What you'll do as a Case Manager for Hep C:
• Facilitate referral to care and system navigation for SUD, HCV, HIM, SMI, and social services to address Social Determinants of Health (SDOH)
• Design, develop, implement, and supervise case management, after care, and follow-up policies and procedures for the program
• Provide specialized case management and aftercare services to clients and conduct follow-ups as scheduled
• May supervise clients in the community, carry a caseload of active clients, conduct group and individual counseling sessions, assist clients in achieving adjustments to specific problems or situations of a difficult or complex nature
• Monitor emotional/psychological status of clients and arrange referrals to ancillary services.
• Conduct workshops and seminars for clients and the community and maintain relations with referral sources and Hep C, health, addiction, social, educational, recreational, and vocational organizations for in-treatment and aftercare planning
• Write reports, maintain necessary records, and assist other staff in discharge and aftercare planning.
• Develop and implement procedures to complete clinical records and correspondence while maintaining compliance with confidentiality procedures and applicable state/federal regulations.
• Occasional driving to conduct therapeutic community sessions.
Knowledge, Skills, and Abilities Needed in This Role:
• Ability to establish and maintain effective working relationships with community agencies, organizations, and the public
• Knowledge of Hepatitis C and intervention/treatment strategies.
• Ability to speak and write effectively.

• REQUIRED: Associate Degree
• REQUIRED: Minimum of 2 years' experience in addiction or a related field
• REQUIRED: Valid Driver's License w/ 2 Points or Less
Pay:
• Starting at $19.50/hour
• Commensuration based on experience, education, and certification!

This includes collaborating with the patient to schedule a mutually agreed aftercare plan of care inclusive of PCP, SUD, MAT and other appointments as well as providing patients with community and other resources that will help ensure their success. The Case Manager serves as a patient advocate, coordinating care with internal and external providers, resources and supports.
The Case Manager engages each patient in their aftercare plan and using teach back method, confirms that the patient and their support system understand the plan and the importance of adhering to the plan. The Case Manager serves as the liaison between the patient and all aftercare providers and resources, ensuring the plan has been established, communicated and confirmed prior to discharge. The Case Manager will also assist patients with any identified outside issues, barriers to accessing care or external stressors that need to be resolved, enabling the patient to focus on treatment (examples: coordination with family for childcare, employer relations, legal concerns, etc.)

KEY RESPONSIBILITIES:
Obtains applicable signed Release of Information (ROI) forms for all identified providers and resources in the Continued Care Plan (CCP) and other patient resources/supports (Employer/FMLA, Legal, Payer programs, Referral sources, Peer Support, etc.)
Completes a new patient admission assessment and documents in Avatar within 72 hrs. of admission and obtains patient history, needs, and individual preferences to inform the patient's treatment and aftercare plans. Reviews the completed Biopsychosocial assessment to help identify all life domain need and incorporates into the Continued Care Plan to ensure all identified patient needs are addressed during the stay and or in the patient's continuing care plan. The admission assessment should address housing, employment, legal, financial, family and health concerns as well as relapse prevention and other issues that patient requires assistance with.
Documents at minimum, a weekly progress note that includes patient progress toward discharge, discussions of discharge planning and recommended aftercare plan, actual or potential barriers to the plan and patient's engagement in their aftercare plan. Discharge planning should be documented in Avatar by the second week of stay. Initiates and documents all referrals specified in the CCP including contact information and confirms the aftercare plan addresses follow up for substance use, mental health, MAT, Social Determinants of Health and other identified life domains. The individual CCP should be completed in collaboration with the patient and if possible, their support system.
Participates in Multi Disciplinary Team (MDT) meetings and actively contributes to discussion re: recommendations for each individual's aftercare plan, discharge date, services and resources to be included in the aftercare plan and what is needed from other members of the team to help ensure the patient's success with recovery.
Schedules SUD/MAT appointments within 7 days of discharge and post discharge PCP follow up appointments when possible. Appointments and referrals must be documented in Avatar prior to the patient's scheduled discharge.
Ensures effective and timely communication of relevant information to post-discharge providers prior to discharge to facilitate a safe and thorough discharge plan.
Ensures the continued care/discharge plans is solidified 1 week prior to discharge and that a Transitions of Care meeting has been scheduled at least 7 days prior to discharge with the patient, the patient's support system, and the therapist to review the recommended aftercare plan. Confirms patient preferences and barriers to care have been identified and addressed in the plan. Ensures all dates, times, contact information, phone numbers, address, etc.. are included in the CCP to help ensure patient's adherence to the plan. Assesses patient's comprehension of the aftercare plan through verbal confirmation and verifies patient's clear understanding of post-discharge care instructions through teach back.
Follows referent protocols and provides timely clinical updates and other information as requested (with signed ROI).
Follows Payer protocols and facilitates timely patient-payer phone calls, referrals to Payer Peer Support programs and provides other information as per contractual agreements.
Initiates and manages FMLA and Short-term Disability applications as needed, with patient consent. Coordinates with patients and their employers to facilitate benefit processes when applicable. Documents all activities related to FMLA or STD in the patient's EMR. Case Management is responsible for facilitating a weekly Aftercare/Next Steps group meeting for all new patients utilizing standardized RCA agenda and collaterals. Facilitates at least once weekly Guesting to help prepare patients for their aftercare recovery plan and works closely with outpatient staff and Alumni to inform the patient of RCA's outpatient continuum and benefits of continuity of care. Conducts outreach phone calls to patients who leave treatment early or unplanned without a solid discharge plan to attempt to re-engage patient in their recovery plan. Ensures the safety, stability and well-being of youth and effectively work with and identify services/supports for youth who may be facing a range of co-occurring challenges, including mental health, domestic violence, substance use, economic challenges, lack of support, and other needs
Liaison with vocational and educational community partners to help youth develop career plans and obstacles to employment.
Provide individual support and counseling and assist with crisis management and medication monitoring
Help youth secure public benefits, and develop recovery strategies.
Requirements:
Bachelor's degree in social service- related field required.
1 year of experience in the field working with children, youth or families.
Knowledge and/or experience in trauma-informed care, mental health and substance abuse.
Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record.
Additional Information:
Base Rate: $19.23
Sign-on Bonus: $1,000
Bilingual: An additional $1.50 added to the base rate if Spanish proficient
Schedule: Two shifts 8:30-4:30p; one shift 11-7p; two shifts 1-9p plus on-call overnights, weekends, and holidays in a rotation

What You'll Do At BCCS:
• Supporting families to prevent the use and misuse of drugs and the development of substance use disorders
• Promoting child safety by identifying substance use and misuse in families and connecting families to interventions or treatment programs that will help the families identify, improve, and change unsafe behaviors so children can be safely cared for in the home.
• Reducing the risk of future harm to children by addressing the substance use needs in families.
• Transports clients to appointments in community and BCCS appointments.
• Provides liaison, support and educational services to DFS staff
• Participates in case presentation at weekly meetings
• Participates in client screening process prior to case management.
• Provides outreach activities to the community
• Administers various screening tools to potential clients and explains the nature of the program and the necessary rules.
• Assists clients in obtaining admission, entitlements, and related benefits.
• This is primarily a community-based position, so work will be performed outside of an office setting on a regular and consistent basis up to 100% of the time
Knowledge, Skills and Abilities:
• Knowledge of screening tools and therapy principles

Qualifications for the position are:

- Certified Peer Recovery Specialist (CPRS)
- Certified Associate Addiction Counselor (CAAD)
- Certified Prevention Specialist (CPS), or
- Certified Family Support Peer (CFSP).

• REQUIRED: Valid driver's license with 2 points or less
• REQUIRED: Ability to use BCCS vehicle or use own vehicle, when needed.

Job Description
JOB SUMMARY: Collaborates with members of the healthcare team, community resources, and with the patient/patient family to promote optimal outcomes for the patient across the continuum of care. Maintains a smaller caseload that requires increased attention and intensive care coordination due to complex social dynamics, legal involvement and connection with DSS/DDA. Formulates unique solutions to complex discharge planning issues (thinking outside the box). Assists with navigating through barriers that may be preventing the safe disposition of patients. This position is goal oriented with a high level of accountability correlated to the continuum of care, quality of care and patient satisfaction. Supports the success of the care coordination of this patient population with a high degree of professionalism and expertise.
Qualifications
Education & Training: Maryland Social Work licensure LMSW or LCSW level required or Maryland licensed RN, Bachelors preferred.
Work Orientation & Experience: Minimum of 3 years of relevant hospital case management experience. Acute care social work or nursing experience required. Knowledge of Case Management, Critical Pathways, and Utilization Management preferred. The applicant may require up to three months of detailed job orientation and training to gain specific capabilities to perform essential job functions
