21 years old, have a Bachelor's degree, and have one year of related experience working with individuals with developmental disabilities.
Additionally, applicants must have a valid driver's license, access to an insured and properly maintained vehicle for transportation, and operate that vehicle in a safe and prudent manner to comply with all applicable laws.

Schedule:
Support Coordinators work a 40-hour work week Monday through Friday. Occasional evening or weekend hours may be needed based on caseload needs. Once the introductory period is successfully completed, Support Coordinators have the opportunity to work a base schedule with some remote hours.

Job Description
· Performs behavioral health triage and crisis management services. Links and coordinates services with medical/surgical departments and other outside agencies, such as transportation and home health care. Coordinates psychiatric diagnoses, behavioral health treatment services, and psychotropic medications with behavioral health providers and physician advisors. Refers members to the appropriate level care and reviews and authorizes care services using InterQual criteria and medical necessity guidelines. Documents treatment authorizations and monitors, tracks and reports behavioral health care utilization.
· Conducts all necessary direct utilization management activities for behavioral health plan members including service coordination for members in crisis, crisis intervention, behavioral health triage, crisis assessment services, pre-certification and authorization of care for both psychiatric and chemical dependency services, all necessary concurrent review with hospitals and mental health centers, discharge and aftercare planning and referrals for continued outpatient care.
· Interacts on a daily basis with hospital representatives and attending psychiatrists and coordinates physician-to-physician reviews with the medical director. Reviews treatment plans and consults with medical director regarding medical necessity criteria for continued inpatient stay. Facilitates community placements, agency linkages and coordinates discharge planning, aftercare, referral and follow-up of inpatients.
· Documents, monitors, tracks, and reports authorizations, denials, treatment plans, clinical findings, progress notes, and case dispositions related to behavioral health services using automated and manual systems and formats.
· Monitors, tracks and reports cases. Monitors utilization trends by hospital and attending psychiatrist with emphasis on managing high volume providers.
· Identifies high risk/high volume members for more intensive follow-up care and conducts hand-off coordination between Clinical Coordinators and providers.
· Coordinates community resources with emphasis on the development of natural support system. Coordinates benefits, regulations, laws and public entitlement programs. Applies psychological and counseling theory and industry standard clinical guidelines with the appropriate ICD-9/DSM IV diagnosis. Maintains HIPAA standards and confidentiality of protected health information. Reports critical incidents and information regarding abuse and neglect.
· Ensures members have access, availability and quality care according to standards.
· Participates in case conferences regarding high risk/high volume regarding provider and hospital issues. Attends and participates in other HBH work groups and meetings as required.
· Responds to incoming telephone calls as assigned, meeting or exceeding the standards for phone access and availability in order to meet the needs of members served by the department.
· Performs all other clinical and administrative duties as assigned.
Qualifications

· Performs behavioral health triage and crisis management services. Links and coordinates services with medical/surgical departments and other outside agencies, such as transportation and home health care. Coordinates psychiatric diagnoses, behavioral health treatment services, and psychotropic medications with behavioral health providers and physician advisors. Assist clients with reporting new claims, gathering required documentation, and understanding coverage, next steps, and timelines.
Review claim status regularly and provide proactive updates to clients and the claims team.
Advocate on behalf of clients in all claim disputes, cases, and challenges.
Act as a Liase between clients and carriers when disagreements arise.
Ensure claims are handled in compliance with carrier guidelines, regulatory requirements, and agency standards.
Coordinate between clients, carriers, adjusters, and claims team to facilitate smooth claims handling and reduce delays.
All other duties as assigned.

YOUR QUALIFICATIONS & EXPERIENCE
Insurance and/or claims experience required
P&C license to be obtained within 90 days of hire (funded by Winter-Dent)
Experience with Microsoft 365 products preferred (Word, Excel, PowerPoint)

Responsibilities
What you can expect to do.
Champion possibility and fuel student success by providing social-emotional services for students who need additional support to shine in school. As a vital, integrated member of a multidisciplinary team, you'll collaborate with school teams and families in delivering SESI's proven, high-quality services-including individual or group counseling services.
* Work with a diverse group of students from varying developmental, cultural, ethnic, racial, and socioeconomic backgrounds.
* Provide individual, group, or push-in counseling services to students in accordance with their individualized education plan (IEP) or behavior intervention plan (BIP).
* Collaborate on creating IEPs, including social-emotional IEP goal setting and IEP meeting participation.
* Complete social-emotional and behavior assessments, analyze data to identify student strengths and weaknesses, and develop functional behavior assessments, BIPs, and other transition plans.
* Implement behavior management and intervention strategies, including de-escalation techniques, crisis management, and physical restraint when necessary to maintain the safety of students and staff.
* Support social-emotional learning skill development through targeted curricula, assessments, and more intensive behavior support as needed.
* Provide in-service training with other teams, including expressive therapy and clinical teams.
* Collaborate with external partners and providers to support students, families, and staff.
* Maintain documents in student files according to regulatory agency requirements.

Qualifications
What we expect of you.
* Professional licensure, including LPC or LCPC, LCSW or LMSW, and LMFT or LMHC, LCMHC.
* Master's degree in social work, human services, or related field (mental health, psychology, counseling).
* 2 years of experience working with children and adolescents in a school, special education, or behavior health setting preferred.
