Coordinate and ensure that staff receives all training needed for implementation of the crisis response system. Provide technical assistance, which may include training, to private SCL providers or community caregivers in the areas of crisis prevention and management. Coordinate D/ID crisis line program details, such as scheduling and assistance that includes on-call/mobile crisis responders. Data collection and analysis of crisis stabilization supports. Complete required Division of Developmental and Intellectual Disabilities (DD/ID) monthly documents. Follow-up on D/ID crisis calls and make appropriate referrals for intensive case management. Collaborate with intensive case manager to provide comprehensive crisis management support for D/ID individuals. Be familiar with 202B protocols and complete when required. Complete assessments from new allocations. Conduct face-to-face and phone interviews when needed. To assist the D/ID Director in any other endeavor to ensure a quality program. Working with an individual and others identified by the individual as their team to develop an Individual Plan of Care (POC) budget and spending plan, and emergency backup plan. Utilize a person-centered planning approach to identify and implement all supports needed/wanted by the individual. Maximum caseload of 40 PDS clients Provide any needed assistance to a PDS participant with any aspect of PDS or blended services (i.e., recruiting/hiring/managing PDS providers, completing and submitting required forms.) Coordinate all services. Monitor a participant's health, safety and welfare. Provide a monthly face-to face home visit. Be available 24 hours per day, 7 days per week, including holidays.
