    How you will change lives As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies. Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community. Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations. What we're looking for Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE). Current licensure (in good standing) in applicable state is required unless employed in the state of AZ, PA or Guam. Must meet any practice requirement(s) for the applicable state. Demonstrated working knowledge of the English language and ability to communicate verbally and in writing. Basic computer skills, including Microsoft Office (Word, Excel, Outlook). Proficiency in all USRC clinical applications required within 90 days of hire. Preferred Previous experience in providing social services to dialysis patients preferred. Intensive Community Manager will serve as a clinical lead for a Community Care team. They will coordinate the team's efforts to stabilize our highest risk patients, with special areas of focus including safe transitions of care from facilities back to our primary care teams, stabilization of our highest risk ambulatory patients and outreach to patients who are assigned to us but are not engaged in care. This person will perform initial assessments and design comprehensive plans of care for many of these patients. This professional will also provide clinical supervision to other team members in delivering the plan of care and in other tasks necessary to meet their needs and engage them in care. Master's Social Work practice may include applying specialized knowledge and advanced practice skills in assessment, treatment, planning, implementation and evaluation, case management, supportive counseling, direct practice, information and referral, education, advocacy, programs and activities. •Work a flexible schedule which includes unscheduled visits with walk-in patients, same day patient visits, and scheduled patient visits as referred by Providers, members of the interdisciplinary team and self-referrals in isolated, church/community-based settings without direct supervision and often with no other staff on site. •Develops an action plan with goals, based on functioning level, physical/medical condition, support network, psychosocial and financial status based on long and short-term needs. •Develops and maintains effective working relationships with individuals and/or families referred for case management services. •Provide social work intervention only in the context of a professional relationship. •A social worker shall be responsible for setting and maintaining professional boundaries. •Facilitates support groups or psychoeducation groups as determined by the Behavioral Health Department for clients/patients. •Communicates effectively with clients, MHM staff, and participants in interdisciplinary team. •Works as part of an interdisciplinary team to provide integrated behavioral health and primary care services. •Develops intervention strategies to meet client short-term and long-term treatment goals. •Serves as client advocate and liaison between public and private agencies. •Promotes interactive communication and develops and refines linkage between MHM programs and community healthcare facilities, social service providers, schools, and other agencies. •Makes appropriate referrals to community agencies and conducts follow up. •Maintains current documentation of available community resources. •Analyzes program data and produces reports as requested. •Adheres to established timelines for documentation and response to clients. •Uses data to assist in improved planning, development and effective use of resources. •Participates in activities to develop and maintain quality assurance indicators that evaluate the effectiveness of the counseling and case management programs. •Assists in the planning, coordinating, and implementing of projects and programs to include but not limited to identifying needs, researching appropriate resources, and managing caseloads. •Implements MHM policies and procedures and as established by the organization. •Maintains administrative aspects of the behavioral health program to include management of supplies, office space, security of records, collection of fees, deposit of funds and management of social workspace at church/community site(s). •Develops and maintains effective working relationships with individuals and families referred for group and case management services. •Develops and maintains a cumulative record for each client receiving case management or group services as required by professional licensure standards and MHM policies and procedures. •Develops and refines linkages between Methodist Healthcare Ministries programs and community health care facilities, social service providers, schools, Wesley Nurses and other agencies. •Participates in community events during business work week, weekends and evenings as needed. •Sustains productivity expectations established by the Behavioral Health Department. •Maintains an active LMSW as designated by the Texas Behavioral Health Executive Council, completing all required continuing education, following all statutes and rules required by the license. •Performs other duties as assigned.
