Maintain accurate files and record keeping, which includes assessments, individual treatment plans, admissions, progress notes, discharge summaries, and discharge recommendations, to meet regulation requirements. Conduct assessments to determine clients' needs, strengths, and goals. Develop and implement individualized service plans in collaboration with clients and clinical staff. Provide ongoing case management, monitoring clients' progress, and adjusting plans as needed. Advocate for clients and link them to appropriate resources, services, and support systems. Work closely with clinical staff to ensure coordinated care and comprehensive support for clients, to include counseling, education programming, family therapy, medical services, and discharge planning. Communicate regularly with external agencies, service providers, and community resources. Facilitate and participate in case conferences, team meetings, and educational presentations. Maintain accurate and up-to-date client records, including assessments, service plans, progress notes, and reports. Ensure compliance with all regulatory requirements and organizational policies. Complete all required documentation and reports in a timely manner. Provide crisis intervention support as needed, responding promptly to emergencies. Collaborate with clinical staff to develop and implement crisis management plans. Complete all required reports and paperwork in a timely manner. Attend and participate in staff training, work teams, and meetings as directed. Ensure client needs are met to the extent possible through effective case management. Serves as a resource throughout the facility at all levels of care. Follows necessary requirements as determined by the state in relations to licensure and/or certification. Provide a variety of support services, assist in coordinating services and communication among the Social Security Administration, VA, and DHHS, identify and access client resources for persons; seek to expand resources independently and confidently; promote and maintain public relations; and complete forms accurately and timely.
