The CaseManager - CT serves clients who are homeless or at risk for homelessness by providing a range of services to reduce the chances of veterans becoming homeless once they leave Valor House. Veterans involved in the Clinical Treatment and Special Needs programs are at increased risk, and are struggling with substance use and/or mental health diagnoses. This position involves both direct and indirect services including goal setting, needs assessments, tracking treatment progress, and connecting clients with a range of resources (e.g., housing, education, employment and medical). The CaseManager - CT assists in other administrative tasks and works in concert with the priorities of the Executive Director, the Senior Management Team, and the Board of Directors. The overall goals of this position are to (a) provide high-quality care for veterans, (b) coordinate services for casemanagement grants and programs, and (c) provide support for the clinical and administrative staff.

Collaborates with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources
Uses the agency electronic health record to enter case-management notes, client information updates, and other relevant client information
Utilization of HIMIS, excel tracking system, and other relevant data management systems
Problem-solving and critical thinking
Mediation/ Conflict Resolution skills/ Crisis Management skills (MI, de-escalation tactics, etc.)

Position Summary
This position provides part-time outreach casemanagement services to unsheltered individuals and/or families in Salt Lake County. Objectives will be to create supportive relationships with homeless individuals through street outreach and provide casemanagement support towards individual stabilization and permanent housing. Additionally, they will work to engage and educate community members on homelessness issues, helping them to gain a better understanding of homelessness issues and empowering them to participate in resolutions.
Essential Duties
Assist clients in developing goals/service plan that will help them overcome homelessness, identify and address barriers to obtaining/maintaining housing. Including but not limited to CE Enrollments, Case plans, CTG, housing waitlist etc.
Maintain a caseload of up to 7 individuals
Identify priority needs, (i.e. Pregnancy, IV/IM substance user, dual diagnosis, and other medical conditions) and coordinate services as needed.
Participate in regularly scheduled clinicalcase reviews in order to ensure appropriate case planning and referral of clients to the most appropriate service provider.
Provide direct, Trauma Informed Care casemanagement services to clients, including crisis intervention, outreach, ongoing assessments (i.e. , SPDAT, Lethality Assessment, Crises response plans etc.), goal setting, creative problem solving, agency and community resource utilization, and network building.
Maintains client records, enters casemanagement notes in the UHMIS system, and collects any statistical data as required, and participate in research and/or special projects as needed, as directed and both timely and consistently.
Assess medical emergencies, and provides immediate and professional response to any emergency following appropriate policies and procedures. (contacting emergency medical, utilize Naloxone kits, LAP assessments, Crisis response planning, Etc.)
Maintain a positive working relationship with community partners, team members, community members, business owners, emergency services, PD, and all other community or government stakeholders.
Assist in resolving conflicts utilizing de-escalation skills and trauma informed practices.
Evaluate own performance to increase effectiveness as an Outreach CaseManager.
Assist clients with moving belongings into housing.
Assist with community education, training, response, and problem solving around homelessness issues in Salt Lake City.
Ability to safely use agency vehicles to provide transportation.
Attend work on a punctual, regular, and predictable basis.
Perform other functions as necessary or assigned.
Attend work as scheduled and arrive at the designated time for a total of 24 hours per week.
Work must be performed on-site due to the need to provide direct client care.

Essential Job Functions

The Housing Focused CaseManager Team Lead will provide trauma-informed, guest-driven casemanagement services to individuals who are experiencing homelessness with the goal of helping them achieve stable housing solutions quickly and efficiently. The Housing Focused CaseManager Team Lead will assist in supervising a small team of CaseManagers through client interactions, administrative needs, and other duties, including but not limited to maintaining professional boundaries, emphasizing teamwork, and creating an environment that fosters growth, all while managing their own case load of clients. The services provided include identifying and engaging with individuals in our Resource Centers and connecting them with resources in the community that will aid in their transition into housing. The Housing Focused CaseManager Team Lead will work within the shelter team to identify common barriers faced by individuals in shelter and help with establishing strategies and procedures for addressing needs on a shelter-wide scale.

Position Summary
The Assessment and Diagnostic Clinician is responsible for completing comprehensive clinical evaluations for clients entering Odyssey House, including adults, youth, parents with children, and clients with serious and persistent mental illness (SPMI). This role conducts diagnostic interviews using DSM-5 criteria, determines ASAM and LOCUS level of care, evaluates substance use and mental health severity, performs suicide and safety risk screening, and produces insurance ready documentation that meets medical necessity, authorization, and regulatory requirements. The clinician participates in insurance peer to peer reviews as needed and works directly with courts and other external partners to provide clinically relevant information in a manner consistent with HIPAA and 42 CFR Part 2.
This position is central to the quality and financial stability of the organization. It requires advanced diagnostic skill, strong clinical judgment, high attention to detail, and exceptional written communication in a high volume admissions environment.
Essential Duties and Responsibilities
Comprehensive Clinical Assessment
Complete biopsychosocial and diagnostic assessments for all assigned admissions across the lifespan, including adults, youth, parents with children, and SPMI populations.
Formulate DSM-5 diagnoses and document clear "as evidenced by" support aligned with payer and audit standards.
Evaluate co-occurring disorders, trauma history, criminogenic needs, functional impairments, social determinants, and client strengths.
Administer and interpret standardized tools including OQ-45.2, PHQ-9, GAD-7, ACE, C-SSRS, CANS, LSI:R-SV and other funder or program required assessments.
2. ASAM and LOCUS Level of Care Determination
Complete ASAM assessments across all six dimensions using ASAM 3rd Edition criteria for substance use and co-occurring conditions.
Complete LOCUS assessments when required to determine mental health level of care and document level of care recommendations.
Recommend level of care with written justification tied to severity, function, risk, recovery environment, and payer criteria.
Consult with admissions leadership and medical or psychiatric providers when diagnostic or level of care questions arise.
3. Suicide Risk and Safety Screening
Administer C-SSRS Screen at admission and initiate C-SSRS Since Last Visit versions as indicated.
Develop Stanley Brown Safety Plans or Safety Plan Cards for clients with elevated suicide risk and ensure these are shared with receiving programs.
Communicate high risk findings promptly to program leadership and participate in safety planning and disposition decisions.
4. Medical Necessity, Insurance Documentation, and Peer to Peer Reviews
Produce timely, accurate documentation to support authorizations, continued stay reviews, and billing for Medicaid, commercial insurance, and county funding.
Clearly document risk factors, functional impairment, and clinical rationales that support level of care and medical necessity.
Participate in insurance peer to peer reviews as needed to clarify diagnostic impressions, risk, and clinical justification for requested levels of care.
Collaborate with utilization management and billing staff to address payer questions and prevent denials.
5. Collaboration, Care Coordination, and External Partners
Communicate diagnostic impressions, ASAM and LOCUS findings, and risk information to receiving clinical teams in a clear, usable format.
Provide consultation to admissions, residential, and outpatient staff on assessment and diagnostic questions.
Work directly with courts, probation and parole, child welfare, and other external partners to supply clinically relevant information in a manner that is compliant with HIPAA and 42 CFR Part 2, using appropriate releases of information and redisclosure limitations.

6. Compliance, Quality, and Data Integrity

Maintain a minimum flexible caseload of 24-36 clients based on facility needs (adjusted for PT staff).
Engage in 75-80% of hours being face-to-face client interactions, including new starts, family sessions, groups (25% of hours), and overseeing treatment on the floor with BHTs.
Maintain comprehensive written documentation at the time of service such as progress and group therapy notes, ensuring high client satisfaction, and managing potential grievances.
Be prepared to explain the clinical rationale verbally, using clear and concise language to support the utilization management of each client and address any questions or concerns the insurance provider may have.
Assume a supervisory role in specific situations, such as intern supervision and BHT-run groups.
Create 3 SMART goals each quarter with the Program Director (APD) to improve your skills as a clinician.
Foster a positive work environment by actively supporting teamwork, encouraging open communication, and modeling a collaborative and respectful approach to interactions with colleagues and team members.
